energy consumption of a last generation full hybrid vehicle compared with a conventional vehicle in real drive conditions
Abstract Hybrid vehicles are one of the most important choices to improve efficiency and reduce CO2 production of vehicles. Benefits in using hybrid powertrains are generally found in urban environment where lower average speeds, higher accelerations make the internal combustion engine run at lower efficiency points. The originality of the present paper consists in the data elaboration and analysis collected in a measurement campaign on road in real driving conditions, on an ad hoc path planned according to the average national daily mileage in metropolitan urban context, which thus acquires a significance generalizable in that specific context, which led to the consumption quantification and an analysis of the main factors that determine the reduction in consumption of full-hybrid vs conventional vehicles. Another important and original aspect of this paper is the analysis of the operating times in ZEV mode of hybrid vehicles, which shows how this solution leads to a significant reduction of pollutant emissions in urban contest. An on-road experimental campaign has been done by comparing two different versions of the same model (Toyota Yaris Hybrid and a conventional one, Toyota Yaris 1.5 gasoline) and a hybrid vehicle with different characteristics (the hybrid born - Toyota Prius), like size, traction battery capacity, generator/motor electric power. Thirty drivers on a fixed path have done this experimental campaign and in this paper, the results are reported. The results show that a strong influence on consumption is due not only to the type of vehicle, but also to driving style and speed. The comparison between the two versions of Yaris, shows a strong reduction in consumption using hybrid vehicle for low and medium speeds (for 20 km/h about 50%), such benefit decreases with the increasing speed and for values higher than 90 km/h both the vehicles have the same consumption. The reduced consumption of the hybrid vehicle at low speeds is due, on the one hand, to the greater efficiency of the hybrid vehicle engine compared to the conventional one and on the other hand to the high functioning in ZEV mode, with the engine off, (63% of time) thanks to the use of the electric motor. The comparison between the two hybrid vehicles with different characteristics (YarisHy and Prius) shows that the consumption trend vs. speeds is similar but the Prius has lower consumption due above all to the high efficiency of the braking energy recovery system, despite the greatest mass. This lead then to significant consumption reduction, but also lower emissions in places where such parameters have an important role: the urban environment
Impact of diet and nutraceutical supplementation on inflammation in elderly people. Results from the RISTOMED study, an open-label randomized control trial.
BACKGROUND & AIMS: Eating habits may influence the life span and the quality of ageing process by modulating inflammation. The RISTOMED project was developed to provide a personalized and balanced diet, enriched with or without nutraceutical compounds, to decrease and prevent inflammageing, oxidative stress and gut microbiota alteration in healthy elderly people. This paper focused on the effect on inflammation and metabolism markers after 56 days of RISTOMED diet alone or supplementation with three nutraceutical compounds.
METHODS:A cohort of 125 healthy elderly subjects was recruited and randomized into 4 arms (Arm A, RISTOMED diet; Arm B, RISTOMED diet plus VSL#3 probiotic blend; Arm C, RISTOMED diet plus AISA d-Limonene; Arm D, RISTOMED diet plus Argan oil). Inflammatory and metabolism parameters as well as the ratio between Clostridium cluster IV and Bifidobacteria (CL/B) were collected before and after 56 days of dietary intervention, and their evolution compared among the arms. Moreover, participants were subdivided according to their baseline inflammatory parameters (erythrocytes sedimentation rate (ESR), C-Reactive Protein, fibrinogen, Tumor Necrosis Factor-alfa (TNF-α), and Interleukin 6) in two clusters with low or medium-high level of inflammation. The evolution of the measured parameters was then examined separately in each cluster.
RESULTS:Overall, RISTOMED diet alone or with each nutraceutical supplementation significantly decreased ESR. RISTOMED diet supplemented with d-Limonene resulted in a decrease in fibrinogen, glucose, insulin levels and HOMA-IR. The most beneficial effects were observed in subjects with a medium-high inflammatory status who received RISTOMED diet with AISA d-Limonene supplementation. Moreover, RISTOMED diet associated with VSL#3 probiotic blend induced a decrease in the CL/B ratio.
CONCLUSIONS:Overall, this study emphasizes the beneficial anti-inflammageing effect of RISTOMED diet supplemented with nutraceuticals to control the inflammatory status of elderly individuals
Evidences of +896 A/G TLR4 Polymorphism as an Indicative of Prevalence of Complications in T2DM Patients
T2DMis today considered as world-wide health problem, with complications responsible of an enhanced mortality and morbidity.
