    Comparative measurement of concentration of lithium, circulating immune complexes and complement activity in blood and lymph in experimental anaphylactic shock and Arthus phenomenon

    Aim. Comparative measurement of concentration of lithium, circulating immune complexes and complement activity in animals’ blood and lymph in experimental anaphylactic shock and Arthus phenomenon. Methods. Experiments were conducted on 27 chinchilla rabbits in double series. The studied values of lithium, concentration of circulating immune complexes and complement activity in intact animals were used as controls. Results. On day 7 of sensibilisation before anaphylactic shock the titer of complement components in the blood decreased by 1.4 times (p <0.001) compared to parameters of intact animals, at the stage of anaphylactic shock sharp decrease of its concentration to undetermined values was observed (p <0.001). The blood concentration of circulating immune complexes increased by 4.4 times, and at the stage of anaphylactic shock (day 21) its increase by 7.1 times was observed (p <0.001). Lithium concentration decreased to 0.03 mmol/L at the stage of anaphylactic shock (p <0.001). In Arthus phenomenon at the stage of sensibilisation (day 5) the blood complement titer decreased by 1.1 times (p <0.001), during resolution period - by 10.1 times (p <0.001). The blood concentration of circulating immune complexes increased by 4.5 times (p <0.001) at the stage of sensibilisation (day 5), and at the stage of Arthus phenomenon - by 11.8 times compared to intact animals (p <0.001). Conclusion. The concentration of circulating immune complexes increases in anaphylactic and immunocomplex reactions, besides, more significantly in Arthus phenomenon (23 r.u. in anaphylactic shock versus 38.1 r.u. in Arthus phenomenon); lithium concentration in anaphylactic shock decreases by 5.2 times which is indicative of inhibitory action of this microelement in allergic reactions

    Immediate and late follow-up results of auto- and alloplastic operations in inguinal hernias

    Objective. To conduct a scientific investigation for comparative investigation of results of auto- and allohernioplastic operations. Materials and methods. The investigation was based on analysis of postoperative results in 400 patients, ageing 16 - 85 yrs old, to whom in Scientific Surgical Centre named after Acad. M. A. Topchibashev and the City of Shirvan Central Hospital in 2009-2019 yrs period surgical treatment for inguinal hernia was conducted. Depending on the procedure of hernioplasty applied, the patients were divided into two Groups. In a control one (n=200) the operations were performed, using open autohernioplasty only. In the main Group (n=200) open procedure and laparoscopic allohernioplasty, using different synthetic nets, were applied. Results. Comparative analysis of intraoperative, postoperative immediate and late follow-up results in the patients after auto- and allohernioplasty was done. In accordance to criteria of the intraoperative and immediate postoperative morbidity rate the statistically meaningful differences between two groups of patients were absent. Good late results of allohernioplasty were noted in (71.8 ± 3.41)%, satisfactory - in (17.2 ± 2.86)%, poor - in (11 ± 2.36)% patients (p<0.05); allohernioplasties - accordingly, in (80.2 ± 2.91)%, (16 ± 2.68)% and (3.8 ± 1.39)% patients (p<0.05). Comparative analysis done have shown the statistically much better results of allohernioplasty. Recurrences after allohernioplasty were observed in 3 times less frequently - 3.8%, and after autohernioplasty this index have constituted 11% (p < 0.05). Сomparative analysis of late follow-up results of various autohernioplastic methods have shown good results of Postemskyi method in (76.7 ± 7.72)% patients, and of Postempskyi method in our modification - in (88.9 ± 7.41)% patients. Good results of autoplasty in other methods were noted merely in equal quantity of the patients - in up to 70%, and statistically significant difference was absent. Conclusion. Basing on own results obtained, we recommend to perform autohernioplasty of the inguinal channel anterior wall in accordance to Girard-Sposokukotskyi method with Kimbarovskyi suture for oblique inguinal hernias, while in direct inguinal hernias, for strengthening of posterior wall, - Postempskyi method in our modification. In large and recurrent hernias in patients of middle and senile age it is expedient to apply a Lichtenstein plasty in our modification. Total extraperitoneal or transabdominal preperitoneal plasty is recommended if technical installments needed and the physician’n laparoscopic skills are available

    Obesity in the elderly: peculiarities of treatment in outpatient practice

    Today there is a worldwide trend of population aging, in addition, the proportion of older people with obesity is increasing. In order to adequately manage these patients in the outpatient setting, it is critical to understand the dynamic relationship between body weight, chronic disease prevalence, development of functional disability, life expectancy, and health care costs. While the obesity epidemic has affected people of all age groups, empirical knowledge about the management of obese older people remains largely scarce. Currently, there is no single therapeutic approach to this problem. Weight loss should be treated with caution in people over 60 years of age. The risk of sarcopenia, malnutrition, bone loss, increased risk of falls and injury must be considered, which can lead to an increase in the number of people with disabilities and the associated costs of medical and social care. The analytical review focuses on the relationship between two of the greatest epidemiological trends: aging and obesity. This paper highlights the features of the pathophysiology of obesity in the elderly, the phenomenon of the «obesity paradox». Also from the perspective of evidence-based medicine approaches to the treatment of obesity in the elderly, including surgical interventions aimed at reducing body weight, are analyzed

The work at the military enterprises is characterized by storage and disposal of chemical weapons and the presence of a number of harmful factors. The effect of carcinogens and reproductive hazards in the operation environment on the hormonal system changes the levels of hormones responsible for the reproductive function of the body. It is known that the reduction in the concentration of testosterone leads to male infertility, and its increase with age can trigger the development of prostate cancer. Testosterone indicators in our study were distributed subject to the contingent, age, length of service and a type of troops. Determination of hormone was conducted by enzyme-linked immunosorbent assay (ELISA) of blood. The study revealed hormonal disorders in all contingents examined. A high-risk group involves retired and former military servicemen (high levels of testosterone were found in 52.1% of the surveyed of this contingent), persons over 70 years old (high levels of testosterone were found in 61.5% of the surveyed in this age group), the workers with length of service of 30-39 years (high levels of testosterone were found in 55.2% of the surveyed with this length of service). Low levels of testosterone were found in private soldiers (3.6% of the surveyed of this contingent), in persons aged 18-29 years (3.3% of the surveyed in this age group), and in those with length of service of 1-2 years (3.6% of the surveyed with this length of service)

    The efficiency of tear substitute therapy at dysfunction tear basic secretion

    Purpose: to evaluate the efficiency and tolerance of drops replacing tear with hyaluronic acid 0.15% (Hylabak, thea, France) for the treatment of tear film diseases, in particular blepharoconjunctival form of dry eye syndrome.Methods: 32 patients (range 38 to 72 years) with blepharoconjunctival form of dry eye syndrome were treated lubricant Hylabak 2-4 times a day during 7-30 days.Results: After treatment was detected the elimination of complaints in 30 of 32 people, increasing the stability of the tear film (increasing values of the samples Norn test from 7.3 ±0.5 to 10.1±0.6 seconds), as well as normalization of Schirmer test (from 8.8±0.4 to 15.2±0.5 mm).Conclusion: the study was showed high efficacy of lubricant Hylabak and subjective tolerability at blepharoconjunctival form of dry eye syndrome
