    Outlier identification in radiation therapy knowledge-based planning: A study of pelvic cases.

    PURPOSE: The purpose of this study was to apply statistical metrics to identify outliers and to investigate the impact of outliers on knowledge-based planning in radiation therapy of pelvic cases. We also aimed to develop a systematic workflow for identifying and analyzing geometric and dosimetric outliers. METHODS: Four groups (G1-G4) of pelvic plans were sampled in this study. These include the following three groups of clinical IMRT cases: G1 (37 prostate cases), G2 (37 prostate plus lymph node cases) and G3 (37 prostate bed cases). Cases in G4 were planned in accordance with dynamic-arc radiation therapy procedure and include 10 prostate cases in addition to those from G1. The workflow was separated into two parts: 1. identifying geometric outliers, assessing outlier impact, and outlier cleaning; 2. identifying dosimetric outliers, assessing outlier impact, and outlier cleaning. G2 and G3 were used to analyze the effects of geometric outliers (first experiment outlined below) while G1 and G4 were used to analyze the effects of dosimetric outliers (second experiment outlined below). A baseline model was trained by regarding all G2 cases as inliers. G3 cases were then individually added to the baseline model as geometric outliers. The impact on the model was assessed by comparing leverages of inliers (G2) and outliers (G3). A receiver-operating-characteristic (ROC) analysis was performed to determine the optimal threshold. The experiment was repeated by training the baseline model with all G3 cases as inliers and perturbing the model with G2 cases as outliers. A separate baseline model was trained with 32 G1 cases. Each G4 case (dosimetric outlier) was subsequently added to perturb the model. Predictions of dose-volume histograms (DVHs) were made using these perturbed models for the remaining 5 G1 cases. A Weighted Sum of Absolute Residuals (WSAR) was used to evaluate the impact of the dosimetric outliers. RESULTS: The leverage of inliers and outliers was significantly different. The Area-Under-Curve (AUC) for differentiating G2 (outliers) from G3 (inliers) was 0.98 (threshold: 0.27) for the bladder and 0.81 (threshold: 0.11) for the rectum. For differentiating G3 (outlier) from G2 (inlier), the AUC (threshold) was 0.86 (0.11) for the bladder and 0.71 (0.11) for the rectum. Significant increase in WSAR was observed in the model with 3 dosimetric outliers for the bladder (P \u3c 0.005 with Bonferroni correction), and in the model with only 1 dosimetric outlier for the rectum (P \u3c 0.005). CONCLUSIONS: We established a systematic workflow for identifying and analyzing geometric and dosimetric outliers, and investigated statistical metrics for outlier detection. Results validated the necessity for outlier detection and clean-up to enhance model quality in clinical practice

    Enhanced acupuncture therapy for radiotherapy-related neuropathic pain in patients with gynecologic cancer: a report of two cases and brief review

    As radiation therapy is increasingly utilized in the treatment of cancer, neuropathic pain (NP) is a common radiotherapy-related adverse effect and has a significant impact on clinical outcomes negatively. However, despite an improved understanding of neuropathic pain management, pain is often undertreated in patients with cancer. Herein, we reported two cases with radiotherapy-related neuropathic pain (RRNP) who presented a positive reaction to acupuncture. Patient 1 (a 73-year-old woman) with gynecologic cancer complained of burning and electric shock-like pain in the lower limb after radiotherapy. With the accepted combination of acupuncture and drugs, the pain was alleviated completely in 8 weeks. Patient 2 (a 64-year-old woman) accepted acupuncture in the absence of medication because of her inability to tolerate the adverse events of anticonvulsant drugs. She achieved remission of pain 4 weeks later. The results of this study showed that acupuncture might be promising for controlling the RRNP in patients with cancer, especially who were intolerant or unresponsive to medications

The asymmetric unit consists of one half-mol­ecule. The Na atom is located on a crystallographic centre of inversion. The six-coordinate Na atom adopts a distorted octa­hedral coordination. In the crystal structure, inter­molecular N—H⋯I and O—H⋯N hydrogen bonds link the mol­ecules into a two-dimensional network

    Research on work zone vehicle queuing behavior based on cellular automata

    A model is proposed to estimate the work zone queue length, and the cellular automata based on empirical data is used for model validation. This estimation model can be applied to work zone organization and management to improve work zone capacity and security. Relationship between the average queue length and the warning zone length can be found, and the appropriate warning zone length can be determined according to design flow. Moreover, the appropriate work zone lane-changing strategies under different design flows are found through the estimation model

    Nurses\u27 Workplace Social Capital and the Influence of Transformational Leadership: A Theoretical Perspective

    Workplace social capital is the relational network, created by respectful interactions among members of a workforce, can contribute to the formation of a wholesome psychological work environment in an organization. Nurses\u27 workplace social capital is a derivative of the workplace social capital, formed because of the complex interactions among the nursing and between the other healthcare professionals. Transformational leadership is a style of leadership that addresses the emotional wellbeing of its workforce and inspires shared group ethics, norms, and goals. The philosophy of transformational leadership is grounded on the premise of workforce as human beings with specific needs. Transformational leadership has been confirmed as a strong predictor of nurses\u27 workplace social capital. Meanwhile, it is of an academic and/or healthcare industry operational value to scholarly assess and discern the theoretical influence of transformational leadership on nurses\u27 workplace social capital. In this paper, we have attempted to explore the associations between transformational leadership and nurses\u27 workplace social capital from a theoretical perspective. We have discussed the importance of each sub-dimension of transformational leadership (modeling the way, inspiring a shared vision, challenging the process, enabling others to act and encouraging the heart) in building up the social capital relational network. Finally, we have proposed a graphic framework of our analysis to facilitate understanding of the associations between the transformational leadership and nurses\u27 workplace social capital, in formation of a healthy work environment which is the foundation for efficiency and productivity of the workforce

    Impact of diurnal temperature fluctuations on larval settlement and growth of the reef coral Pocillopora damicornis

    Diurnal fluctuations in seawater temperature are ubiquitous on tropical reef flats. However, the effects of such dynamic temperature variations on the early stages of corals are poorly understood. In this study, we investigated the responses of larvae and new recruits of Pocillopora damicornis to two constant temperature treatments (29 and 31 degrees C) and two diurnally fluctuating treatments (28-31 and 30-33 degrees C with daily means of 29 and 31 degrees C, respectively) simulating the 3 degrees C diel oscillations at 3m depth on the Luhuitou fringing reef (Sanya, China). Results showed that the thermal stress on settlement at 31 degrees C was almost negated by the fluctuating treatment. Further, neither elevated temperature nor temperature fluctuations caused bleaching responses in recruits, while the maximum excitation pressure over photosystem II (PSII) was reduced under fluctuating temperatures. Although early growth and development were highly stimulated at 31 degrees C, oscillations of 3 degrees C had little effects on budding and lateral growth at either mean temperature. Nevertheless, daytime encounters with the maximum temperature of 33 degrees C in fluctuating 31 degrees C elicited a notable reduction in calcification compared to constant 31 degrees C. These results underscore the complexity of the effects caused by diel temperature fluctuations on early stages of corals and suggest that ecologically relevant temperature variability could buffer warming stress on larval settlement and dampen the positive effects of increased temperatures on coral growth
