Modulation of Immunoglobulin Production by Invariant Vα19-Jα33 TCR-Bearing Cells

We have previously shown that invariant Vα19-Jα33 TCR+ (Vα19i T) cells suppress the disease progress in some models for organ specific autoimmune diseases and type IV allergy that deteriorate along with decline to excess in Th1- or Th17- immunity. In this study, we examined the effects of over-generation of Vα19i T cells on the Th2-controlled immunoglobulin isotype production in the models for type I allergy. IgE production by invariant Vα19-Jα33 TCR transgenic (Tg) mice was suppressed compared with that by non-Tg controls following administration with goat anti-mouse IgD antiserum or OVA, while IgG2a production was not influenced by the introduction of the transgene into the recipients. IgE production by wild type mice was similarly reduced when they were subjected to adoptive transfer with invariant Vα19-Jα33 TCR Tg+ but not Tg− cells prior to immunization. Furthermore, the suppression of IgE production by these recipients was enhanced when they were previously administered with a Vα19i T cell activator, one of the modified α-mannosyl ceramides. In summary, it is suggested that Vα19i T cells have potential to participate in the homeostasis of immunity and that they suppress disease progression resulting from not only Th1- but also Th2- immunity excess

Improved Hypertension by Investigating Circadian Rhythm of Blood Pressure

Case: The case is 86-year-old male hypertensive patient with anti-hypertensive drug for 5 years. He has been provided Amlodipine besilate 5mg at 0800h and doxazosin mesylate 2mg at 2300h for long. In June 2020, he noticed unstable fluctuation of Blood Pressure (BP) during morning, afternoon and night. Results: Then, he checked the circadian rhythm of BP, which showed higher BP in early morning, decreasing BP 0800-1000h, minimum BP during 1000-1400h, increasing BP during 1400-1800h and stable BP during 1800-2400h. Due to the result, he changed to take amlodipine at 2300h. Consequently, his BP gradually became stable during 24 hours after 2 weeks. Discussion: Some factors may exist for contributing improved BP fluctuation. They include a) pathophysiological characteristics of BP circadian rhythm, b) effective time for anti-hypertensive drug, c) accuracy of the obtained BP data and d) the social and psychological reliability of the patient. Regarding d) he was engaged in research and development work as a senior researcher at a chemical company. Inhibition by drugs of passive cutaneous anaphylaxis-induced skin histamine decrease.

Passive cutaneous anaphylaxis (PCA) was produced in the rat with mouse IgE-rich antiserum. The effect of drugs on the PCA-induced skin histamine decrease and leakage of protein-bound dye was studied. Salbutamol (0.5 mg/kg i.v. or 1.0 mg/kg s.c.) and cromoglycate (10 mg/kg i.v.) significantly inhibited the skin histamine decrease. A combination of salbutamol (0.5 mg/kg i.v. or 1.0 mg/kg s.c.) and aminophylline (25 mg/kg i.v. or 75 mg/kg s.c.) had an additive or greater than additive effect on the histamine decrease. Salbutamol (1.0 mg/kg s.c.) inhibited the dye leakage markedly, and aminophylline (75 mg/kg s.c.) slightly. These results indicate that the decrease in the skin histamine content is useful as an index of the in vivo inhibitory effect of antiallergic drugs on the antigen-induced histamine release.</p

Fieldservers and Sensor Service Grid as Real-time Monitoring Infrastructure for Ubiquitous Sensor Networks

The fieldserver is an Internet based observation robot that can provide an outdoor solution for monitoring environmental parameters in real-time. The data from its sensors can be collected to a central server infrastructure and published on the Internet. The information from the sensor network will contribute to monitoring and modeling on various environmental issues in Asia, including agriculture, food, pollution, disaster, climate change etc. An initiative called Sensor Asia is developing an infrastructure called Sensor Service Grid (SSG), which integrates fieldservers and Web GIS to realize easy and low cost installation and operation of ubiquitous field sensor networks

Availability of a remote online hemodynamic monitoring system during treatment in a private dental office for medically high-risk patients

The importance of systemic management to prevent accidents is increasing in dentistry because co-morbid illnesses in an aging society and invasive surgical procedures are increasing. In this prefecture, a new medical system called the remote online hemodynamic monitoring system (ROHMs) was started in 2001. Eight private dental offices participated in this trial. When dental practitioners feel the risk of a dental procedure, they can contact via ROHMs to this hospital. Then, the hemodynamic data (blood pressure, heart rate, ECG, SpO2, and RPP) of the patient in the clinic can be transmitted here via the internet, and the images and the voice can be transmitted as well. The availability of this system was assessed in 66 patients (98 cases). The most frequent complications were hypertension, heart disease, and diabetes mellitus. Systemic management included monitoring during the dental procedure (71.4%), checking vital signs after an interview (15.3%), and monitoring under sedation (13.3%). There were 35.7% of all cases where an unscheduled procedure was necessary for the systemic management. Based on a questionnaire, the majority of the patients felt relieved and safe. This system creates a situation where a specialist is almost present during the procedure. This system will provide significant assistance for future medical cooperation for risk management

