    A Novel Triterpene Saponin from Gypsophila capillaris

    A novel C-28 tetraglycoside of quillaic acid (1) has been isolated from Gypsophila capillaris. The structure was elucidated by 1 D NMR (NOE difference, DEPT, selective13C{1H} INEPT), 2D NMR (1H,1H and1H,13C COSY,1H,1H,1H RELAY, ROESY and TOCSY) and other spectroscopic and chromatographic evidences.     Diffusion Tensor Imaging in a Large Longitudinal Series of Patients With Cervical Spondylotic Myelopathy Correlated With Long-Term Functional Outcome

    BACKGROUND Fractional anisotropy (FA) of the high cervical cord correlates with upper limb function in acute cervical cord injury. We investigated the correlation between preoperative FA at the level of maximal compression and functional recovery in a group of patients after decompressive surgery for cervical spondylotic myelopathy (CSM). OBJECTIVE To determine the usefulness of FA as a biomarker for severity of CSM and as a prognostic biomarker for improvement after surgery. METHODS Patients received diffusion tensor imaging (DTI) scans preoperatively. FA values of the whole cord cross-section at the level of maximal compression and upper cervical cord (C1-2) were calculated. Functional status was measured using the modified Japanese Orthopedic Association (mJOA) scale preoperatively and at follow-up up to 2 yr. Regression analysis between FA and mJOA was performed. DTI at C4-7 was obtained in controls. RESULTS Forty-four CSM patients enrolled prior to decompression were compared with 24 controls. FA at the level of maximal compression correlated positively with preoperative mJOA score. Preoperative FA correlated inversely with recovery throughout the postoperative period. This was statistically significant at 12 mo postoperation and nearly so at 6 and 24 mo. Patients with preoperative FA0.55. CONCLUSION In the largest longitudinal study of this kind, FA promises a valid biomarker for severity of CSM and postoperative improvement. FA is an objective measure of function and could provide a basis for prognosis. FA is particularly useful if preoperative values are less than 0.55

    STR-990: SUSTAINABLE GROUTED HELICAL PILES: MATERIALS AND PERFORMANCE

    Cementitious materials are widely used as a construction material all over the world. However, cement industry has high environmental impact such as the release of CO2 and the consumption of natural resources for its manufacturing energy. Therefore, reducing cement consumption is vital to achieve sustainable green construction practices. In this study, the effects of using treated oil sand waste (TOSW) as a partial replacement of cement in grouted helical pile applications were investigated. Fresh and hardened properties of the green grout incorporating different percentages of TOSW were evaluated. In addition, a model scale grouted helical pile with the green grout was tested to characterize its performance. The experimental results show that the properties of TOSW grout mixes were comparable to conventional grout and satisfy the strength and construction requirements of grouted piles. Moreover, tested grouted helical pile using the developed mixture exhibited similar geotechnical performance as those installed using conventional grout mix. Hence, TOSW can be implemented in grouted helical pile applications, which would assist in achieving sustainable construction

    Perioperative Levosimendan Infusion in Patients With End-Stage Heart Failure Undergoing Left Ventricular Assist Device Implantation

    Left ventricular assist device (LVAD) therapy has been instrumental in saving lives of patients with end-stage heart failure (HF). Recent generation devices have short-to-mid-term survival rates close to heart transplantation. Unfortunately, up to 1 in 4 patients develop a life-threatening right-sided HF (RHF) early post LVAD implantation, with high morbidity and mortality rate, necessitating prolonged ICU stay, prolonged inotropic support, and implantation of a right-ventricular assist device. Pre-operative optimization of HF therapy could help in prevention, and/or mitigation of RHF. Levosimendan (LEVO) is a non-conventional inotropic agent that works by amplifying calcium sensitivity of troponin C in cardiac myocytes, without increasing the intra-cellular calcium or exacerbating ischemia. LEVO acts as an inodilator, which reduces the cardiac pre-, and after-load. LEVO administration is associated with hemodynamic improvements. Despite decades long of the use of LVAD and more than two decades of the use of LEVO for HF, the literature on LEVO use in LVAD is very limited. In this paper, we sought to conduct a systematic review to synthesize evidence related to the use of LEVO for the mitigation and/or prevention of RHF in patients undergoing LVAD implantation

    Inline Water Quality Monitoring System (IWQMS)

