Clinical review: Can we predict which patients are at risk of complications following surgery?
There are a vast number of operations carried out every year, with a small proportion of patients being at highest risk of mortality and morbidity. There has been considerable work to try and identify these high-risk patients. In this paper, we look in detail at the commonly used perioperative risk prediction models. Finally, we will be looking at the evolution and evidence for functional assessment and the

Real-time edge-enhanced optical correlator
The performance of five symbol lock detectors are compared. They are the square-law detector with overlapping (SQOD) and non-overlapping (SQNOD) integrators, the absolute value detectors with overlapping and non-overlapping (AVNOD) integrators and the signal power estimator detector (SPED). The analysis considers various scenarios when the observation interval is much larger or equal to the symbol synchronizer loop bandwidth, which has not been considered in previous analyses. Also, the case of threshold setting in the absence of signal is considered. It is shown that the SQOD outperforms all others when the threshold is set in the presence of signal, independent of the relationship between loop bandwidth and observation period. On the other hand, the SPED outperforms all others when the threshold is set in the presence of noise only
Large-amplitude chirped coherent phonons in tellurium mediated by ultrafast photoexcited carrier diffusion
We report femtosecond time-resolved reflectivity measurements of coherent
phonons in tellurium performed over a wide range of temperatures (3K to 296K)
and pump laser intensities. A totally symmetric A coherent phonon at 3.6
THz responsible for the oscillations in the reflectivity data is observed to be
strongly positively chirped (i.e, phonon time period decreases at longer
pump-probe delay times) with increasing photoexcited carrier density, more so
at lower temperatures. We show for the first time that the temperature
dependence of the coherent phonon frequency is anomalous (i.e, increasing with
increasing temperature) at high photoexcited carrier density due to
electron-phonon interaction. At the highest photoexcited carrier density of
1.4 10cm and the sample temperature of 3K, the
lattice displacement of the coherent phonon mode is estimated to be as high as
0.24 \AA. Numerical simulations based on coupled effects of optical
absorption and carrier diffusion reveal that the diffusion of carriers
dominates the non-oscillatory electronic part of the time-resolved
reflectivity. Finally, using the pump-probe experiments at low carrier density
of 6 10 cm, we separate the phonon anharmonicity to
obtain the electron-phonon coupling contribution to the phonon frequency and

Laparoscopic management of an endometrioma complicated by an ovarian abscess
An ovarian abscess is defined as a primary infection of the ovary without the involvement of the fallopian tube. The possible factors for the cause of ovarian abscess are: disruption of the ovarian capsule, giving access to bacteria, haematogenous and lymphatic spread. A woman with a ruptured ovarian abscess presents with features of diffuse peritonitis. An unruptured abscess is more difficult to diagnose. Unruptured abscess can be dealt with either laparotomy or laparoscopy. Ruptured ovarian abscess requires an aggressive and primarily surgical approach.
Treatment of multidrug-resistant tuberculosis in a remote, conflict-affected area of the Democratic Republic of Congo.
The Democratic Republic of Congo is a high-burden country for multidrug-resistant tuberculosis. Médecins Sans Frontières has supported the Ministry of Health in the conflict-affected region of Shabunda since 1997. In 2006, three patients were diagnosed with drug-resistant TB (DR-TB) and had no options for further treatment. An innovative model was developed to treat these patients despite the remote setting. Key innovations were the devolving of responsibility for treatment to non-TB clinicians remotely supported by a TB specialist, use of simplified monitoring protocols, and a strong focus on addressing stigma to support adherence. Treatment was successfully completed after a median of 24 months. This pilot programme demonstrates that successful treatment for DR-TB is possible on a small scale in remote settings
Study of the technique of stellar occultation
The results are reported of a study of the stellar occultation technique for measuring the composition of the atmosphere. The intensity of starlight was monitored during the occultation using the Wisconsin stellar ultraviolet photometers aboard the Orbiting Astronomical Observatory (OAO-A2). A schematic diagram of an occultation is shown where the change in intensity at a given wavelength is illustrated. The vertical projection of the attenuation region is typically 60 km deep for molecular oxygen and 30 km deep for ozone. Intensity profiles obtained during various occultations were analyzed by first determining the tangential columm density of the absorbing gases, and then Abel inverting the column densities to obtain the number density profile. Errors are associated with each step in the inversion scheme and have been considered as an integral part of this study
Transient quantum evolution of 2D electrons under photoexcitation of a deep center
We have considered the ballistic propagation of the 2D electron Wigner
distribution, which is excited by an ultrashort optical pulse from a
short-range impurity into the first quantized subband of a selectively-doped
heterostructure with high mobility. Transient ionization of a deep local state
into a continuum conduction c-band state is described. Since the quantum nature
of the photoexcitation, the Wigner distribution over 2D plane appears to be an
alternating-sign function. Due to a negative contribution to the Wigner
function, the mean values (concentration, energy, and flow) demonstrate an
oscillating transient evolution in contrast to the diffusive classical regime
