Interpreting positive signs of the supraspinatus test in screening for torn rotator cuff.
The purpose of this study was to investigate the validity of the supraspinatus test as a screening test for detecting torn rotator cuff and to determine what its valuable positive signs were. Both the empty-can test and full-can test were performed on 200 shoulders diagnosed by magnetic resonance imaging (MRI)-and in some cases, surgical findings-to have full-thickness or partial-thickness torn rotator cuff s, or no tear in the rotator cuff . During the maneuver, the presence of pain or weakness or both pain and weakness were recorded as positive signs, and the distribution of these signs were analyzed according to the degree of tear. The predictive values were calculated in 2 ways by considering (1) only full-thickness tears as tears and (2) both full- and partial-thickness tears as tears. The 2 tests and the 2 ways of considering partial-thickness tears were compared. Pain and weakness were severity-dependent, and the empty-can test had a higher incidence of pain. The sensitivities of the 2 supraspinatus tests in all positive signs were higher when including partial-thickness tears in the tear group ; however, their specificities were higher when excluding partial-thickness tears. Both pain and weakness were interpretive for the supraspinatus test, and both tests were sensitive to full- and partial- thickness tears and specific for full-thickness tears
DS-ARP: A New Detection Scheme for ARP Spoofing Attacks Based on Routing Trace for Ubiquitous Environments
Despite the convenience, ubiquitous computing suffers from many threats and security risks. Security considerations in the ubiquitous network are required to create enriched and more secure ubiquitous environments. The address resolution protocol (ARP) is a protocol used to identify the IP address and the physical address of the associated network card. ARP is designed to work without problems in general environments. However, since it does not include security measures against malicious attacks, in its design, an attacker can impersonate another host using ARP spoofing or access important information. In this paper, we propose a new detection scheme for ARP spoofing attacks using a routing trace, which can be used to protect the internal network. Tracing routing can find the change of network movement path. The proposed scheme provides high constancy and compatibility because it does not alter the ARP protocol. In addition, it is simple and stable, as it does not use a complex algorithm or impose extra load on the computer system
A new method for speaker adaptation using bilinear model
ABSTRACT In this paper, a novel method for speaker adaptation using bilinear model is proposed. Bilinear model can express both characteristics of speakers (style) and phonemes across speakers (content) independently in a training database. The mapping from each speaker and phoneme space to observation space is carried out using bilinear mapping matrix which is independent of speaker and phoneme space. We apply the bilinear model to speaker adaption. Using adaptation data from a new speaker, speaker-adapted model is built by estimating the style(speaker)-specific matrix. Experimental results showed that the proposed method outperformed eigenvoice and MLLR. In vocabulary-independent isolated word recognition for speaker adaptation, bilinear model reduced word error rate by about 38% and about 10% compared to eigenvoice and MLLR respectively using 50 words for adaptation
JNK pathway is involved in the inhibition of inflammatory target gene expression and NF-kappaB activation by melittin
<p>Abstract</p> <p>Background</p> <p>Bee venom therapy has been used to treat inflammatory diseases including rheumatoid arthritis in humans and in experimental animals. We previously found that bee venom and melittin (a major component of bee venom) have anti-inflammatory effect by reacting with the sulfhydryl group of p50 of nuclear factor-kappa B (NF-κB) and IκB kinases (IKKs). Since mitogen activated protein (MAP) kinase family is implicated in the NF-κB activation and inflammatory reaction, we further investigated whether activation of MAP kinase may be also involved in the anti-inflammatory effect of melittin and bee venom.</p> <p>Methods</p> <p>The anti-inflammatory effects of melittin and bee venom were investigated in cultured Raw 264.7 cells, THP-1 human monocytic cells and Synoviocytes. The activation of NF-κB was investigated by electrophoretic mobility shift assay. Nitric oxide (NO) and prostaglandin E<sub>2 </sub>(PGE<sub>2</sub>) were determined either by Enzyme Linked Immuno Sorbent Assay or by biochemical assay. Expression of IκB, p50, p65, inducible nitric oxide synthetase (iNOS), cyclooxygenase-2 (COX-2) as well as phosphorylation of MAP kinase family was determined by Western blot.