    Provision of information and health care in a maternity ward: More effective use of corridor notice boards

    病棟における情報提供の一つとして掲示板は手軽に利用されるが,これに関する研究はないといっても過言ではない。しかし,日本医療機能評価機構(Japan Council for Quality Health Care)においては自己評価票の項目として「掲示物の配慮」について評価するようになっている。そこで,O大学附属病院産科病棟の廊下にある掲示板を使用し,掲示板周辺の色彩の視感測定と照度測定を実施し後,掲示物の内容の整理をし,その美観について整理前後の評価をSD法(Semantic Diferenial Method)で評価しその結果, 整理後は好転していた。さらに,掲示物への意見の調査,希望する掲示内容に関する調査なども実施した。これらのことから掲示板の有効活用の仕方と情報提供の内容の選別の2点が明確化した。それぞれの留意点は以下のとおりである。1.掲示物の重要度と緊急性に応じた貼り方をする。2.文字や絵の大きさにより,掲示板の有効活用をする。1.産科領域の患者教育および健康教育などの提供の必要性がある。2.掲示物の内容は患者本人に必要なことか,家族にも必要なことなのか,対象者の明確化をして貼付する必要がある。3.一般ニュースの提供や絵や写真などの癒しの必要性もある。Although bulletin boards in hospitals are considered a readily accessible means of information sharing, there are virtually no studies available on this subject. However, as a self-assessment survey of the Japan Council for Quality Health Care includes a rating of "attention given to posted materials" as one of its items, we thus conducted a study focused on the bulletin board situated in the corridor of O University Hospital's maternity ward. After measuring color visibility and lighting intensity around the board, we reorganized the layout of posted materials; after which we evaluated the appearance of the board using the Semantic Differential Method. The results were positive; and in addition, we conducted other surveys intended to obtain comments and requests. As a result of such efforts, we came up with two criteria to consider when designing a bulletin board; namely, on how to use bulletin boards effectively and on classifying the type of announcement. 1. Arrange display materials according to importance and urgency levels 2. Use font sizes and drawings as eye-catching means 1. There is a need to provide patient education and health information related to the field of obstetrics. 2. There is a need to clearly identify the targets, i.e. whether the posted materials are intended for the patients and/or for family members. 3. There is also a need to provide general news or to post drawings and pictures for the purpose of comforting patients

    Depressive symptoms related to low fractional anisotropy of white matter underlying the right ventral anterior cingulate in older adults with atherosclerotic vascular disease

    We sought to characterize the relationship between integrity of the white matter underlying the ventral anterior cingulate (vAC) and depressive symptoms in older adults with atherosclerotic vascular disease (AVD), a condition associated with preferential degeneration of the white matter. The ventral anterior cingulate was defined as including white matter underlying ventral Brodmann Area 24 and Brodmann Area 25, corresponding with the subcallosal and subgenual cingulate respectively. This region of interest was chosen based on the preponderance of evidence that the white matter in the region plays a critical role in the manifestation of depressive symptoms. Participants had current unequivocal diagnoses of AVD and were between 55 and 90 years old. Fractional anisotropy (FA) was used as an index of white matter integrity and organization. Whole-brain mean diffusivity (MD) was used as an index of global white matter lesion burden. Depressive symptoms were measured using the Symptom Checklist-90-Revised (SCL-90-R) Depression Scale. Depressive symptoms were significantly related to low FA in the right vAC (r=-.356, DF=30, p=.045) but not the left vAC (r=.024, DF=30, p=.896) after controlling for total brain MD (a statistical control for global white matter lesion burden). Further, depressive symptoms were significantly related to low FA in the right vAC (r=-0.361, DF=31, p=.039), but not the left vAC (r=.259, DF=31, p=.145) when controlled for the contralateral vAC FA. The correlation coefficients for this follow-up analysis were found to be significantly different between left and right vAC (Z=2.310, p=.021).Poor white matter health in the vAC may be a biological mechanism for depressive symptoms in older adults with vascular disease. Further studies may corroborate that the right vAC plays a unique role in depressive symptom manifestation in cases where the white matter is preferentially affected, as is the case in AVD.
