Intestinal transplantation under tacrolimus monotherapy after perioperative lymphoid depletion with rabbit anti-thymocyte globulin (thymoglobulin®)
Modifications in the timing and dosage of immunosuppression can ameliorate the morbidity and mortality that has prevented widespread use of intestinal transplantation (ITx) in children. Thirty-six patients receiving ITx, aged 5 months to 20 years were given 2-3 mg(kg of rabbit anti-thymocyte globulin (rATG, thymoglobulin®) just before ITx, and 2-3 mg(kg post-operatively (total 5 mg(kg). Twice daily doses of tacrolimus (TAC) were begun enterally within 24 h after graft reperfusion with reduction of dose quantity or frequency after 3 months. Prednisone or other agents were given to treat breakthrough rejection. After 8-28 months follow-up (mean 15.8 ± 5.3), 1- and 2-year patient and graft survival is 100% and 94%, respectively. Despite a 44% incidence of acute rejection in the first month, 16 of the 34 (47%) survivors are on TAC (n = 14) or sirolimus (n = 2) monotherapy; 15 receive TAC plus low dose prednisone; one each receive TAC plus sirolimus, TAC plus azathioprine and TAC plus sirolimus and prednisone. There was a low incidence of immunosuppression-related complications. This strategy of immunosuppression minimized maintenance TAC exposure, facilitated the long-term control of rejection, decreased the incidence of opportunistic infections, and resulted in a high rate of patient and graft survival. The A-X infrared bands of Aluminum Oxide in stars: search and new detections
We describe a search for the A-X infrared bands of AlO with a view to better
understand the characteristics of this radical. These bands are infrequently
encountered in astronomical sources but surprisingly were very prominent in the
spectra of two well-known, nova-like variables (V838 Mon and V4332 Sgr) thereby
motivating us to explore the physical conditions necessary for their
excitation. In this study, we present the detection of A-X bands in the spectra
of 13 out of 17 stars, selected on the basis of their J-K colors as potential
candidates for detection of these bands. The majority of the AlO detections are
in AGB stars viz. 9 OH/IR stars, 2 Mira variables and 2 bright infrared
sources. Our study shows that the A-X bands are fairly prevalent in sources
with low temperature and O-rich environments. Interesting variation in strength
of the AlO bands in one of the sources (IRAS 18530+0817) is reported and the
cause for this is examined. Possible applications of the present study are
discussed in terms of the role of AlO in alumina dust formation, the scope for
estimating the radioactive Al content in AGB stars from the A-X bands,
and providing possible targets for further mm/radio studies of AlO which has

Pancreatic transplantation at the University of Pittsburgh.
Campath-1H preconditioning with tacrolimus monotherapy is an effective immunosuppressive regimen for pancreas transplantation, with acceptable patient and graft survival rates early after transplantation. Rejection rates are low under this protocol if the tacrolimus level is kept consistently >10 ng/ml. This immunosuppressive protocol, combined with recent technical refinements, has resulted in lower rates of thrombosis and overall complications. Pancreatic transplantation en-bloc with visceral grafts has the following unique features: Diabetes is a rare indication, and HLA matching is not required. The gland is immunologically protected by the simultaneously transplanted visceral organs. Disease gravity, surgical complexity and gut alloimmunity influence the overall pancreatic allograft survival. The current UNOS listing criteria and data registry should be modified for obvious logistic and scientific reasons
The Thermal Design, Characterization, and Performance of the SPIDER Long-Duration Balloon Cryostat
We describe the SPIDER flight cryostat, which is designed to cool six
millimeter-wavelength telescopes during an Antarctic long-duration balloon
flight. The cryostat, one of the largest to have flown on a stratospheric
payload, uses liquid helium-4 to deliver cooling power to stages at 4.2 and 1.6
K. Stainless steel capillaries facilitate a high flow impedance connection
between the main liquid helium tank and a smaller superfluid tank, allowing the
latter to operate at 1.6 K as long as there is liquid in the 4.2 K main tank.
Each telescope houses a closed cycle helium-3 adsorption refrigerator that
further cools the focal planes down to 300 mK. Liquid helium vapor from the
main tank is routed through heat exchangers that cool radiation shields,
providing negative thermal feedback. The system performed successfully during a
17 day flight in the 2014-2015 Antarctic summer. The cryostat had a total hold
time of 16.8 days, with 15.9 days occurring during flight.Comment: 15 pgs, 17 fig
Under-Sodium Viewing: A Review of Ultrasonic Imaging Technology for Liquid Metal Fast Reactors
This current report is a summary of information obtained in the "Information Capture" task of the U.S. DOE-funded "Under Sodium Viewing (USV) Project." The goal of the multi-year USV project is to design, build, and demonstrate a state-of-the-art prototype ultrasonic viewing system tailored for periodic reactor core in-service monitoring and maintenance inspections. The study seeks to optimize system parameters, improve performance, and re-establish this key technology area which will be required to support any new U.S. liquid-metal cooled fast reactors
Distances and ages of globular clusters using Hipparcos parallaxes of local subdwarfs
We discuss the impact of Population II and Globular Cluster (GCs) stars on
the derivation of the age of the Universe, and on the study of the formation
and early evolution of galaxies, our own in particular. The long-standing
problem of the actual distance scale to Population II stars and GCs is
addressed, and a variety of different methods commonly used to derive distances
to Population II stars are briefly reviewed. Emphasis is given to the
discussion of distances and ages for GCs derived using Hipparcos parallaxes of
local subdwarfs. Results obtained by different authors are slightly different,
depending on different assumptions about metallicity scale, reddenings, and
corrections for undetected binaries. These and other uncertainties present in
the method are discussed. Finally, we outline progress expected in the near

A prospective multi-center cohort study of acute non-displaced fractures of the scaphoid: operative versus non-operative treatment [NCT00205985]
BACKGROUND: Acute scaphoid fractures are common in active adults and do lead to reasonable time lost to work. One important goal of treatment is early return to work or sport. On this background, the adequate treatment of non-displaced acute scaphoid fractures is still under discussion. The aim of this study is to compare time to return to previous activity level comparing surgical versus non-surgical treatment of non-displaced acute scaphoid fractures. METHODS/DESIGN: The study is designed as a non-randomized multiple center cohort study including 12 sites in Germany and Austria. The inclusion period is planned to be 12 months with a follow up of 6 months. Allocation to operative or non-operative treatment is choosen by the patient together with his treating surgeon. The primary outcome is time to return to previous activity level adapted for loading of the wrist in daily life as measured by a newly developed questionnaire (PLDL-wrist). Factors identified a priori to be associated with the outcome, e.g., poverty status, age, education, smoking status, gender, and occupation, are measured to ensure adequate control for their potential confounding effects. DISCUSSION: The rationale and the design of a multiple center cohort study are presented. As it is not considered feasible to randomize patients in this study, potential confounding effects need to be controlled adequately
