MECHANICS OF AMPUTEE JUMPING – CONSIDERATION FOR LOADING
Amputees must develop compensatory mechanisms to overcome the constraints imposed by a mechanical prosthesis. In completing a bilateral countermovement jump, amputees must accommodate the limited ankle dorsiflexion angle and adapt to the limited plantar-flexor moment that occurs at the prosthetic joint. The aim of this research was to determine the loading on the limbs and the joint kinetics adopted by transtibial amputees in order to achieve a jump. Six amputee (AMP) and 10 able-bodied (AB) participants performed maximal vertical jumps on two force plates while kinematic data was recorded using a 9-camera VICON infrared system. The amputees did not jump as high as the AB participants. The AMPs raised the prosthetic heel from the floor to compensate for the restricted motion at the ankle. Consequently, kinematic symmetry was maintained at the knee and the hip. The knee flexion places the prosthetic shank in a more horizontal position. This is a vulnerable position due to the reduced strength in the knee extensors as a consequence of the amputation. In order to reduce the instability and loading at the knee, the maximum propulsive vGRF on the prosthetic side was reduced and the intact limb assumed a dominant role. Until amputees can take the loading on the prosthetic side, it is not recommended that they participate in jumping

The purpose of this study was determine if dynamic elastic response (DER) prostheses could absorb energy in the eccentric phase of a vertical jump performed by trans-tibial amputees phase and return this energy in the propulsive phase. Further, given the active nature of the ankle, the study aimed to determine the mechanisms required at the remaining joints to compensate for the pathological ankle. Six amputee (AMP) and 10 able-bodied participants (AB) performed maximal vertical jumps on two force plates which were synchronised with a 9-camera VICON infra red system. The amputees did not jump as high as the AB participants. Only minimal negative work was recorded at the prosthetic ankle in the eccentric phase which resulted in minimal positive work at the ankle in the concentric phase. The intact side produced greater work than the affected side in the concentric phase. The amputees generally adopted a hip strategy to generate positive work. The work recorded at the knee was reduced on the intact and affected side and indicates the prosthesis influences the movement on both sides. To enable amputees to participate in activities which require jumping, prostheses need to be developed and amputees need to be taught how to adjust their biomechanics to store and release energy in the prosthesis
Giving away used injection equipment: missed prevention message?
Abstract
Background
Our objective was to examine factors associated with distributive injection equipment sharing and how needle exchange programs (NEPs) can help reduce distributive sharing among injection drug users (IDUs).
Methods
145 English speaking Canadian IDUs ages 16 years and over who had injected in the past 30 days were recruited for a cross-sectional survey. Participants were asked about their socio-demographic characteristics, HIV risk behaviours, social support, drug treatment readiness, program satisfaction, health and social service use and NEP drug use. Bivariate statistics and logistic regression were used to characterize the population and examine correlates of sharing behaviour.
Results
More IDUs reported distributive sharing of cookers (45%) than needles (36%) or other types of equipment (water 36%; filters 29%; swabs 8%). Regression analyses revealed the following factors associated with distributing used cookers: a history of cocaine/crack injection, an Addiction Severity Index (ASI) score indicative of a mental health problem, and older than 30 years of age. Factors associated with giving away used water included: male, injected methadone, injected other stimulants and moved 3+ times in the past 6 months. Factors associated with giving away used filters included: injected cocaine/crack or stayed overnight on the street or other public place. Factors associated with giving away swabs included: an ASI mental health score indicative of a mental health problem, and HCV negative status.
Conclusions
Our findings show that more IDUs give away cookers than needles or other injection equipment. While the results showed that correlates of sharing differed by piece of equipment, each point to distributive sharing by the most marginalized IDUs. Targeting prevention efforts to reduce equipment sharing in general, and cookers in particular is warranted to reduce use of contaminated equipment and viral transmission
The use of continuous spectral analysis for the assessment of postural stability changes after sports-related concussion
© 2019 Elsevier Ltd Impaired postural stability is associated with a variety of pathologies including sports-related concussion (SRC). Quantification of centre of pressure (COP) movement is the most common focus of instrumented assessment. Frequency-domain COP analyses have focused primarily on summary measures or pre-defined frequency bands but continuous analysis may provide novel and complementary insight into pathological control mechanisms. Our aims were (i) to compare post-SRC COP trajectory changes identified using clinician scores (Modified Balance Error Scoring System (M-BESS)), time-domain COP variables and continuous frequency spectral comparison; and (ii) to characterise frequency spectra changes. Male rugby players aged 15–19 years (n = 135) completed a pre-season baseline assessment comprising vision-obscured double-leg, single-leg and tandem stances on a force platform. Participants diagnosed with SRC during the season (n = 15) underwent repeat testing (median 4 days post-SRC; IQR 2.5–6.5). Baseline and post-SRC COP trajectories were compared using common time-domain COP variables, M-BESS scores and continuous frequency spectra. Post-SRC changes were identified using all three approaches. Spectral analysis revealed the largest effect size (Cliff's delta 0.39) and was the only method to identify differences in all three stances and in double-leg stance. All post-SRC increases in spectral content were in the anteroposterior direction; all decreases were in the mediolateral direction. Changes were localised to higher frequencies (1.7–8 Hz) except for double-leg stance anteroposterior direction, for which increases were observed throughout the analysed range. Our findings suggest that this method of spectral comparison may provide a more responsive and meaningful measure of postural stability changes after SRC than other commonly-used variables
The Object Coordination Class Applied to Wavepulses: Analysing Student Reasoning in Wave Physics
Detailed investigations of student reasoning show that students approach the
topic of wave physics using both event-like and object-like descriptions of
wavepulses, but primarily focus on object properties in their reasoning.
Student responses to interview and written questions are analysed using diSessa
and Sherin's coordination class model which suggests that student use of
specific reasoning resources is guided by possibly unconscious cues. Here, the
term reasoning resources is used in a general fashion to describe any of the
smaller grain size models of reasoning (p-prims, facets of knowledge, intuitive
rules, etc) rather than theoretically ambiguous (mis)conceptions. Student
applications of reasoning resources, including one previously undocumented, are
described. Though the coordination class model is extremely helpful in
organising the research data, problematic aspects of the model are also
