Knowledge and Awareness of Congenital Cytomegalovirus Among Women
Background. Congenital cytomegalovirus (CMV) infection is a leading cause of disabilities in children, yet the general public appears to have little awareness of CMV. Methods. Women were surveyed about newborn infections at 7 different geographic locations. Results. Of the 643 women surveyed, 142 (22%) had heard of congenital CMV. Awareness increased with increasing levels of education (P < .0001). Women who had worked as a healthcare professional had a higher prevalence of awareness of CMV than had other women (56% versus 16%, P < .0001). Women who were aware of CMV were most likely to have heard about it from a healthcare provider (54%), but most could not correctly identify modes of CMV transmission or prevention. Among common causes of birth defects and childhood illnesses, women's awareness of CMV ranked last. Conclusion. Despite its large public health burden, few women had heard of congenital CMV, and even fewer were aware of prevention strategies
XRCC1 mutation is associated with PARP1 hyperactivation and cerebellar ataxia
XRCC1 is a molecular scaffold protein that assembles multi-protein complexes involved in DNA single-strand break repair1,2. Here we show that biallelic mutations in the human XRCC1 gene are associated with ocular motor apraxia, axonal neuropathy, and progressive cerebellar ataxia. Cells from a patient with mutations in XRCC1 exhibited not only reduced rates of single-strand break repair but also elevated levels of protein ADP-ribosylation. This latter phenotype is recapitulated in a related syndrome caused by mutations in the XRCC1 partner protein PNKP3,4,5 and implicates hyperactivation of poly(ADP-ribose) polymerase/s as a cause of cerebellar ataxia. Indeed, remarkably, genetic deletion of Parp1 rescued normal cerebellar ADP-ribose levels and reduced the loss of cerebellar neurons and ataxia in Xrcc1-defective mice, identifying a molecular mechanism by which endogenous single-strand breaks trigger neuropathology. Collectively, these data establish the importance of XRCC1 protein complexes for normal neurological function and identify PARP1 as a therapeutic target in DNA strand break repair-defective disease
Disclosure of HIV serostatus and condomless sex among men living with HIV/AIDS in Florida.
Despite campaigns to increase safer sex practices, there are people living with HIV/AIDS (PLWH) who do not disclose their HIV status to sexual partners and engage in condomless sex. The purpose of this research was to: 1) describe factors associated with disclosure of HIV status to sexual partners; and 2) determine if disclosure and/or receipt of prevention counseling are independently associated with condomless sex. We used the Florida Medical Monitoring Project to analyze data from 376 HIV positive men with more than one sexual partner. Results indicated that 55% consistently disclosed their HIV status to sexual partners, 30% inconsistently disclosed, 15% did not disclose, and 48% reported any condomless sex. The odds of having condomless sex was 3.3 (CI = 1.5, 7.3) times greater in men who disclosed to all partners. Results suggest that men who disclose are also those who are more likely to have condomless sex. More research is needed to better understand the complex nature of disclosure and sexual risk behaviors and how disclosure impacts sexual risk
Increasing awareness and uptake of the MenB vaccine on a large university campus
Objective: At a large public university, we aimed to evaluate an intervention designed to increase serogroup B meningococcal (MenB) vaccine uptake and awareness. Methods: Using a pretest-posttest design with a double posttest, we evaluated an intervention conducted by a local foundation and the Florida Department of Health that distributed MenB vaccine on campus and conducted an educational campaign. Prior to intervention activities, we recruited students to complete a survey about their MenB knowledge and attitudes. For survey participants who provided contact information, we sent two follow-up surveys and assessed MenB vaccine records. We used chi-square tests, adjusted for nonindependence, to compare preintervention to postintervention (three-month and one-year) vaccination and attitudes. Results: Among the 686 students with accessible vaccine records, MenB vaccine initiation increased 9% (from 24% to 33%) and completion increased 8% (from 13% to 21%) from before the intervention to one year after the intervention. When restricting to students who completed the relevant follow-up surveys, the percentage of students who heard of the MenB vaccine increased by 15% (p >Ā .001) from before the intervention to three months after (nĀ =Ā 188 students) and maintained a 10% increase (p >Ā .001) one year after the intervention (nĀ =Ā 261 students). Among students that heard of the MenB vaccine, the percentage of students who thought they needed the MenB vaccine even though they received the MenACWY increased 14% (p =Ā .03) by the three-month postintervention survey and up to 18% by the one-year follow-up (p =Ā .002). Conclusions: A university-wide, on-campus vaccination and educational campaign increased college studentsā MenB vaccine initiation, completion, and knowledge. Clinicaltrials.gov ID: NCT02975596
Promoting Rural-Residing Parentsā Receptivity to HPV Vaccination: Targeting Messages and Mobile Clinic Implementation
Interventions are needed to increase low HPV vaccination rates within rural areas in the United States, particularly in the state of Florida, which has the seventh highest number of HPV-related cancers. Florida also ranks low compared to other states in terms of HPV vaccination. Rural-residing parents may benefit from two evidence-based strategies to increase vaccination rates: reminder messages informing and prompting vaccination appointments and mobile clinics to reduce transportation barriers. We sought to identify parental attitudes towards (1) message features that promote rural-residing parentsā receptivity to HPV vaccination; (2) parentsā acceptability of three reminder message modalities (text, postcard, phone); and (3) implementation factors that promote parentsā acceptability of using a mobile clinic for vaccination. We recruited 28 rural-residing parents of 9- to 12-year-old children (unvaccinated for HPV) for focus group and individual interviews and thematically analyzed transcripts. Three features promoted parentsā receptivity to HPV vaccination messages: source credibility, specificĀ information coverage, and personalization (name and birthday wishes). Parents most preferred text messages and identified three factors promoting parentsā mobile clinic use: convenience and feasibility, trustworthiness, and detailed information. The findings indicate rural-residing parentsā acceptability of reminder messages and mobile clinics as well as the importance of trust and feasibility when implementing these evidence-based strategies for rural-residing parents
Substance Use as a Risk Factor for Intimate Partner Violence Overlap
Intimate partner violence (IPV) research often focuses on either the victims of IPV or the perpetrators of IPV. Recent studies have documented the existence of a group of victim-perpetrators, for example, they perpetrate IPV and are also the victims of IPV. The current study examines this overlap in IPV perpetration and victimization among a nationally representative, longitudinal sample of 1,488 Hispanics with a focus on generational status. Results from group-based trajectory models and survey multinomial regression techniques suggest that alcohol and marijuana use over time are salient risk factors for IPV perpetration, IPV victimization, and IPV overlap. Study limitations and implications are discussed
