Graphite Materials Testing in the ATR for Lifetime Management of Magnox Reactors
A major feature of the Magnox gas cooled reactor design is the graphite core, which acts as the moderator but also provides the physical structure for fuel, control rods, instrumentation and coolant gas channels. The lifetime of a graphite core is dependent upon two principal aging processes: irradiation damage and radiolytic oxidation. Irradiation damage from fast neutrons creates lattice defects leading to changes in physical and mechanical properties and the accumulation of stresses. Radiolytic oxidation is caused by the reaction of oxidizing species from the carbon dioxide coolant gas with the graphite, these species being produced by gamma radiation. Radiolytic oxidation reduces the density and hence the moderating capability of the graphite, but also reduces strength affecting the integrity of core components. In order to manage continued operation over the planned lifetimes of their power stations, BNFL needed to extend their database of the effects of these two phenomena on their graphite cores through an irradiation experiment. This paper will discuss the background, purpose, and the processes taken and planned (i.e. post irradiation examination) to ensure meaningful data on the graphite core material is obtained from the irradiation experiment
A patient satisfaction survey investigating pre- and post-operative information provision in lower limb surgery
Background
Planned lower limb surgery is common, with over 90,000 hip replacements, 95,000 knee replacements and 15,000 anterior cruciate ligament reconstructions performed in the UK each year. These procedures are primarily indicated to treat osteoarthritis, sporting injuries and trauma. Patient satisfaction is an important element of healthcare provision, which is usually measured by functional outcomes but influenced by other factors. Few studies have assessed patientsâ views on the information given to them pertaining surgery and patients are infrequently consulted when designing leaflets and information packs, which can lead to confusion during the recovery period and poor long-term outcomes. Furthermore, previous studies have not directly asked patients what resources they would prefer, or which format would suit them best. This project aimed to assess if patients were satisfied with the information they received around their operations and to identify potential improvements.
Methods
Set in a National Health Service (NHS) run major trauma centre in the West Midlands, a multiple choice and free-text answer survey was administered to patients who used the orthopaedic service over the course of 1 month. Surveys were designed in Qualtrics and administered face-to-face on paper. Thematic content analysis was performed on the results.
Results
Eighty patients completed the survey, of which 88.8% of patients were satisfied with the information they received. Discussions with surgeons were the most useful resource and 53% of patients requested more internet resources. Post-operative patients were statistically more likely to be dissatisfied with information provision than pre-operative patients. Over 20% of the study population requested more information on post-operative pain and recovery timelines.
Conclusions
Although patients were satisfied in general, areas for change were identified. Suggested resources took the form of webpages and mobile platforms. These resources could contain educational videos, patient experience blogs or interactive recovery timelines, to be of benefit to patients. These suggestions may enable NHS Trusts to âget into the digital ageâ, however, more research on patient satisfaction around information provision and the impact it has on recovery and decision making is needed
A guide for assessing the potential impacts on ecosystems of leakage from CO2 storage sites
Evidence to date indicates that leakage is of low probability if site selection, characterisation and storage project design are undertaken correctly. In Europe, the Storage Directive (EC, 2009) provides a legislative framework, implemented by Member States, which requires appropriate project design to ensure the storage of CO2 is permanent and safe. However, it is incumbent on storage site operators to demonstrate an understanding of the potential impacts on surface ecosystems should a leak occur.
The RISCS (Research into Impacts and Safety in CO2 Storage) project has produced a Guide to potential impacts of leakage from CO2 storage (the âGuideâ). RISCS assessed the potential effects of CO2 leakage from geological storage on both onshore and offshore near-surface ecosystems and on potable ground water. This assessment was achieved through laboratory and field experiments, through observations at sites of natural CO2 seepage and through numerical simulations. The Guide summarises some of the key findings of the project.
