    Functional Locomotor Consequences of Uneven Forefeet for Trot Symmetry in Individual Riding Horses

    ABSTRACT: Left-right symmetrical distal limb conformation can be an important prerequisite for a successful performance, and it is often hypothesized that asymmetric or uneven feet are important enhancing factors for the development of lameness. On a population level, it has been demonstrated that uneven footed horses are retiring earlier from elite level competition, but the biomechanical consequences are not yet known. The objectives of this study were to compare the functional locomotor asymmetries of horses with uneven to those with even feet. Hoof kinetics and distal limb kinematics were collected from horses (n = 34) at trot. Dorsal hoof wall angle was used to classify horses as even or uneven (1.5° difference between forefeet respectively) and individual feet as flat (55°). Functional kinetic parameters were compared between even and uneven forefeet using MANOVA followed by ANOVA. The relative influences of differences in hoof angle between the forefeet and of absolute hoof angle on functional parameters were analysed using multiple regression analysis (P<0.05). In horses with uneven feet, the side with the flatter foot showed a significantly larger maximal horizontal braking and vertical ground reaction force, a larger vertical fetlock displacement and a suppler fetlock spring. The foot with a steeper hoof angle was linearly correlated with an earlier braking-propulsion transition. The conformational differences between both forefeet were more important for loading characteristics than the individual foot conformation of each individual horse. The differences in vertical force and braking force between uneven forefeet could imply either an asymmetrical loading pattern without a pathological component or a subclinical lameness as a result of a pathological development in the steeper foot

    Prevalence of Anxiety and Depression among Outpatients with Type 2 Diabetes in the Mexican Population

    Depression and anxiety are common in diabetic patients; however, in recent years the frequency of these symptoms has markedly increased worldwide. Therefore, it is necessary to establish the frequency and factors associated with depression and anxiety, since they can be responsible for premature morbidity, mortality, risk of developing comorbidities, complications, suffering of patients, as well as escalation of costs. We studied the frequency of depression and anxiety in Mexican outpatients with type 2 diabetes and identified the risk factors for depression and anxiety.We performed a study in 820 patients with type 2 diabetes. The prevalence of depression and anxiety was estimated using the Hamilton Depression Rating Scale and the Hamilton Anxiety Rating Scale, respectively. We calculated the proportions for depression and anxiety and, after adjusting for confounding variables, we performed multivariate analysis using multiple logistic regressions to evaluate the combined effect of the various factors associated with anxiety and depression among persons with type 2 diabetes. The rates for depression and anxiety were 48.27% (95% CI: 44.48–52.06) and 55.10% (95% CI: 51.44–58.93), respectively. Occupation and complications in diabetes were the factors associated with anxiety, whereas glucose level and complications in diabetes were associated with depression. Complications in diabetes was a factor common to depression and anxiety (p<0.0001; OR 1.79, 95% CI 1.29–2.4).Our findings demonstrate that a large proportion of diabetic patients present depression and/or anxiety. We also identified a significant association between complications in diabetes with depression and anxiety. Interventions are necessary to hinder the appearance of complications in diabetes and in consequence prevent depression and anxiety

    A horse’s locomotor signature: COP path determined by the individual limb

    Introduction Ground reaction forces in sound horses with asymmetric hooves show systematic differences in the horizontal braking force and relative timing of break-over. The Center Of Pressure (COP) path quantifies the dynamic load distribution under the hoof in a moving horse. The objective was to test whether anatomical asymmetry, quantified by the difference in dorsal wall angle between the left and right forelimbs, correlates with asymmetry in the COP path between these limbs. In addition, repeatability of the COP path was investigated. Methods A larger group (n = 31) visually sound horses with various degree of dorsal hoof wall asymmetry trotted three times over a pressure mat. COP path was determined in a hoof-bound coordinate system. A relationship between correlations between left and right COP paths and degree of asymmetry was investigated. Results Using a hoof-bound coordinate system made the COP path highly repeatable and unique for each limb. The craniocaudal patterns are usually highly correlated between left and right, but the mediolateral patterns are not. Some patterns were found between COP path and dorsal wall angle but asymmetry in dorsal wall angle did not necessarily result in asymmetry in COP path and the same could be stated for symmetry. Conclusion This method is a highly sensitive method to quantify the net result of the interaction between all of the forces and torques that occur in the limb and its inertial properties. We argue that changes in motor control, muscle force, inertial properties, kinematics and kinetics can potentially be picked up at an early stage using this method and could therefore be used as an early detection method for changes in the musculoskeletal apparatus

