    Distinct Cytoplasmic Expression of KL-6 Mucin in Chromophobe Renal Cell Carcinoma: A Comparative Immunohistochemical Study with Other Renal Epithelial Cell Tumors

We compared the immunohistochemical expression of sialyl glycoproteins in chromophobe RCC with that in other types of renal tumors. Formalin-fixed, paraffin-embedded tissues of surgically resected renal tumors (chromophobe RCC, 14 cases [10 cases of classic type and 4 cases of eosinophilic variant]; oncocytoma, 7 cases; and clear cell RCC, 9 cases) and kidneys from immature infants (4 cases) were immunostained with antibodies against sialyl glycoproteins (anti-KL-6 and anti-sialyl MUC1 antibodies). Cytoplasmic expression of KL-6 and sialyl MUC1 was distinctive in the chromophobe RCC and renal oncocytoma cells, and in the intercalated cells in collecting duct epithelia. Apical-surface staining of these sialyl glycoproteins was predominantly observed in clear RCC, in the epithelia of the distal tubule and collecting duct, and in the neonatal renal proximal tubule, but not in those of the adult renal proximal tubule. The above-mentioned observations provide additional evidence for similar phenotypic profiles of chromophobe RCC and renal oncocytoma, and the intercalated cells in collecting ducts and the oncofetal expression of sialyl glycoproteins in clear cell RCC.     β(2)-Adrenergic and M(2)-muscarinic receptors decrease basal t-tubular L-type Ca2+ channel activity and suppress ventricular contractility in heart failure

    L-Lype Ca2+ channels (LTCC) play a crucial role in cardiac excitation-contraction coupling. We previously found that in failing ventricular myocytes of mice chronically treated with isoproterenol, basal t-tubular (TT) LTCC activity was halved by activation of protein phosphatase (PP)2A whereas basal surface sarcolemmal (SS) LTCC activity was doubled by inhibition of PP1. Interestingly, chronic treatment of these mice with pertussis toxin almost completely normalized TT and SS LTCC densities and cardiac contractility. In the present study, we therefore sought to identify the G(i/o) protein coupled receptors in cardiac myocytes (i.e. beta(2)-adrenergic, M-2-muscarinic and A(1)-adenosine receptors) that are responsible for these abnormalities in heart failure by chronically administrating mice a selective antagonist of each receptor (ICI118,551, atropine and 8-cyclopentyl-1,3-dipropilxanthine (DPCPX), respectively) with isoproterenol. Compared with mice treated with isoproterenol alone, mice treated with isoproterenol plus ICI118,551 or atropine, but not DPCPX showed significantly lower lung weight/tibial length, higher fractional shortening, lower left ventricular end-diastolic pressure and higher dP/dt(max) and dP/dt(min). In addition, ventricular myocytes of mice treated with isoproterenol plus ICI118,551 or atropine, but not DPCPX exhibited significantly higher TT and lower SS LTCC current densities than those of mice treated with isoproterenol alone due to normalization of the PP activities. These results indicate that beta(2)-adrenergic, M-2-muscarinic, but not A(1)-adenosine receptors contribute to reduced ventricular contractility at least partially by decreasing basal TT LTCC activity in heart failure.     Complete Response to S-1 Monotherapy Over 3 Years in an Elderly Patient with Advanced Non-Small-Cell Lung Cancer

    The number of elderly patients with cancer continues to increase.The optimal chemotherapeutic regimen for inoperable non-small-cell lung cancer (NSCLC) in elderly patients, especially over 80 years old, remains controversial.S-1,a newly developed oral fluorouracil antitumor drug,has been reported to be effective in the treatment of gastrointestinal and pancreatic tumors and/or NSCLC. We experienced a case of an elderly patient with advanced-stage NSCLC who responded well to S-1 monotherapy with tolerable toxicities and has had three years of disease-free survival. S-1 appears to have significant activity with mild toxicities for selected elderly NSCLC patients and in our view warrants further clinical study.     <Articles>Local Politics in the Migration between Vietnam and Cambodia: Mobility in a Multiethnic Society in the Mekong Delta since 1975

    This paper examines the history of cross-border migration by (primarily) Khmer residents of Vietnam’s Mekong Delta since 1975. Using a multiethnic village as an example case, it follows the changes in migratory patterns and control of crossborder migration by the Vietnamese state, from the collectivization era to the early Đổi Mới reforms and into the post-Cold War era. In so doing, it demonstrates that while negotiations between border crossers and the state around the social acceptance (“licit-ness”) and illegality of cross-border migration were invisible during the 1980s and 1990s, they have come to the fore since the 2000s. Despite the border being legally closed in the 1980s and 1990s, undocumented migration as a livelihood strategy was rampant due to the porous nature of the border. In the 2000s, the border began to be officially opened to local people and simultaneously function as a political boundary to regulate belonging and identities. The changes in migratory patterns indicate that the mapping and establishment of a national border alone is not enough to etch it in the minds of people, especially minorities who have connections with people in the neighboring country. Rather, a border “hardens” through continuous negotiations between state actors, who become suspicious of influence from a foreign country, and cross-border migrants, who become dependent on the state for their needs

    Mediastinal Growing Teratoma Syndrome Succesfully Treated by Multiple Modality Therapies

    A 43-year-old-man was diagnosed as having primary mediastinal nonseminomatous germ cell tumor based on pericutaneous biopsy and an elevated serum concentration of alpha-fetoprotein (AFP). Although AFP decreased after chemotherapy, the mass size grew and developed tracheal compression by the tumor. The patient was treated with mechanical ventilation and subsequent stent implantation for keeping airway patency. After three cycles of chemotherapy and normalization of AFP, the increased mass was successfully resected and the pathological examination demonstrated a mature teratoma. This case showed a rare clinical manifestation of mediastinal growing teratoma syndrome and successful outcome by multimodal therapies.ArticleINTERNAL MEDICINE. 50(6):607-610 (2011)journal articl