The Guide provides information on the best approaches to evaluate potential impacts of hypothetical leakage from CO2 storage sites and to provide guidance on appraising these impacts. This information will be relevant to regulators and operators in particular, but also to other stakeholders who are concerned with CO2 storage, such as national and local governments, and members of the public
Gaps in detailed knowledge of human papillomavirus (HPV) and the HPV vaccine among medical students in Scotland
<p>Background: A vaccination programme targeted against human papillomavirus (HPV) types16 and 18 was introduced in the UK in 2008, with the aim of decreasing incidence of cervical disease. Vaccine roll out to 12â13 year old girls with a catch-up programme for girls aged up to 17 years and 364 days was accompanied by a very comprehensive public health information (PHI) campaign which described the role of HPV in the development of cervical cancer.</p>
<p>Methods: A brief questionnaire, designed to assess acquisition of knowledge of HPV infection and its association to cervical cancer, was administered to two different cohorts of male and female 1st year medical students (school leavers: 83% in age range 17â20) at a UK university. The study was timed so that the first survey in 2008 immediately followed a summer's intensive PHI campaign and very shortly after vaccine roll-out (150 students). The second survey was exactly one year later over which time there was a sustained PHI campaign (213 students).</p>
<p>Results: We addressed three research questions: knowledge about three specific details of HPV infection that could be acquired from PHI, whether length of the PHI campaign and/or vaccination of females had any bearing on HPV knowledge, and knowledge differences between men and women regarding HPV. No female student in the 2008 cohort had completed the three-dose vaccine schedule compared to 58.4% of female students in 2009. Overall, participantsâ knowledge regarding the sexually transmitted nature of HPV and its association with cervical cancer was high in both year groups. However, in both years, less than 50% of students correctly identified that HPV causes over 90% of cases of cervical cancer. Males gave fewer correct answers for these two details in 2009. In 2008 only around 50% of students recognised that the current vaccine protects against a limited subset of cervical cancer-causing HPV sub-types, although there was a significant increase in correct response among female students in the 2009 cohort compared to the 2008 cohort.</p>
<p>onclusions: This study highlights a lack of understanding regarding the extent of protection against cervical cancer conferred by the HPV vaccine, even among an educated population in the UK who could have a vested interest in acquiring such knowledge. The intensive PHI campaign accompanying the first year of HPV vaccination seemed to have little effect on knowledge over time. This is one of the first studies to assess detailed knowledge of HPV in both males and females. There is scope for continued improvements to PHI regarding the link between HPV infection and cervical cancer.</p>
Detector design for high-resolution MeV photon imaging of cargo containers using spectralinformation
Monte Carlo simulations of a pixelated detector array of inorganic scintillators for high spatial resolution imaging of 1-9 MeV photons are presented. The results suggest that a detector array of 0.5 cm x 0.5 cm x 5 cm pixels of bismuth germanate may provide sufficient efficiency and spatial resolution to permit imaging of an object with uncertainties in dimension of several mm. The cross talk between pixels is found to be in the range of a few percent when pixels are shielded by {approx} 1mm of lead or tungsten. The contrast at the edge of an object is greatly improved by rejection of events depositing less than {approx} 1 MeV. Given the relatively short decay time of BGO, the simulations suggest that such a detector may prove adequate for the purpose of rapid scanning of highly-shielded cargos for possible presence of high atomic number (including clandestine fissionable) materials when used with low current high duty factor x-ray sources
Assessment of regression-based methods to adjust for publication bias through a comprehensive simulation study
<p>Abstract</p> <p>Background</p> <p>In meta-analysis, the presence of funnel plot asymmetry is attributed to publication or other small-study effects, which causes larger effects to be observed in the smaller studies. This issue potentially mean inappropriate conclusions are drawn from a meta-analysis. If meta-analysis is to be used to inform decision-making, a reliable way to adjust pooled estimates for potential funnel plot asymmetry is required.</p> <p>Methods</p> <p>A comprehensive simulation study is presented to assess the performance of different adjustment methods including the novel application of several regression-based methods (which are commonly applied to detect publication bias rather than adjust for it) and the popular Trim & Fill algorithm. Meta-analyses with binary outcomes, analysed on the log odds ratio scale, were simulated by considering scenarios with and without i) publication bias and; ii) heterogeneity. Publication bias was induced through two underlying mechanisms assuming the probability of publication depends on i) the study effect size; or ii) the p-value.</p> <p>Results</p> <p>The performance of all methods tended to worsen as unexplained heterogeneity increased and the number of studies in the meta-analysis decreased. Applying the methods conditional on an initial test for the presence of funnel plot asymmetry generally provided poorer performance than the unconditional use of the adjustment method. Several of the regression based methods consistently outperformed the Trim & Fill estimators.</p> <p>Conclusion</p> <p>Regression-based adjustments for publication bias and other small study effects are easy to conduct and outperformed more established methods over a wide range of simulation scenarios.</p
Reaction hijacking inhibition of Plasmodium falciparum asparagine tRNA synthetase
Malaria poses an enormous threat to human health. With ever increasing resistance to currently deployed drugs, breakthrough compounds with novel mechanisms of action are urgently needed. Here, we explore pyrimidine-based sulfonamides as a new low molecular weight inhibitor class with drug-like physical parameters and a synthetically accessible scaffold. We show that the exemplar, OSM-S-106, has potent activity against parasite cultures, low mammalian cell toxicity and low propensity for resistance development. In vitro evolution of resistance using a slow ramp-up approach pointed to the Plasmodium falciparum cytoplasmic asparaginyl-tRNA synthetase (PfAsnRS) as the target, consistent with our finding that OSM-S-106 inhibits protein translation and activates the amino acid starvation response. Targeted mass spectrometry confirms that OSM-S-106 is a pro-inhibitor and that inhibition of PfAsnRS occurs via enzyme-mediated production of an Asn-OSM-S-106 adduct. Human AsnRS is much less susceptible to this reaction hijacking mechanism. X-ray crystallographic studies of human AsnRS in complex with inhibitor adducts and docking of pro-inhibitors into a model of Asn-tRNA-bound PfAsnRS provide insights into the structure-activity relationship and the selectivity mechanism.</p