Thus, new strategies for its prevention and therapy are necessary. For this reason, the research interest has focused its attention
on TLR4 and its polymorphisms, particularly the rs4986790. However, no conclusive findings have been reported until now about
the role of this polymorphism in development of T2DM and its complications, even if a recent meta-analysis showed its T2DM
association in Caucasians. In this study, we sought to evaluate the weight of rs4986790 polymorphism in the risk of the major
T2DMcomplications, including 367 T2DMpatients complicated for the 55.6%. Patients with A/A and A/G TLR4 genotypes showed
significant differences in complication\u2019s prevalence. In particular, AG carriers had higher risk prevalence for neuropathy (P =
0.026), lower limb arteriopathy (P = 0.013), and the major cardiovascular pathologies (P = 0.017). Their cumulative risk was
significant (P = 0.01), with a threefold risk to develop neuropathy, lower limb arteriopathy, and major cardiovascular events in AG
cases compared to AA cases.The adjusted OR for the confounding variables was 3.788 (95% CI: 1.642\u20138.741).Thus, the rs4986790
polymorphism may be an indicative of prevalence of complications in T2DM patients
Remodelling of biological parameters during human ageing: evidence for complex regulation in longevity and in type 2 diabetes.
Factor structure analyses have revealed the presence of specific biological
system markers in healthy humans and diseases. However, this type of approach in
very old persons and in type 2 diabetes (T2DM) is lacking. A total sample of
2,137 Italians consisted of two groups: 1,604 healthy and 533 with T2DM. Age
(years) was categorized as adults (≤65), old (66-85), oldest old (>85-98) and
centenarians (≥99). Specific biomarkers of routine haematological and biochemical
testing were tested across each age group. Exploratory factorial analysis (EFA)
by principal component method with Varimax rotation was used to identify factors
including related variables. Structural equation modelling (SEM) was applied to
confirm factor solutions for each age group. EFA and SEM identified specific
factor structures according to age in both groups. An age-associated reduction of
factor structure was observed from adults to oldest old in the healthy group
(explained variance 60.4% vs 50.3%) and from adults to old in the T2DM group
(explained variance 57.4% vs 44.2%). Centenarians showed three-factor structure
similar to those of adults (explained variance 58.4%). The inflammatory component
became the major factor in old group and was the first one in T2DM. SEM analysis
in healthy subjects suggested that the glucose levels had an important role in
the oldest old. Factorial structure change during healthy ageing was associated
with a decrease in complexity but showed an increase in variability and
inflammation. Structural relationship changes observed in healthy subjects
appeared earlier in diabetic patients and later in centenarians
Stratification of the risk of developing severe or lethal Covid-19 using a new score from a large Italian population: A population-based cohort study
Objectives To develop a population-based risk stratification model (COVID-19 Vulnerability Score) for predicting severe/fatal clinical manifestations of SARS-CoV-2 infection, using the multiple source information provided by the healthcare utilisation databases of the Italian National Health Service. Design Retrospective observational cohort study. Setting Population-based study using the healthcare utilisation database from five Italian regions. Participants Beneficiaries of the National Health Service, aged 18-79 years, who had the residentship in the five participating regions. Residents in a nursing home were not included. The model was built from the 7 655 502 residents of Lombardy region. Main outcome measure The score included gender, age and 29 conditions/diseases selected from a list of 61 conditions which independently predicted the primary outcome, that is, severe (intensive care unit admission) or fatal manifestation of COVID-19 experienced during the first epidemic wave (until June 2020). The score performance was validated by applying the model to several validation sets, that is, Lombardy population (second epidemic wave), and the other four Italian regions (entire 2020) for a total of about 15.4 million individuals and 7031 outcomes. Predictive performance was assessed by discrimination (areas under the receiver operating characteristic curve) and calibration (plot of observed vs predicted outcomes). Results We observed a clear positive trend towards increasing outcome incidence as the score increased. The areas under the receiver operating characteristic curve of the COVID-19 Vulnerability Score ranged from 0.85 to 0.88, which compared favourably with the areas of generic scores such as the Charlson Comorbidity Score (0.60). A remarkable performance of the score on the calibration of observed and predicted outcome probability was also observed. Conclusions A score based on data used for public health management accurately predicted the occurrence of severe/fatal manifestations of COVID-19. Use of this score may help health decision-makers to more accurately identify high-risk citizens who need early preventive or treatment interventions
Identification, replication and characterization of epigenetic remodelling in the aging genome:A cross population analysis