Detail Investigation of Frequent Nocturia and its Improvement by the Administration of Diuretic

Background: For elderly people, nocturia has been often found associated with other diseases. Case presentation: The case is 88-year-old male with hypertension, Cardiovascular Disease (CVD), Diabetes, Chronic Kidney Disease (CKD). He developed nocturia and insomnia, and then checked daily water balance by himself. Results: Basic situation showed water intake/urine volume were 750mL/1030mL/day, in which urine ratio of day/night was 27%/73% with waking up twice during night. He was given furosemide 20mg, per os at 1500h. Same study showed 960mL/1090 mL, and urine ratio as 77%/23% with no waking up. Discussion and Conclusion: From some reports, Odds Ratio (OR) for combination of nocturia shows 1.25 for hypertension, 1.23/1.74 for CVD (mild/severe). Consequently, current treatment would contribute adequate managements for nocturia problem

Long-term results of the open stent-grafting technique for extended aortic arch disease

ObjectiveThis report elucidates the long-term safety and effectiveness of extended aortic arch replacement with an open stent-grafting technique from our 12 years of experience.MethodsFrom 1994 to 2004, 126 patients (mean age 67.8 years) with different pathologic conditions of the aortic arch with extension to the descending aorta (57 dissections [acute/chronic = 31/26] and 69 aneurysms) were operated on with an open stent-grafting technique. During deep hypothermic circulatory arrest with selective cerebral perfusion, the stent graft was delivered through the transected proximal aortic arch, and arch replacement with a 4-branched prosthesis was performed.ResultsOperative mortality within 30 days was 3.2%. Perioperative morbidity included 7 (5.6%) strokes and 8 (6.3%) spinal injuries (paraplegia in 3, transient paraparesis in 5). Sixty-three percent of the patients were extubated within 24 hours. In long-term follow-up (mean 60.4 ± 36.5 months, maximum 153 months), survival was 81.1%, 63.3%, and 53.7% at 1, 5, and 8 years. Five (3.9%) late endoleaks were observed but treated with successful additional endovascular repair. Freedom from endoleaks was 98.0%, 91.1%, and 91.1% for 1, 5, and 8 years, respectively.ConclusionLong-term observation showed safety and good durability of the open stent-grafting technique for aortic arch disease. This technique could be an attractive treatment option for aortic arch aneurysm with distal extension and aortic dissection requiring aortic arch replacement

Energy transfer from Cr to Nd in substitutional crystal Y3GaxAl5-x O12 codoped with Nd and Cr

Garnet crystals codoped with Nd3+ and Cr3+ ions are a candidate for solar-pumped laser materials. Substitutional disordered crystals Y3GaxAl5-xO12 were prepared to improve the pumping efficiency of Nd3+ luminescence and energy transfer from Cr3+ to Nd3+ ions. The substitutional disordered crystal host produced inhomogeneous broadening of the Nd3+ and Cr3+ optical spectra. Enhancement of overlapping between the Cr3+ absorption bands and the solar spectrum, and between the Cr3+ luminescence bands and the Nd3+ absorption lines led to the increases of the pumping efficiency and the energy transfer rates, respectively. The excitation spectrum of the Nd3+ luminescence, the nonexponential decay curves of the Cr3+ luminescence, and the quantum yields of the Cr3+ and Nd3+ luminescence have given evidence on the energy transfer from Cr3+ to Nd3+ ions

Penetration of the sigmoid colon to the posterior uterine wall secondary to diverticulitis: a case report

<p>Abstract</p> <p>Introduction</p> <p>Penetration of the colon to the posterior uterine wall secondary to diverticulitis is unusual, with diagnostic methods not yet established. Non-invasive imaging, such as computed tomography and magnetic resonance imaging may help to establish a proper diagnosis, but confirmation may be reached only after surgical exploration.</p> <p>Case presentation</p> <p>We report the case of a 78-year-old Japanese woman who presented with a low grade fever and mild diarrhea which occurred two or three times a week. Computed tomography and magnetic resonance imaging demonstrated a capsular lesion including an air structure with a diameter of 5 cm, between the posterior aspect of the uterine body and the sigmoid colon. A gastrograffin enema and colonoscopy demonstrated a giant diverticulum of the sigmoid colon with no evidence of malignancy. These data confirmed the diagnosis of diverticulitis complicated by a giant diverticulum. Because of a relapsing fever after therapy with antibiotics, the patient had en bloc surgical treatment of the uterus, fallopian tubes, ovaries and sigmoid colon, the organs involved in the diverticulitis, followed by an uneventful recovery.</p> <p>Conclusion</p> <p>This is a rare case report of penetration of the sigmoid colon to the posterior uterine wall secondary to diverticulitis.</p
