Erythrocyte membrane fatty acid profile and saturation index in gallbladder carcinogenesis: a case-control study
BACKGROUND: Gallbladder cancer is a common neoplasm of biliary tract, with an unknown etiology. PATIENTS AND METHODS: This study was carried out to evaluate the changes in the membrane fatty acid profile and saturation index in patients with gallbladder cancer. The study sample consisted of 50 newly diagnosed cases of gallbladder cancer and 50 patients undergoing cholecystectomy for cholelithiasis were recruited as controls. The fatty acid estimation was carried out by high performance liquid chromatography (HPLC). Statistical analysis was carried out by student 't' test and one-way ANOVA. Pearson's correlation coefficient was also obtained. RESULTS: A significant lowering of erythrocyte membrane stearic acid (p = 0.000), arachidonic acid (p = 0.001), and saturation index (p = 0.001) was observed in patients with gallbladder cancer compared to controls. A significant inverse relation was observed between stearic and oleic acid (r = -0.269 p = 0.007). CONCLUSION: Results of the present study suggest an increase in the delta 9 desaturation in cancer patients compared to controls however, a possible role of biliary stasis due to altered gallbladder motility or derangements of signal transduction secondary to altered membrane lipid bilayer cannot be ruled out
Effect of glimepiride compared with glibenclamide on post prandial blood sugar in type II diabetes mellitus patients
Background: Diabetes mellitus is a major public health problem with many complications. The Global Burden of Diabetes Study has projected that there will be a 122% increase in the number of people with diabetes mellitus worldwide in 2025 compared to 1995. Despite the availability of new agents for the treatment of type 2 diabetes mellitus oral sulfonylureas remain a cornerstone of therapy.Glibenclamide and glimepiride are widely used sulfonylurea antidiabetic drugs.Methods: A randomized, open, parallel group study was conducted by the Department of Pharmacology in association with Department of Medicine at Maharishi Markandeshwar Institute of Medical Sciences & Research (MMIMSR), Mullana, Ambala. Total 50 patients were divided into two groups I & II. In Group I (n=25) glibenclamide (5-15 mg/day) & in Group II (n=25) glimepiride (1-6mg/day) was administered for a period of 24 weeks and data analyzed by Student’s “t”- test.Results: There was a significant improvement in the post prandial blood sugar score (p0.05) between the two groups.Conclusions: Both the drugs glibenclamide and glimepiride were effectively reduced post prandial blood sugar in both the groups. But these sulfonylurea drugs lowered post prandial blood sugar to a similar degree without significant difference between the two groups
Contemporary spatial association of genetic diversity determinants in Asian Dipterocarps: a systematic review
Considering the research gaps and areas to be prioritized specifically in the forestry research sector with stress given on conservation genetics and tree improvement, we make an effort to understand the spatial patterns and identify the key determinants, which produce major effects on genetic diversity of Asian Dipterocarps. This review focuses on identifying patterns and establishing relationships between genecological parameters derived on the basis of molecular markers with factors, such as geographical range, vertical profile and IUCN categories along with recognizing research gaps pertaining to operational forestry and terrestrial ecosystems.
Corresponding to 47 research papers, meta-analysis of 50 species under subfamily Dipterocarpoideae revealed significant differences in genetic parameters, namely expected heterozygosity (HE) and number of alleles per locus (NA), for most genera and factors. These parameters showed significant correlations with vertical strata (rk=-0.241; p<0.05) and altitude (rs=0.283; p<0.01). However, on the basis of co-dominant and dominant markers, the parameters exhibited contrasting results for the species’ characteristics. Further, pollen exchange and seed dispersal predominantly explained the genetic variations among the contributing factors, generally believed to be correlated with vertical strata and geographical range of the species. Conclusively, two major clusters were formed through principal component analysis (PCA), where HE and NA were the main deciding factors. Anthropogenic interferences, viz. forest fragmentation and deforestation found to be subsidising major impacts, which increase inbreeding and genetic drift, causing the loss of rare alleles and consequently, decreasing genetic variation. The study emphasizes the importance of genecological conservation and access to diverse genetic resources information, which will ensure global forest conservation and climate change mitigation network for sustainable development
Development and validation of dissolution method for carvedilol compression-coated tablets
The present study describes the development and validation of a dissolution method for carvedilol compression-coated tablets. Dissolution test was performed using a TDT-06T dissolution apparatus. Based on the physiological conditions of the body, 0.1N hydrochloric acid was used as dissolution medium and release was monitored for 2 hours to verify the immediate release pattern of the drug in acidic pH, followed by pH 6.8 in citric-phosphate buffer for 22 hours, to simulate a sustained release pattern in the intestine. Influences of rotation speed and surfactant concentration in medium were evaluated. Samples were analysed by validated UV visible spectrophotometric method at 286 nm. 1% sodium lauryl sulphate (SLS) was found to be optimum for improving carvedilol solubility in pH 6.8 citric-phosphate buffer. Analysis of variance showed no significant difference between the results obtained at 50 and 100 rpm. The discriminating dissolution method was successfully developed for carvedilol compression-coated tablets. The conditions that allowed dissolution determination were USP type I apparatus at 100 rpm, containing 1000 ml of 0.1N HCl for 2 hours, followed by pH 6.8 citric-phosphate buffer with 1% SLS for 22 hours at 37.0 ± 0.5 ºC. Samples were analysed by UV spectrophotometric method and validated as per ICH guidelines.O presente estudo descreve o desenvolvimento e a validação de método de dissolução para comprimidos revestidos de carvedilol. O teste de dissolução