Algorithm based new Tone Reservation method for mitigating PAPR in OFDM systems
There are abundant methods to mitigate PAPR in OFDM signals among which algorithm based tone reservation is of great popularity owing to its low complexity as well as decent BER. Here we have put forward a new distinct algorithm based Tone Reservation technique which is not only less complex and calculates its own threshold as well as PRT signal (unlike other algorithms requiring predetermined threshold and PRT) but also aptly modifies the data by bit by bit comparison with a modified copy of itself (algorithm modified) thus scaling the peaks as and providing a decent BER and good PAPR reduction.

The hypothalamus, the master regulator of circadian rhythm, in association with peripheral clocks, play crucial roles in glucose metabolism. Impairment in cerebral sensing, uptake and processing of glucose has been suggested in various animal and human diabetic models. Diabetes Mellitus has been largely superseded by the discovery of insulin and insulin resistance. Expanding horizons of knowledge of the roles of the hypothalamus in glucose metabolism and the overlapping neural pathways of sugar addictionwith other classically described substance and behavioral addictions networks have again thrown some light on the cerebral theory of DM pathogenesis
Chronic inflammation in polycystic ovary syndrome: A case–control study using multiple markers
Background: Polycystic ovary syndrome (PCOS) is associated with insulin resistance and elevated risk of cardiovascular disease and diabetes. Chronic inflammation has been observed in PCOS in several studies but there is also opposing evidence and a dearth of research in Indians.
Objective: To estimate chronic inflammation in PCOS and find its relationship with appropriate anthropometric and biochemical parameters.
Materials and Methods: Chronic inflammation was assessed in 30 women with PCOS (Group A) and 30 healthy controls (Group B) with highly sensitive C-reactive protein (hsCRP), interleukin-6 (IL-6), tumour necrosis factor alpha (TNFα), and platelet microparticles (PMP). In group A, the relationship of chronic inflammation with insulin resistance, waist hip ratio (WHR) serum testosterone, and serum glutamate pyruvate transaminase (SGPT) were examined.
Results: In group A, the hsCRP, TNFα, and PMP were significantly elevated compared to group B. However, IL-6 level was similar between the groups. In group A, PMP showed a significant positive correlation with waist-hip ratio and serum testosterone. IL-6 showed a significant positive correlation with insulin sensitivity and significant negative correlation with insulin resistance and serum glutamate pyruvate transaminase.
Conclusion: PCOS is associated with chronic inflammation and PMP correlates positively with central adiposity and biochemical hyperandrogenism in women with PCOS.

Role of Lipid, Protein-Derived Toxic Molecules, and Deficiency of Antioxidants behind the Pathogenesis of Diabetic Retinopathy (DR) in Type 2 Diabetes Mellitus
To determine the role of NADPH-oxidase mediated formation of different lipid, protein-derived molecules, and depletion of vitamin-C level in vitreous behind the endothelial dysfunction-induced vascular endothelial growth factor secretion and pathogenesis of diabetic retinopathy (DR) in type 2 diabetes mellitus (T2DM). Fourteen T2DM patients with mild non-proliferative diabetic retinopathy (MNPDR), 11 patients without diabetic retinopathy (DNR), 17 T2 DM subjects with high-risk proliferative diabetic retinopathy (HRPDR), and 5 healthy individuals without DM underwent vitreous analysis for estimation NADPH oxidase, lipid peroxide like malondialdehyde (MDA), 4-Hydroxy-noneal (HNE) and advanced lipoxidation end product (ALE) like Hexanoyl-lysine (HLY), protein carbonyl compound (PCC), Vitamin-C and concentration of vascular endothelial growth factor (VEGF) secretion following standard spectrophotometric methods and enzyme-linked immunosorbent assay (ELISA). Vitreous concentration of NADPH-oxidase, different protein and lipid-derived molecule, and VEGF were found to be significantly elevated among DNR and of DR subjects with different grades compared to HC subjects whereasthe vitamin-C level was found to be decreased among different DR subjects and DNR subjects in comparison to healthy individuals. Oxidative stress-mediated lipid and protein-derived biomolecules not only add important mediators in the pathogenesis of DR, but also accelerate the progression and severity of microangiopathy
Parathyroid carcinoma and persistent hypercalcemia: A case report and review of therapeutic options
Parathyroid carcinomas are very uncommon, accounting for 0.1% to 5% of all causes of primary hyperparathyroidism. Parathyroid–jaw tumor syndrome, with a mutation in HRPT2 that encodes parafibromin, is the most common genetic association. Unique features include aggressive clinical course and a lack of preoperative definitive diagnostic criteria. The authors report a case of a 33-year-old male with bilateral nephrocalcinosis, a left-sided neck mass, high calcium, very high parathormone level and a history of parathyroid adenectomy. Computed tomography and 99m-technetium methoxyisobutylisonitrile scan revealed a localized tumor in the left inferior parathyroid region. The patient underwent radical surgery, and histopathology revealed characteristic features of parathyroid carcinoma. Preoperative identification with clinical clues is very important to plan a more radical surgical approach, as both radiotherapy and chemotherapy are ineffective. Recurrence is common and mostly occurs within 2–3 years after surgery. Patient's age, histology and tumor DNA aneuploidy are predictors of survival. Hypercalcemia is controlled with calcimimetics, bisphosphonates and denosumab in inoperable cases. Furthermore, biologic therapy with parafibromin and telomerase inhibitors is under development
Lean metabolic syndrome: A concept or a reality?
Aims and Objectives: Metabolic syndrome (MetS) is fairly common in India. International Diabetes Federation (IDF) has defined it as the presence of central obesity with region/ethnicity-specific waist circumference as a mandatory defining criterion along with the presence of two other features from dysglycemia, systolic and/or diastolic hypertension, hypertriglyceridemia, and low high-density lipoprotein. Although obesity is not prevalent in rural parts of India, especially among the underprivileged population, other individual components of the syndrome are not infrequent among these lean/nonobese persons. In this study, we evaluated the prevalence of MetS in rural West Bengal. We also examined that if those persons above the recommended cutoff for waist are excluded, and thus only lean/nonobese persons are included, and the same definition of MetS (herein termed as lean MetS) is applied (three out of four criteria), what will be the prevalence of MetS and will it be lower in the nontribal population compared to the tribal population? Materials and Methods: A population-based study was done in the rural area of West Bengal comprising 200 peoples from scheduled tribe (ST) population and 205 people from the neighborhood community from non-ST population to compare the metabolic health with respect to anthropologic and biochemical parameters. Results: The prevalence of MetS from rural areas of West Bengal was found to be 21.48% as per IDF criterion, but applying NCEP ATP III criteria, the overall prevalence of MetS rose to 31.1%. The prevalence of normal waist/lean MetS was 12.8%, and there was no significant difference between nontribals versus tribal cohort (14.6% of the nontribes versus 11.6% of the tribal cohort, P = 0.436). Conclusion: A significant number of persons with nonobese/lean MetS exist in the rural area. We suggest that if region-specific waist criteria are not satisfied, the diagnosis should still be sorted by NCEP ATP III criteria
