In this work, we present a generalization to varieties and sheaves of the fundamental ideal of the Witt ring of a field by defining a sheaf of fundamental ideals $\tilde{I}$ and a sheaf of Witt rings $\tilde{W}$ in the obvious way. The Milnor conjecture then relates the associated graded of $\tilde{W}$ to Milnor K-theory and so allows the classical invariants of a bilinear space over a field to be extended to our setting using \'etale cohomology. Osteoporosis: Prevention, Screening, and Treatment in the Primary Care Setting

Osteoporosis, with its related fragility fractures, is largely a preventable disorder, not an inevitable part of aging. Through effective education, screening, and treatement in the primary care setting, it may be possible to curb the impending rise of osteoporosis and costly fractures as the population ages. A time-conserving patient education pamphlet on osteoporosis was developed for a family medicine practice in central Vermont. Ontogenetic changes in cutaneous and branchial ionocytes and morphology in yellowfin tuna (Thunnus albacares) larvae.

The development of osmoregulatory and gas exchange organs was studied in larval yellowfin tuna (Thunnus albacares) from 2 to 25 days post-hatching (2.9-24.5 mm standard length, SL). Cutaneous and branchial ionocytes were identified using Na+/K+-ATPase immunostaining and scanning electron microscopy. Cutaneous ionocyte abundance significantly increased with SL, but a reduction in ionocyte size and density resulted in a significant decrease in relative ionocyte area. Cutaneous ionocytes in preflexion larvae had a wide apical opening with extended microvilli; however, microvilli retracted into an apical pit from flexion onward. Lamellae in the gill and pseudobranch were first detected ~ 3.3 mm SL. Ionocytes were always present on the gill arch, first appeared in the filaments and lamellae of the pseudobranch at 3.4 mm SL, and later in gill filaments at 4.2 mm SL, but were never observed in the gill lamellae. Unlike the cutaneous ionocytes, gill and pseudobranch ionocytes had a wide apical opening with extended microvilli throughout larval development. The interlamellar fusion, a specialized gill structure binding the lamellae of ram-ventilating fish, began forming by ~ 24.5 mm SL and contained ionocytes, a localization never before reported. Ionocytes were retained on the lamellar fusions and also found on the filament fusions of larger sub-adult yellowfin tuna; however, sub-adult gill ionocytes had apical pits. These results indicate a shift in gas exchange and NaCl secretion from the skin to branchial organs around the flexion stage, and reveal novel aspects of ionocyte localization and morphology in ram-ventilating fishes

The Discourse of Management and the Management of Discourse

Discourse is a pervasive tool of management; one might even say that discourse is what managers do. A widespread assumption among managers is that discourse is not only a pervasive tool, but an effective one for precise communication of information, for making decisions, and for enlisting action, essentially a transmission tool. This paper maintains that the transmission view is a limited conception of language use, one which leads to a faulty conception of what managers do. It ignores the need for an ethics of communication and misjudges the creative aspects of language use. Management discourse is a far more complex and fluid phenomenon, one requiring not just effective use, but management itself. In other words consideration of the discourse of management leads us to the need for the management of discourse.

Use of Standardized Assessments and Online Resources in Stroke Rehabilitation

Background: The extent to which movement-related standardized assessments and online resources are used in stroke rehabilitation is unclear in the United States. Method: The researchers used a cross-sectional descriptive survey that examined (a) therapists use of movement-related standardized assessments, (b) factors influencing learning of new assessments, and (c) use of frequency of online resources by occupational therapists and physical therapists in the United States. Results: Of 151 respondents (46.4% occupational therapists, 53.6% physical therapists), the most frequently used movement-related assessments by occupational and physical therapists were the Berg and Fugl-Meyer Assessment, respectively. More physical therapists use motor-related standardized assessments regularly than occupational therapists, and physical therapists showed more consensus among standardized assessments. Both professions cited quality of patient care for motivating them to integrate outcome measures into practice. Most therapists in stroke rehabilitation used online resources to access movement-related standardized assessment content at least 25% of the time. The Rehabilitation Measures Database was the most frequently used website. Conclusion: Both occupational and physical therapists use online resources for movement-related standardized assessments on a regular basis. However, occupational therapists do not use standardized assessments as frequently as physical therapists. A systematic study of factors that impact the integration of standardized assessments is needed to further identify barriers and inform clinical practice change