</p> <p>Results</p> <p>Melittin (0.5–5 μg/ml) and bee venom (5 and 10 μg/ml) inhibited lipopolysaccharide (LPS, 1 μg/ml) and sodium nitroprusside (SNP, 200 μM)-induced activation of c-Jun NH2-terminal kinase (JNK) in RAW 264.7 cells in a dose dependent manner. However, JNK inhibitor, anthra [1,9-cd]pyrazole-6 (2H)-one (SP600215, 10–50 μM) dose dependently suppressed the inhibitory effects of melittin and bee venom on NF-κB dependent luciferase and DNA binding activity via suppression of the inhibitory effect of melittin and bee venom on the LPS and SNP-induced translocation of p65 and p50 into nucleus as well as cytosolic release of IκB. Moreover, JNK inhibitor suppressed the inhibitory effects of melittin and bee venom on iNOS and COX-2 expression, and on NO and PGE<sub>2 </sub>generation.</p> <p>Conclusion</p> <p>These data show that melittin and bee venom prevent LPS and SNP-induced NO and PGE<sub>2 </sub>production via JNK pathway dependent inactivation of NF-κB, and suggest that inactivation of JNK pathways may also contribute to the anti-inflammatory and anti-arthritis effects of melittin and bee venom.</p
β-Lapachone Significantly Increases the Effect of Ionizing Radiation to Cause Mitochondrial Apoptosis via JNK Activation in Cancer Cells
β-lapachone (β-lap), has been known to cause NQO1-dependnet death in cancer cells and sensitize cancer cells to ionizing radiation (IR). We investigated the mechanisms underlying the radiosensitization caused by β-lap. cells induced ROS generation, triggered ER stress and stimulated activation of ERK and JNK. Inhibition of ROS generation by NAC effectively attenuated the activation of ERK and JNK, induction of ER stress, and subsequent apoptosis. Importantly, inhibition of ERK abolished ROS generation and ER stress, whereas inhibition of JNK did not, indicating that positive feedback regulation between ERK activation and ROS generation triggers ER stress in response to combined treatment. Furthermore, prevention of ER stress completely blocked combination treatment-induced JNK activation and subsequent apoptotic cell death. In addition, combined treatment efficiently induced the mitochondrial translocation of cleaved Bax, disrupted mitochondrial membrane potential, and the nuclear translocation of AIF, all of which were efficiently blocked by a JNK inhibitor. Caspases 3, 8 and 9 were activated by combined treatment but inhibition of these caspases did not abolish apoptosis indicating caspase activation played a minor role in the induction of apoptosis. cells are treated with combination of IR and β-lap, positive feedback regulation between ERK and ROS leads to ER stress causing JNK activation and mitochondrial translocation of cleaved Bax. A Photometric and Spectroscopic Study of the Short-Period Algol EW Bo\"{o}tis with a Sct Pulsator
In this paper, we present TESS photometry and high-resolution spectra of the
short-period Algol EW Boo. We obtained double-lined radial velocities (RVs)
from the time-series spectra and measured the effective temperature of the
primary star as = 8560 118 K. For the orbital period
study, we collected all times of minima available for over the last 30 years.
It was found that the eclipse timing variation of the system could be
represented by a periodic oscillation of 17.6 0.3 years with a
semi-amplitude of 0.0041 0.0001 d. The orbital and physical parameters
were derived by simultaneously analyzing the TESS light and RV curves using the
Wilson-Devinney (WD) binary star modeling code. The component masses and radii
were showed over 3% precision: = 2.67 0.08 M,
= 0.43 0.01 M, = 2.01 0.02 R, and
= 1.35 0.01 R. Furthermore, multiple frequency
analyses were performed for the light-curve residuals from the WD model. As a
result, we detected 17 pressure-mode pulsations in the region of 40.15 - 52.37
d. The absolute dimensions and pulsation characteristics showed that the
Sct pulsator was the more massive and hotter primary star of the EW

Evaluation of the pathogenicity of GJB3 and GJB6 variants associated with nonsyndromic hearing loss
AbstractA number of genes responsible for hearing loss are related to ion recycling and homeostasis in the inner ear. Connexins (Cx26 encoded by GJB2, Cx31 encoded by GJB3 and Cx30 encoded by GJB6) are core components of gap junctions in the inner ear. Gap junctions are intercellular communication channels and important factors that are associated with hearing loss. To date, a molecular genetics study of GJB3 and GJB6 as a causative gene for hearing loss has not been performed in Korea. This study was therefore performed to elucidate the genetic characteristics of Korean patients with nonsyndromic sensorineural hearing loss and to determine the pathological mechanism of hearing loss by analyzing the intercellular communication function of Cx30 and Cx31 variants. Sequencing analysis of the GJB3 and GJB6 genes in our population revealed a total of nine variants, including four novel variants in the two genes. Three of the novel variants (Cx31-p.V27M, Cx31-p.V43M and Cx-30-p.I248V) and two previously reported variants (Cx31-p.V84I and Cx30-p.A40V) were selected for functional studies using a pathogenicity prediction program and assessed for whether the mutations were located in a conserved region of the protein. The results of biochemical and ionic coupling tests showed that both the Cx31-p.V27M and Cx31-p.V84I variants did not function normally when each was expressed as a heterozygote with the wild-type Cx31. This study demonstrated that two variants of Cx31 were pathogenic mutations with deleterious effect. This information will be valuable in understanding the pathogenic role of GJB3 and GJB6 mutations associated with hearing loss