    Inline Water Quality Monitoring System (IWQMS) is a system that performs scheduled testing for the water quality in the infrastructure pipes without human interventions. This system was designed to monitor the water quality based on the working area requirements and the user's aims. It is supposed to observe water in power plants working by steam, biomedical systems, pharmaceuticals production lines, food factories, desalination plant, water wall, and laboratories. However, water quality depends on the physical and chemical parameters of the water. Two of these parameters are concentration of salts in the water or total dissolved solid (TDS) and electrical conductivity (EC). IWQMS uses these two parameters to determine the water quality by measuring the electrical waves in the tested water sample. It consists of three designed subsystems operating together; these subsystems are mechanical, electronic and software. The mechanical subsystem is designed to automatically collect water samples without human intervention from the pipes. The electronic subsystem is used for controlling and sensing. Sensing is performed using a sensor especially designed to be used for IWQMS. This sensor measures two important parameters TDS and EC. The software subsystem is used to process the collected data, arrange the operations, and store the obtained data for the operator or send it through wireless sensor network. Many iterations and tests were performed on the system and it produced satisfactory results with very small error variation compared with laboratories testing devices

    Local Revenue Development in Egypt / الإيرادات المحلية في مصر

    Local administration in Egypt faces several challenges that hinder fulfilling its responsibilities towards citizens sufficiently. The main challenges are the lack of financial resources due to over-regulation as well as the non-enforcement of laws and regulations that allow the local administration to collect its own tax share, the local administration staff’s lack of skills to manage and develop revenue sources, and lack of accountability mechanisms. This policy paper seeks to propose policies to aid the local administration in increasing its financial resources. In this paper, the authors depended on desk research as well as interviews held with officials from the Ministry of Finance and the Ministry of Local Development. Based on the information gathered, the authors proposed three policy options. Finally, they selected the “Local Non-Tax Revenue Mobilization” policy option based on the evaluation criteria and provided recommendations for this policy’s successful implementation

    Peguero Electrocardiographic Left Ventricular Hypertrophy Criteria and Risk of Mortality

    Background: Peguero electrocardiographic left ventricular hypertrophy (ECG-LVH) criteria are newly developed criteria that have shown better diagnostic performance than the traditional Cornell-voltage and Sokolow-Lyon criteria. However, prediction of poor outcomes rather than detection of increased left ventricular mass is becoming the primary use for ECG-LVH criteria which requires investigating any new ECG-LVH criteria in terms of prediction.Aims: To examine the prognostic significance of the newly developed Peguero ECG-LVH criteria.Methods: We compared the prognostic significance of Peguero ECG-LVH with Cornell-voltage and Sokolow-Lyon ECG-LVH criteria in 7,825 participants (age 59.8 ± 13.4 years; 52.7% women) from the third National Health and Nutrition Examination Survey who were free of major intraventricular conduction defects. ECG-LVH criteria were derived from digital ECG tracings processed at a central core laboratory.Results: At baseline, ECG-LVH was detected in 11.8% by Peguero; in 4.3% by Cornell voltage and in 6.4% by Sokolow-Lyon. During a median follow up of 13.8 years, 2,796 all-cause mortality events occurred. In multivariable models adjusted for demographics and cardiovascular risk factors, presence of Peguero ECG-LVH was associated with increased risk of all-cause mortality [HR (95% CI): 1.29 (1.16, 1.44)]. This association was not significantly different from the associations of Cornell voltage-LVH or Sokolow-Lyon LVH with all-cause mortality [HR (95%CI): 1.32 (1.12, 1.55) and 1.24 (1.07, 1.43), respectively; p-values for comparisons of these HRs with the HR of Peguero ECG-LVH 0.817 and 0.667, respectively]. Similar patterns of associations were observed with cardiovascular, ischemic heart disease and heart failure mortalities.Conclusion: Peguero ECG-LVH is predictive of increased risk of death similar to the traditional ECG-LVH criteria

    CdSe Quantum Dots for Solar Cell Devices

    CdSe quantum dots have been prepared with different sizes and exploited as inorganic dye to sensitize a wide bandgap TiO2 thin films for QDs solar cells. The synthesis is based on the pyrolysis of organometallic reagents by injection into a hot coordinating solvent. This provides temporally discrete nucleation and permits controlled growth of macroscopic quantities of nanocrystallites. XRD, HRTEM, UV-visible, and PL were used to characterize the synthesized quantum dots. The results showed CdSe quantum dots with sizes ranging from 3 nm to 6 nm which enabled the control of the optical properties and consequently the solar cell performance. Solar cell of 0.08% performance under solar irradiation with a light intensity of 100 mW/cm2 has been obtained. CdSe/TiO2 solar cells without and with using mercaptopropionic acid (MPA) as a linker between CdSe and TiO2 particles despite a Voc of 428 mV, Jsc of 0.184 mAcm-2, FF of 0.57, and η of 0.05% but with linker despite a Voc of 543 mV, Jsc of 0.318 mAcm-2 , FF of 0.48, and η of 0.08%, respectively