Aging is a complex biological process regulated by multiple cellular pathways and molecular mechanisms including epigenetics. Using genome-wide DNA methylation data measured in a large collection of Scottish old individuals, we performed discovery association analysis to identify age-methylated CpGs and replicated them in two independent Danish cohorts. The double-replicated CpGs were characterized by distribution over gene regions and location in relation to CpG islands. The replicated CpGs were further characterized by involvement in biological pathways to study their functional implications in aging. The economic impact of moderate stage Alzheimer's disease in Italy: Evidence from the UP-TECH randomized trial
Background: There is consensus that dementia is the most burdensome disease for modern societies. Few cost-of-illness studies examined the complexity of Alzheimer's disease (AD) burden, considering at the same time health and social care, cash allowances, informal care, and out-of-pocket expenditure by families. Methods: This is a comprehensive cost-of-illness study based on the baseline data from a randomized controlled trial (UP-TECH) enrolling 438 patients with moderate AD and their primary caregiver living in the community. Results: The societal burden of AD, composed of public, patient, and informal care costs, was about �20,000/yr. Out of this, the cost borne by the public sector was �4,534/yr. The main driver of public cost was the national cash-for-care allowance (�2,324/yr), followed by drug prescriptions (�1,402/yr). Out-of-pocket expenditure predominantly concerned the cost of private care workers. The value of informal care peaked at �13,590/yr. Socioeconomic factors do not influence AD public cost, but do affect the level of out-of-pocket expenditure. Conclusion: The burden of AD reflects the structure of Italian welfare. The families predominantly manage AD patients. The public expenditure is mostly for drugs and cash-for-care benefits. From a State perspective in the short term, the advantage of these care arrangements is clear, compared to the cost of residential care. However, if caregivers are not adequately supported, savings may be soon offset by higher risk of caregiver morbidity and mortality produced by high burden and stress. Socioeconomic Predictors of the Employment of Migrant Care Workers by Italian Families Assisting Older Alzheimer's Disease Patients: Evidence from the Up-Tech Study
Background: The availability of family caregivers of older people is decreasing in Italy as the number of migrant care workers (MCWs) hired by families increases. There is little evidence on the influence of socioeconomic factors in the employment of MCWs. Method: We analyzed baseline data from 438 older people with moderate Alzheimer's disease (AD), and their family caregivers enrolled in the Up-Tech trial. We used bivariate analysis and multilevel regressions to investigate the association between independent variables - education, social class, and the availability of a care allowance - and three outcomes - employment of a MCW, hours of care provided by the primary family caregiver, and by the family network (primary and other family caregivers). Results: The availability of a care allowance and the educational level were independently associated with employing MCWs. A significant interaction between education and care allowance was found, suggesting that more educated families are more likely to spend the care allowance to hire a MCW. Discussion: Socioeconomic inequalities negatively influenced access both to private care and to care allowance, leading disadvantaged families to directly provide more assistance to AD patients. Care allowance entitlement needs to be reformed in Italy and in countries with similar long-term care and migration systems. � 2015 The Author 2015. Epigenetic mechanisms of endothelial dysfunction in type 2 diabetes
The development of type-2 diabetes mellitus (T2DM) and its complications is largely due to the complex interaction between genetic factors and environmental influences, mainly dietary habits and lifestyle, which can either accelerate or slow down disease progression. Recent findings suggest the potential involvement of epigenetic mechanisms as a crucial interface between the effects of genetic predisposition and environmental factors. The common denominator of environmental factors promoting T2DM development and progression is that they trigger an inflammatory response, promoting inflammation-mediated insulin resistance and endothelial dysfunction. Proinflammatory stimuli, including hyperglycemia, oxidative stress, and other inflammatory mediators, can affect epigenetic mechanisms, altering the expression of specific genes in target cells without changes in underlying DNA sequences. DNA methylation and post-translational histone modifications (PTHMs) are the most extensively investigated epigenetic mechanisms. Over the past few years, non-coding RNA, including microRNAs (miRNAs), have also emerged as key players in gene expression modulation. MiRNAs can be actively released or shed by cells in the bloodstream and taken up in active form by receiving cells, acting as efficient systemic communication tools. The miRNAs involved in modulation of inflammatory pathways (inflammamiRs), such as miR-146a, and those highly expressed in endothelial lineages and hematopoietic progenitor cells (angiomiRs), such as miR-126, are the most extensively studied circulating miRNAs in T2DM. However, data on circulating miRNA signatures associated with specific diabetic complications are still lacking. Since immune cells and endothelial cells are primarily involved in the vascular complications of T2DM, their relative contribution to circulating miRNA signatures needs to be elucidated. An integrated approach encompassing different epigenetic mechanisms would have the potential to provide new mechanistic insights into the genesis of diabetes and its severe vascular complications and identify a panel of epigenetic markers with diagnostic/prognostic and therapeutic relevance