High Extracellular Calcium Increased Expression of Ank, PC-1 andOsteopontin in Mouse Calvarial Cells
In the process of bone remodeling, mineral phase of bone
is dissolved by osteoclasts, resulting in elevation of calcium
concentration in micro-environment. This study was performed
to explore the effect of high extracellular calcium
(Ca
2+
e) on mineralized nodule formation and on the expression
of progressive ankylosis (Ank), plasma cell membrane
glycoprotein-1 (PC-1) and osteopontin by primary cultured
mouse calvarial cells. Osteoblastic differentiation and mineralized
nodule formation was induced by culture of mouse
calvarial cells in osteoblast differentiation medium containing
ascorbic acid and β-glycerophosphate. Although Ank, PC-1
and osteopontin are well known inhibitors of mineralization,
expression of these genes were induced at the later stage of
osteoblast differentiation during when expression of osteocalcin,
a late marker gene of osteoblast differentiation, was
induced and mineralization was actively progressing. High
Ca
2+
e (10 mM) treatment highly enhanced mRNA expression
of Ank, PC-1 and osteopontin in the late stage of osteoblast
differentiation but not in the early stage. Inhibition of p44/42
MAPK activation but not that of protein kinase C suppressed
high Ca
2+
e-induced expression of Ank, PC-1 and
osteopontin. When high Ca
2+
e (5 mM or 10 mM) was present
in culture medium during when mineral deposition was
actively progressing, matrix calcifiation was significantly
increased by high Ca
2+
e. This stimulatory effect was abolished
by pyrophosphate (5 mM) or levamisole (0.1-0.5 mM), an
alkaline phosphatase inhibitor. In addition, probenecid (2mM),
an inhibitor of Ank, suppressed matrix calcification in both
control and high Ca
2+
e-treated group, suggesting the possible
role of Ank in matrix calcification by osteoblasts. Taken
together, these results showed that high Ca
2+
e stimulates expression of Ank, PC-1 and osteopontin as well as matrix
calcification in late differentiation stage of osteoblasts and
that p44/42 MAPK activation is involved in high Ca
2+
e-
induced expression of Ank, PC-1 and osteopontin
Comparing the cut score for the borderline group method and borderline regression method with norm-referenced standard setting in an objective structured clinical examination in medical school in Korea
Purpose Setting standards is critical in health professions. However, appropriate standard setting methods do not always apply to the set cut score in performance assessment. The aim of this study was to compare the cut score when the standard setting is changed from the norm-referenced method to the borderline group method (BGM) and borderline regression method (BRM) in an objective structured clinical examination (OSCE) in medical school. Methods This was an explorative study to model the implementation of the BGM and BRM. A total of 107 fourth-year medical students attended the OSCE at 7 stations for encountering standardized patients (SPs) and at 1 station for performing skills on a manikin on July 15th, 2021. Thirty-two physician examiners evaluated the performance by completing a checklist and global rating scales. Results The cut score of the norm-referenced method was lower than that of the BGM (P<0.01) and BRM (P<0.02). There was no significant difference in the cut score between the BGM and BRM (P=0.40). The station with the highest standard deviation and the highest proportion of the borderline group showed the largest cut score difference in standard setting methods. Conclusion Prefixed cut scores by the norm-referenced method without considering station contents or examinee performance can vary due to station difficulty and content, affecting the appropriateness of standard setting decisions. If there is an adequate consensus on the criteria for the borderline group, standard setting with the BRM could be applied as a practical and defensible method to determine the cut score for OSCE
Antimicrobial Activity of Thinned Strawberry Fruits at Different Maturation Stages
Among the phenolic compounds that is generally present in strawberry fruits, five simple phenolics, three flavonoids, and a stilbene were tested for their antimicrobial activity against seven fungi and one oomycete. trans-Cinnamic acid showed strong antimicrobial activity, and the antimicrobial effect of the simple phenolics decreased with an increase in the number of hydroxyl groups. Phytophthora capsici was the most susceptible to the phenolic compounds tested in this study. trans-Cinnamic acid, p-hydroxybenzoic acid, and kaempferol were mainly detected in 'Seolhyang' strawberry fruits, and the total phenolic contents of the fruits decreased during their development. Extracts of the green #1-10% red color# and red #above 90% red color# strawberry fruits reduced the mycelial growth and zoospore germination rate of P. capsici, and the extract of red strawberry fruit showed strong antimicrobial activity against the zoospore germination of P. capsici.
