    Blood rheology, cardiovascular risk factors, and cardiovascular disease: The West of Scotland Coronary Prevention Study

    The West of Scotland Coronary Prevention Study (WOSCOPS) showed that pravastatin reduced the risk of coronary heart disease (CHD) events in 6,595 middle-aged hypercholesterolaemic men aged 45-64 years without prior myocardial infarction followed for an average of 4.9 years. We hypothesised prospectively (a) that baseline levels of haemorheological variables were related to baseline and incident CHD and to mortality; and (b) that reduction in lipoproteins by pravastatin would lower plasma and blood viscosity, a potential contributory mechanism to CHD events. We therefore studied plasma and blood viscosity, fibrinogen, haematocrit, and blood cell counts at baseline and 1 year. At baseline, plasma and blood viscosity were related to risk factors, CHD measures, and claudication. On univariate analysis, baseline levels of all rheological variables (except platelet count) were related to incident CHD; CHD mortality; and total mortality. On multivariate analysis including baseline CHD and risk factors, plasma and blood viscosity, haematocrit and white cell count each remained significantly associated with incident CHD; while fibrinogen remained an independent predictor of mortality (all p<0.03). After one year, lipoprotein reduction by pravastatin was associated with significant reductions (about one quarter of a standard deviation) in plasma viscosity (mean difference 0.02 mPa.s, p<0.001) and in blood viscosity (mean difference 0.06 mPa.s, p<0.001), but was not associated with significant changes in other rheological variables. We therefore suggest that pravastatin therapy, which reduces elevated lipoproteins in hypercholesterolaemic men, may lower risks of CHD and mortality partly by lowering plasma and blood viscosity. Further studies are required to test this hypothesis

    Choosing treatment for localised prostate cancer: A patient-conducted-interview study

    Objectives: Treatment choice can be particularly difficult in localised prostate cancer because of the uncertainty involved. Indeed, some men prefer maintaining their masculine identity and quality of life to potentially securing longer-term survival through surgery or radiotherapy. UK health services are now obliged to leave the choice of treatment to the patient and the aim of this study is to improve understanding of patients’ experiences of choosing treatment. Methods: A one-day participative workshop where men of six months post-diagnosis design and conduct audio and video interviews on each other about their experiences of choosing treatment. Results: The findings show that treatment choice is a complex process combining emotional and rational elements. Information gathering and delegation to professional expertise were two key themes that emerged. Conclusions: The findings emphasise that treatment choice for localised prostate cancer is little like the traditional notions of consumerism from which it is derived. Importantly, the results illustrate, from a patient perspective, how health professionals can engage in their roles as information providers and as experts

    The Starburst in the Central Kiloparsec of Markarian 231

    We present VLBA observations at 0.33 and 0.61 GHz, and VLA observations between 5 and 22 GHz, of subkiloparsec scale radio emission from Mrk 231. In addition to jet components clearly associated with the AGN, we also find a smooth extended component of size 100 - 1000 pc most probably related to the purported massive star forming disk in Mrk 231. The diffuse radio emission from the disk is found to have a steep spectrum at high frequencies, characteristic of optically thin synchrotron emission. The required relativistic particle density in the disk can be produced by a star formation rate of 220 Msolar/yr in the central kiloparsec. At low frequencies the disk is absorbed, most likely by ionized gas with an emission measure of 8 x 10^5 pc cm-6. We have also identified 4 candidate radio supernovae that, if confirmed, represent direct evidence for ongoing star formation in the central kiloparsec.Comment: in press at ApJ for v. 519 July 1999, 14 page LaTeX document includes 6 postscript figure

    Kinematic Age Estimates for 4 Compact Symmetric Objects from the Pearson-Readhead Survey

