    Inpatient multidisciplinary care can prevent deterioration of renal function in patients with chronic kidney disease: a nationwide cohort study

    BackgroundMultidisciplinary care is necessary to prevent worsening renal function and all-cause mortality in patients with chronic kidney disease (CKD) but has mostly been investigated in the outpatient setting. In this study, we evaluated the outcome of multidisciplinary care for CKD according to whether it was provided in an outpatient or inpatient setting.MethodsThis nationwide, multicenter, retrospective, observational study included 2954 Japanese patients with CKD stage 3–5 who received multidisciplinary care in 2015–2019. Patients were divided into two groups: an inpatient group and an outpatient group, according to the delivery of multidisciplinary care. The primary composite endpoint was the initiation of renal replacement therapy (RRT) and all-cause mortality, and the secondary endpoints were the annual decline in the estimated glomerular filtration rate (ΔeGFR) and the changes in proteinuria between the two groups.ResultsMultidisciplinary care was provided on an inpatient basis in 59.7% and on an outpatient basis in 40.3%. The mean number of health care professionals involved in multidisciplinary care was 4.5 in the inpatient group and 2.6 in the outpatient group (P < 0.0001). After adjustment for confounders, the hazard ratio of the primary composite endpoint was significantly lower in the inpatient group than in the outpatient group (0.71, 95% confidence interval 0.60-0.85, P = 0.0001). In both groups, the mean annual ΔeGFR was significantly improved, and proteinuria significantly decreased 24 months after the initiation of multidisciplinary care.ConclusionMultidisciplinary care may significantly slow deterioration of eGFR and reduce proteinuria in patients with CKD and be more effective in terms of reducing initiation of RRT and all-cause mortality when provided on an inpatient basis

    Technical Notes on Endoscopic Transnasal Transsphenoidal Approach for Clival Chondrosarcoma

    Although there are various operative approaches for clival tumors, a transsphenoidal approach is one of choices when the main tumor extention is in an anterior-posterior direction with a slight lateral extension. However, this approach sometimes provides only narrow and deep operative field. Recently, endoscopic transnasal transsphenoidal approach is quite an effective approach for clival tumors because of the improvement of surgical instruments, image guidance systems, and techniques and materials of wound closure. In this paper, we describe the effectiveness, technical problems, and solution of this approach based on our experiences with two clival chondrosarcomas that was removed by endoscopic transnasal transsphenoidal approach

    Abdominoperineal Resection for Unexpected Distal Intramural Spreading of Rectal Cancer

    Introduction: In rectal cancer, distal intramural spread may sometimes occur, but a maximum extent of distal spread of > 6 cm is very rare. Case Presentation: A 65-year-old Japanese male with an advanced rectal cancer tumor with para-aortic lymph node metastasis was admitted. We performed a low anterior resection with lymphadenectomy, but the intraoperative frozen-section analysis of margins revealed malignant cell positivity; we, therefore, performed an abdominoperineal resection. Pathological findings showed that the maximum extent of distal spread was 6 cm. After 12 courses of FOLFOX4 as adjuvant chemotherapy, abdominal computed tomography revealed whole lymph node metastases, including Virchow’s node. Though FOLFIRI + panitumumab was started, he was not eligible for additional chemotherapy after 10 cycles. Conclusion: An intraoperative frozen pathology examination was helpful for the additional resection, when unexpected distal spreading had occurred in rectal cancer. The evidence of a distal negative margin should not be underestimated

    Structural and magnetic properties of rapidly quenched (Sm,R)(Fe,Co)11.4Ti0.6 (R = Y, Zr) with ThMn12 structure

    The structural and magnetic properties of (Sm0.8R0.2)(Fe0.8Co0.2)11.4Ti0.6 (R = Y, Zr) prepared by rapid quenching are investigated focusing on the differences between the Y- and Zr-substituted alloys. An almost single phase with ThMn12 structure was obtained for both cases of R = Y and Zr. The μ0Ms of (Sm0.8Y0.2)(Fe0.8Co0.2)11.4Ti0.6 determined from the magnetization curve was 1.52 T, whereas that for (Sm0.8Zr0.2)(Fe0.8Co0.2)11.4Ti0.6 was 1.46 T. To clarify the difference in magnetic properties between the samples, hyperfine splitting was measured by Mössbauer spectroscopy and it was found that the magnetic moment of the Y-substituted alloy was larger than that of the Zr-substituted alloy in all three Fe sites, and this result was consistent with the difference in μ0Ms obtained by magnetization measurement