foi efetuado utilizando-se o aparelho para dissolução TDT-06T. Com base nas condições fisiológicas do organismo, utilizou-se ácido clorídrico 0,1 N como meio de dissolução e a liberação foi monitorada por 2 horas para se verificar o padrão de liberação imediata do fármaco em condições de pH baixo, seguidas por pH 6,8 em tampão cítrico-fosfato por 22 horas, para simular o padrão de liberação controlada no intestino. Avaliou-se a influência da velocidade de rotação e a concentração de tensoativo no meio. As amostras foram analisadas por método espectrofotométrico UV-visível validado, em 286 nm. O laurilsulfato sódico a 1% (SLS) mostrou-se ótimo para aumentar a solubilidade do carvedilol em pH 6,8 em tampão cítrico-fosfato. A análise da variância não mostrou diferença significativa entre os resultados obtidos a 50 e a 100 rpm. O método da dissolução discriminante foi desenvolvido com sucesso para os comprimidos revestidos de carvedilol. As condições que permitiram a determinação da dissolução foram: aparelho USP tipo I a 100 rpm, contendo 1000 mL de HCL 0,1 N por 2 horas, seguido de pH 6,8 com tampão cítrico-fosfato, com 1% de SLS por 22 horas a 37,0 ± 0,5 ºC. Amostras foram analisadas por método espectrofotométrico e validadas pelas normas ICH
Intussusception due to non Hodgkin’s lymphoma; different experiences in two children: two case reports
In children, non-Hodgkin’s lymphoma has been found to be the lead point in intussusception involving the terminal ileum. We present here two cases of ileal primary non-Hodgkin’s lymphoma which presented as intussusception, highlighting the differing presentations of these children and their outcome, with a brief review of the literature
Случай раннего выявления метастазов в бедренной кости у больного раком полового члена
Background. Bone metastasis is very common in the advanced stage of numerous carcinomas. In penile carcinoma, lymph nodes metastasis is somehow common but it is very rare reported to be secondary from penile cancer. till the date, there are only few cases of penis carcinoma reported bone metastasis in literature worldwide.Case Presentation. Herein, We presented a 51-year-old Nepalese male with squamous cell carcinoma of penis. computed tomography (ct) scan of the patient revealed that there was carcinoma involving glans penis and precure with bilateral external & internal inguinal lymphadenopathies. After then, the patient was under gone for partial penectomy and bilateral inguinal lymphadenectomy and complete 6-cycle chemotherapy. After one year of treatment, patient developed thigh pain and headache and he advised to have magnetic Resonance imaging (mRi) of brain, 99mTc-MDP whole body bone scan and ct scan of pelvis and thigh. The examination report reveals that there was a sclerotic change in vertex of skull bone and moderate 99mTc-MDP uptake in right proximal shaft of femur just below the neck d/d metastasis. The histopathological examination of the true cut biopsy taken from the lesion of the femur showed metastatic keratinizing squamous cell carcinoma which is rare case of femoral shaft bone metastasis secondary from penile carcinoma. Then patient was sent for surgical reconstruction of femur. Based on the case studies review femur shaft bone metastasis from penile cancer is extremely rare.Conclusion. The best of our knowledge; this is the first early detected bone metastases to shaft of the femur in a patient with penile cancer. early diagnosis helps to radical treatment as well as palliative treatment. surgery is the preferred option of the treatments, especially for metastatic foci in the long bones.Аннотация Метастазы в кости часто встречаются на поздних стадиях разных злокачественных новообразований. Для рака полового члена характерно лимфогенное метастазирование, костные метастазы встречаются очень редко. На данный момент в мировой литературе зарегистрировано лишь несколько случаев рака полового члена с метастазами в кости.Описание клинического случая. Мы представляем описание клинического случая плоскоклеточного рака полового члена у 51-летнего мужчины из Непала. Компьютерная томография (КТ) выявила рак полового члена, локализованный на головке и крайней плоти с двусторонней паховой лимфаденопатией. Пациенту выполнена частичная пенэктомия, двусторонняя паховая лимфаденэктомия, проведено 6 курсов адъювантной химиотерапии. Через один год после завершения лечения у пациента появились боли в бедре и головная боль, рекомендована МРТ головного мозга, сканирование костей скелета с 99mTc-MDP и КТ таза и бедра. Обследование показало наличие склеротических изменений в теменной кости черепа и умеренное накопление 99mTc-MDP в проксимальном отделе правом бедренной кости ниже шейки. Гистологическое исследование биоптата, взятого из очага в бедренной кости, выявило метастаз ороговевающего плоскоклеточного рака, что является редким случаем метастазирования рака полового члена в диафиз бедренной кости. Пациент был направлен на хирургическую реконструкцию бедренной кости. Согласно обзору клинических исследований, метастазы в диафизе бедренной кости при раке полового члена встречаются крайне редко.Заключение. Представлен первый случай раннего выявления костных метастазов в диафизе бедренной кости у пациента с раком полового члена. Ранняя диагностика помогает как радикальному, так и паллиативному лечению. Хирургическое вмешательство является предпочтительным вариантом лечения, особенно при метастатических очагах в длинных костях

BLOOM: A 176B-Parameter Open-Access Multilingual Language Model
Large language models (LLMs) have been shown to be able to perform new tasks
based on a few demonstrations or natural language instructions. While these
capabilities have led to widespread adoption, most LLMs are developed by
resource-rich organizations and are frequently kept from the public. As a step
towards democratizing this powerful technology, we present BLOOM, a
176B-parameter open-access language model designed and built thanks to a
collaboration of hundreds of researchers. BLOOM is a decoder-only Transformer
language model that was trained on the ROOTS corpus, a dataset comprising
hundreds of sources in 46 natural and 13 programming languages (59 in total).
We find that BLOOM achieves competitive performance on a wide variety of
benchmarks, with stronger results after undergoing multitask prompted
finetuning. To facilitate future research and applications using LLMs, we
publicly release our models and code under the Responsible AI License