    Based on multi-epoch observations at 15 and 43 GHz with the Very Long Baseline Array (VLBA) we detect significant angular expansions between the two hot spots of 4 Compact Symmetric Objects (CSOs). From these relative motions we derive kinematic ages of between 300 and 1200 years for the radio emission. These ages lend support to the idea that CSOs are produced in a recent phase of activity. These observations also allow us to study the evolution of the hot spots dynamically in individual sources. In all 4 sources the hot spots are separating along the source axis, but in 1031+567 the tip of one hot spot appears to be moving almost orthogonally to the source axis. Jet components, seen in 3 of the 4 sources observed, are found to be moving relativistically outward from the central engines towards the more slowly moving hot spots.Comment: in press at ApJ for v. 541 Oct. 1, 2000, 23 page LaTeX document includes 6 postscript figure

    A Coordinated Radio Afterglow Program

    We describe a ground-based effort to find and study afterglows at centimeter and millimeter wavelengths. We have observed all well-localized gamma-ray bursts in the Northern and Southern sky since BeppoSAX first started providing rapid positions in early 1997. Of the 23 GRBs for which X-ray afterglows have been detected, 10 have optical afterglows and 9 have radio afterglows.     Revisiting the identification of wintertime atmospheric circulation regimes in the Euro-Atlantic sector

    Atmospheric circulation is often clustered in so-called circulation regimes, which are persistent and recurrent patterns. For the Euro-Atlantic sector in winter, most studies identify four regimes: the Atlantic Ridge, the Scandinavian Blocking and the two phases of the North Atlantic Oscillation. These results are obtained by applying k-means clustering to the first several empirical orthogonal functions (EOFs) of geopotential height data. Studying the observed circulation in reanalysis data, it is found that when the full field data is used for the k-means cluster analysis instead of the EOFs, the optimal number of clusters is no longer four but six. The two extra regimes that are found are the opposites of the Atlantic Ridge and Scandinavian Blocking, meaning they have a low-pressure area roughly where the original regimes have a high-pressure area. This introduces an appealing symmetry in the clustering result. Incorporating a weak persistence constraint in the clustering procedure is found to lead to a longer duration of regimes, extending beyond the synoptic timescale, without changing their occurrence rates. This is in contrast to the commonly-used application of a time-filter to the data before the clustering is executed, which, while increasing the persistence, changes the occurrence rates of the regimes. We conclude that applying a persistence constraint within the clustering procedure is a superior way of stabilizing the clustering results than low-pass filtering the data

    The Radio Afterglow From GRB 980519: A Test of the Jet and Circumstellar Models

    We present multi-frequency radio observations from the afterglow of GRB 980519 beginning 7.2 hours after the gamma-ray burst and ending 63 days later. The fast decline in the optical and X-ray light curves for this burst has been interpreted either as afterglow emission originating from a collimated outflow -- a jet -- or the result of a blast wave propagating into a medium whose density is shaped by the wind of an evolved massive star. These two models predict divergent behavior for the radio afterglow, and therefore, radio observations are capable, in principle, of discriminating between the two. We show that a wind model describes the subsequent evolution of the radio afterglow rather well. However, we see strong modulation of the light curve, which we interpret as diffractive scintillation.     Ignition and chemical kinetics of acrolein-oxygen-argon mixtures behind reflected shock waves

    In order to address increasing greenhouse gas emissions, the future fossil fuel shortage and increasingly stringent pollutant emission regulations, a variety of biofuels are being progressively incorporated into conventional transportation fuels. Despite the beneficial impact of biofuels on most regulated pollutants, their combustion induces the increase of a variety of aldehydes that are being considered for specific regulations due to their high toxicity. One of the most hazardous aldehyde compounds is acrolein, C_2H_3CHO. Despite its high toxicity and increased formation during bioalcohol and biodiesel combustion, no experimental data are available for acrolein combustion. In the present study, we have investigated the ignition of acrolein–oxygen–argon mixtures behind reflected shock wave using three simultaneous emission diagnostics monitoring OH∗, CH∗ and CO_2∗. Experiments were performed over a range of conditions: Φ = 0.5–2; T_5 = 1178–1602 K; and P_5 = 173–416 kPa. A tentative detailed reaction model, which includes sub-mechanisms for the three measured excited species, was developed to describe the high-temperature chemical kinetics of acrolein oxidation. Reasonable agreement was found between the model prediction and experimental data
