    CT appearance of the spleen following conservative management of traumatic injury

    Objective: To describe the long-term changes in the traumatized spleen following conservative management in pediatric patients. Methods: Between 1991 and 1997, 92 children were imaged with splenic trauma. The study population includes the 25 boys and 11 girls with follow-up computed tomography (CT) imaging at our institution. The follow-up CT studies were evaluated to determine the evolution of splenic injury. Results: On initial CT there were 6 grade I, 12 grade II, 9 grade III, and 9 grade IV–V splenic injuries. In follow-up 11 spleens were normal (30 %), including at least one in each grade of severity of injury. Splenic abnormalities were identified on follow-up in 25 children. These findings comprised clefts in 8 children, small cysts in 4, and devascularized segments involving less than 1 cm 3 in 6, 1–2 cm 3 in 2, and 2–4 cm 3 in 5 children. Conclusions: All grades of splenic injury can resolve completely on subsequent CT imaging. In this series 30 % of patients had a normal follow-up CT.     Breast cancer is the second most common cancer in women and causes the death of 519,000 people worldwide. Many cancer genes are posttranscriptionally regulated by RNA-binding proteins (RBPs) and microRNAs. The RBP HuR binds to the AU-rich (ARE) regions of labile mRNAs, such as proto-oncogenes, stabilizing their mRNA and facilitating their translation into protein. HuR has been described to control genes in multiple areas of the acquired capabilities model of cancer and has been hypothesized to be a tumor maintenance gene, allowing for cancers to proliferate once they are established.     The relationships between smoking cigarettes and drinking alcohol and visiting the emergency department in the past 12 months

    Two key issues in healthcare today are the over-utilization of emergency departments (ED) and the lack of preventative care participation. Successful preventative care can help to reduce ED utilization and harmful behaviors such as drinking and smoking. Exposing patients to the benefits of preventative or primary care benefits could start in the ED. This report sought relationships between ED utilization for personal health and smoking cigarettes or drinking alcohol in a 12 month period. This study utilized the CHIS 2011-2012 data set to find such a relationship. A relationship was found between smoking and drinking during a 12 month period and visiting the ED for personal health. This report identifies an opportunity for improvement within health benefit utilization. Smokers and drinkers can be exposed to cessation programs in the ED, while also helping to encourage patients to better utilize primary and preventative services

    Engineering Naval Resilience: Expeditionary Command and Control

    Command and control (C2) systems are essential to US Navy expeditionary forces’ ability to adapt to uncertain, complex, and rapidly evolving missions and work settings. Various portions of the expeditionary command structure include circumstances that a common operating picture can benefit. A C2 system can serve as a source of resilience for these forces by providing a common operating picture that is available to each level in the command hierarchy. Such a display makes it possible for each level in the command hierarchy to perform needed cognitive work from planning and replanning to problem detection, sensemaking, situation assessment, naturalistic decision making, and coordination. Our project for the U.S. Navy used cognitive systems engineering to develop requirements, a C2 display prototype, and a guide to integrate human-centered design into the small scale systems acquisition process.
