Total and partial IgA deficiency in blood serum in children. Clinical observation
We conducted a 13-year observation of 9 patients with total and partial IgA deficiency. The age of children in primary treatment was 3.0 Β± 0.2 years on average. The observation was carried out on the basis of the outpatient service of Irkutsk State Regional Children's Clinical Hospital. The examination included immunological, clinical-allergic and functional methods of research. To assess the humoral link of the immune system, serum Ig concentrations of classes A, M, G (mg/dl) were determined in the first 3 years annually, and then every 2-3 years. As a result, it was found that among the patients with IgA immunodeficiency, boys predominate over girls in a ratio of 3.5:1. With a partial IgA deficiency detected at an early age, the majority of patients have normalization of their IgA level, but sometimes there is a transformation into a selective IgA deficiency with an increase in the number of clinical markers of this condition. With a selective deficiency of IgA detected at an early age, half of the patients have normalization of the IgA level by the age of adolescence, but another half still maintain a significant decrease in serum IgA. Clinical manifestations in the majority of observed children with selective IgA deficiency included atopic respiratory diseases - bronchial asthma and allergic rhinitis, in one case - diabetes mellitus. In some children, the selective deficiency of IgA was asymptomatic
Aetiology of urticaria in children
Urticaria is a frequent disease in childhood, which almost every child had. Usually, the presence of distinct urticaria does not present difficulties in nosological diagnosis, but many factors in account for its development cause challenge for doctors in choosing tactics for therapy. Traditionally the triggers of urticaria in children are food allergens and drugs, and that is why pediatricians unreasonably eliminate many foods from the baby's nutrition for a long time. We analyzed the causes of acute and chronic urticaria in children hospitalized in the department of pediatrics over the ten-year period. In acute urticaria, in most cases, triggers were food (46.7 % children) and drugs (22.5 % children), in children with chronic urticaria, food was the cause of rashes only in 20 % of cases, and the infectious etiology of the disease was confirmed in 8 children. In almost a half of children with chronic urticaria the cause of the rash was not established. Based on the research, recommendations for the examination of patients with this pathology are suggested. Except for testing for food allergens and drugs, the diagnostic protocol of a patient with chronic urticaria should include the laboratory tests for infectious agents, as well as conducting fibrogastroduodenoscopy with biopsy of the gastric mucosa according to indications
Follow-up Study of a Child with Severe Combined Immune Deficiency
We present the results of 7-year follow-up of a patient with primary immunodeficiency, such as severe combined immune deficiency, X-linked variant. The child has been ill from 11 month of age. He was taken to the Regional Infectious Disease Hospital in Irkutsk by sanitary aviation in extremely serious condition, which threatened his life. At 1 year 2 months he was transferred to Irkutsk State Regional Childrenβs Clinical Hospital due to deterioration of the general condition, prolonged fever, expressed by hypoxemia. At 1 year 3 months he was diagnosed with primary immunodeficiency, X-linked severe combined immune deficiency, persistent CMV infection in the department of clinical immunology of the Republican Childrenβs Clinical Hospital in Moscow. The diagnosis was confirmed by molecular genetic method (mutation Ρ.664Π‘ΛΠ’ was detected in exon 5 of the IL2RG gene in the hemizygotic state). At 1 year 9 months, haploidentical transplantation of hematopoietic stem cells from the father was performed. According to the chimerism, immune transplant rejection was observed after 1.5 months. At 2 years 11 months, the boy successfully underwent allogeneic bone marrow transplantation from an unrelated donor in the Childrenβs Oncology and Hematology Hospital of the University Hospital Freiburg (Germany). The child is being regularly observed in Irkutsk State Regional Childrenβs Clinical Hospital. He suffers from respiratory infections 4β5 times a year in a mild form. He corresponds to peers in physical and psychomotor development

Using fMRI Brain Activation to Identify Cognitive States Associated with Perception of Tools and Dwellings
Previous studies have succeeded in identifying the cognitive state corresponding to the perception of a set of depicted categories, such as tools, by analyzing the accompanying pattern of brain activity, measured with fMRI. The current research focused on identifying the cognitive state associated with a 4s viewing of an individual line drawing (1 of 10 familiar objects, 5 tools and 5 dwellings, such as a hammer or a castle). Here we demonstrate the ability to reliably (1) identify which of the 10 drawings a participant was viewing, based on that participant's characteristic whole-brain neural activation patterns, excluding visual areas; (2) identify the category of the object with even higher accuracy, based on that participant's activation; and (3) identify, for the first time, both individual objects and the category of the object the participant was viewing, based only on other participants' activation patterns. The voxels important for category identification were located similarly across participants, and distributed throughout the cortex, focused in ventral temporal perceptual areas but also including more frontal association areas (and somewhat left-lateralized). These findings indicate the presence of stable, distributed, communal, and identifiable neural states corresponding to object concepts
