    Integrated Information System for Sustainable Urban Regeneration

    Information systems are widely used in urban planning process for communication between different side actors. However, most of them have been implemented without providing possibilities for decision makers to participate together with urban planners in the process. This research aims to outline a framework where an interactive model for decision making plays a key role in creating a collaborative environment. The proposal regards the information representation as the main instrument for encouraging a constructive dialog between different actors. The focus is on the relationship between three elements of information representation: level of detail, type of visualization and interaction. Combining these elements, information can be provided in a dynamic way enabling more effective exploration and understanding. The proposed strategy implements a digital model that operates on different scales and levels in order to support the key stages of the planning process for sustainable urban regeneration in Bulgaria. Positional approach is used to define the functionality and decision making operation for the selected process. As a result research ideas about the use of the digital model are presented

    Functional analysis of the Sec62 C-terminal domain in membrane protein biogenesis

    About 30 % of proteome in yeast are targeted to the endoplasmic reticulum (ER) membrane either by the signal recognition particle (SRP)-dependent pathway or the SRP-independent pathway. In the SRP-independent pathway, an essential protein Sec62p cooperates with Sec61p, a main protein conducting channel for the ER targeted membrane and secretory proteins. However, the molecular mechanism of Sec62p in this process has not been elucidated in detail. The cytosolic C-terminal domain of Sec62p has been proposed as an acceptor site for N-terminal signal sequences of secretory proteins. To further study the role of the C-terminus of Sec62p in membrane protein biogenesis, C-terminal mutants of Sec62p were prepared and the membrane insertion of model membrane proteins was examined. We found that P219A mutation reduced the C-terminal translocation of model membrane proteins, suggesting that the C-terminus of Sec62p may function on the insertion of membrane proteins into the ER membrane.     Effect of Aroma Hand Massage on Anxiety and Immune Function in Patients with Gynecology Surgery under Local Anesthesia

    PURPOSE: The purpose of this study was to examine the effects of aroma hand massage on anxiety and immune function in patients who had gynecology surgery under local anesthesia. METHODS: The research design was a nonequivalent control group with pre- and posttest design. Data were collected from June 5 to October 6, 2010. Participants included 20 patients in the aroma hand massage group, 20 patients in a hand massage group, and 20 in a control group. As an experimental treatment, hand massage was carried out following the hand massage protocol. Measures consisted of the State Trait Anxiety, Numeric Rating Scale (NRS) for anxiety, vital signs (systolic and diastolic blood pressure, pulse rate), and salivary cortisol for anxiety, and immunoglobulin A for immune function. RESULTS: Aroma hand massage and hand massage group showed lower levels in NRS for anxiety, systolic and diastolic blood pressure, and pulse rate (p<.001) compared to controls. No group differences were found for state anxiety, salivary cortisol and immunoglobulin A. CONCLUSION: The results indicate that aroma hand massage and hand massage are effective in reducing anxiety and can be complementary alternative interventions for women having gynecology surgery under local anesthesia

    Neck control after definitive radiochemotherapy without planned neck dissection in node-positive head and neck cancers

    <p>Abstract</p> <p>Background</p> <p>The purpose of this study was to evaluate neck control outcomes after definitive radiochemotherapy without planned neck dissection in node-positive head and neck cancer.</p> <p>Methods</p> <p>We retrospectively reviewed medical records of fifty patients with node-positive head and neck cancer who received definitive radiochemotherapy. Twelve patients subsequently underwent neck dissection for suspicious recurrent or persistent disease. A median dose of 70 Gy (range 60-70.6) was delivered to involved nodes. Response evaluation was performed at a median of 5 weeks after completion of radiotherapy.</p> <p>Results</p> <p>Neck failure was observed in 11 patients and the 3-year regional control (RC) rate was 77.1%. Neck dissection was performed in 10 of the 11 patients; seven of these cases were successfully salvaged, and the ultimate rate of neck control was 92%. The remaining two patients who received neck dissection had negative pathologic results. On univariate analysis, initial nodal size > 2 cm, a less-than-complete response at the primary site, post-radiotherapy nodal size > 1.5 cm, and post-radiotherapy nodal necrosis were associated with RC. On multivariate analysis, less-than-complete primary site response and post-radiotherapy nodal necrosis were identified as independent prognostic factors for RC.</p> <p>Conclusions</p> <p>The neck failure rate after definitive radiochemotherapy without planned neck dissection was 22%. Two-thirds of these were successfully salvaged with neck dissection and the ultimate neck control rate was 92%. Our results suggest that planned neck dissection might not be necessary in patients with complete response of primary site, no evidence of residual lesion > 1.5 cm, or no necrotic lymph nodes at the 1-2 months follow-up evaluation after radiotherapy.</p

    Harmful alcohol use among mothers of under-five child: findings from the Panel Study on Korean Children

    Background Harmful alcohol use in young mothers as a primary caregiver has a profound impact on their own health and interaction with the child. We studied the epidemiology of harmful alcohol use among Korean mothers and their partners of under-five children. Methods We used a longitudinal data of 2,150 Korean mothers of the Panel Study on Korean Children (PSKC). We assessed temporal trend and risk factors for harmful alcohol use in mothers using generalized estimating equation (GEE) model. To estimate the impact of maternal harmful alcohol use on the child, the association between neurodevelopment of the child measured by the Korean-Ages & Stages Questionnaire (K-ASQ) and maternal harmful alcohol use was analyzed using GEE and generalized linear mixed model. We included mothers age, education, preterm, year of the survey and time-varying covariates (including maternal working status, monthly household income, plan for additional childbirth, psychological stress related with childcare, current smoking, and partners harmful alcohol use) in the model. Results Mean age of mothers at the baseline was 31.3 years. Annual prevalence of harmful alcohol use increased from 0.7% in the 1st year to 2.6% in the 4th year (P for linear trend < 0.001). Prevalence of harmful alcohol use increased by 48% per year among the mothers (adjusted odds ratio (OR) = 1.48, 95% confidence interval (CI): 1.22, 1.78). Lower education than college or university graduation was associated with higher odds of harmful alcohol use (2.52, 95% CI: 1.43, 4.45). Current smoking (7.44, 95% CI: 3.00, 18.45), harmful use of alcohol of partner (2.66, 95% CI: 1.57, 4.49) were associated with higher risk of harmful alcohol use in mothers. The association between low K-ASQ score and maternal harmful alcohol use was toward positive which did not reach statistical significance. Conclusions In a cohort of Korean mothers participating in the PKSC, risk of harmful alcohol use increases over time and is associated with harmful alcohol use of their partners. We suggest an approach targeting both parents can be more effective in controlling the harmful alcohol use of mothers.This research was supported by a Korea University Grant (K2022961) and a Korea University Medical Center Grant (K2025301).