    Expression of inflammasome proteins and inflammasome activation occurs in human, but not in murine keratinocytes

    Inflammasomes are multimeric protein complexes that assemble upon sensing of a variety of stress factors. Their formation results in caspase-1-mediated activation and secretion of the pro-inflammatory cytokines pro-interleukin(IL)-1β and -18, which induce an inflammatory response. Inflammation is supported by a lytic form of cell death, termed pyroptosis. Innate immune cells, such as macrophages or dendritic cells, express and activate inflammasomes. However, it has also been demonstrated that human primary keratinocytes activate different types of inflammasomes in vitro, for example, upon UVB irradiation or viral infection. Keratinocytes are the main cell type of the epidermis, the outermost layer of the body, and form a protective barrier consisting of a stratified multi-layered epithelium. In human, gain-of-function mutations of the NLRP1 gene cause syndromes mediated by inflammasome activation in keratinocytes that are characterised by skin inflammation and skin cancer susceptibility. Here we demonstrate that murine keratinocytes do not activate inflammasomes in response to stimuli, which induce IL-1β and -18 secretion by human keratinocytes. Whereas murine keratinocytes produced caspase-1 and proIL-18, expression of the inflammasome proteins Nlrp1, Nlrp3, Aim2, Asc, and proIL-1β was, compared to human keratinocytes or murine dendritic cells, very low or even undetectable. Priming of murine keratinocytes with cytokines commonly used for induction of proIL-1β and inflammasome protein expression did not rescue inflammasome activation. Nevertheless, UVB-induced inflammation and neutrophil recruitment in murine skin was dependent on IL-1β and caspase-1. However, also under these conditions, we did not detect expression of proIL-1β by keratinocytes in murine skin, but by immune cells. These results demonstrate a higher immunological competence of human compared to murine keratinocytes, which is reflected by stress-induced IL-1β secretion that is mediated by inflammasomes. Therefore, keratinocytes in human skin can exert immune functions, which are carried out by professional immune cells in murine skin

    Pesquisa descritiva, exploratória e retrospectiva, com abordagem quantitativa, realizada em uma comunidade carente de São Paulo com o objetivo de verificar se a manutenção do aleitamento exclusivo (AE) nos primeiros seis meses é influenciada pelas variáveis: contato precoce na primeira hora após o nascimento, permanência em alojamento conjunto, tipo de parto e tipo de hospital. Os dados foram coletados de 75 prontuários e analisados com a metodologia de equações de estimação generalizada. Os resultados mostraram que as variáveis alojamento conjunto, tipo de hospital e tipo de parto interferiram na manutenção do AE, o mesmo não ocorrendo com o contato precoce. Concluiu-se que os índices de AE foram maiores nos casos em que mãe e bebê permaneceram constantemente juntos após o parto, em hospitais amigos da criança e após partos normais. Constatou-se que a assistência recebida pela mulher durante o processo de parto e nascimento influencia de forma direta a amamentação.<br>Se trata de una investigación descriptiva, exploratoria y retrospectiva, con abordaje cuantitativo, realizada en una comunidad carente de San Pablo, con el objetivo de verificar si la manutención del amamantar exclusivo (AE) en los primeros seis meses fue influenciada por las variables: contacto precoz en la primera hora después del nacimiento, permanencia en el mismo alojamiento, tipo de parto y tipo de hospital. Los datos fueron recolectados de 75 registros y analizados con la metodología de ecuaciones de cálculo generalizado. Los resultados mostraron que las variables alojamiento conjunto, tipo de hospital y tipo de parto interfirieron en la manutención del AE, lo mismo no ocurrió con el contacto precoz. Se concluye que los índices de AE fueron mayores en los casos en que la madre y el bebé permanecieron constantemente juntos después del parto, en hospitales amigos del niño y después de partos normales. Se constató que la asistencia recibida por la mujer durante el proceso de parto y nacimiento influye de forma directa en el amamantar.<br>This is a descriptive, exploratory and retrospective study, with a quantitative approach, performed in a low-income community in São Paulo, with the purpose to identify whether the maintenance of exclusive breastfeeding (EBF) in the first six months is influenced by the following variables: early contact in the first hour after birth, permanence in joint lodging, type of delivery and type of hospital. Data were collected from 75 medical records and analyzed with the methodology of generalized estimate equations. The results showed that the variables joint lodging, type of hospital and type of delivery interfered in the maintenance of EBF; however, that was not the case with early contact. It was concluded that the EBF indexes were higher in cases where the mother and the baby remained together after the birth, in baby-friendly hospitals and after normal deliveries.
