    Prevalence and concentration of pesticides in European waters: A protocol for systematic review and meta-analysis

    There is currently a growing interest in the so-called emerging pollutants, such as pesticides, pharmaceuticals, personal hygiene care products, drugs, etc., whose presence in natural ecosystems is not necessarily recent, but the development in latest years of new and more sensitive methods of analysis has allowed their detection. They can be present in the natural environment, food, and many products of everyday origin, which suggests that human exposure to them is massive and universal. Therefore, the study of this type of substances is becoming one of the priority lines of research of the main agencies dedicated to the protection of public and environmental health, such as the World Health Organization (WHO), United States Environmental Protection Agency (USEPA) or European Union (EU). In this sense, it is of vital importance to know the nature and quantity of this type of contaminants, to establish preventive mechanisms that minimize its presence in aquatic systems, with special requirements for human consumption. This study aimed to describe a protocol for a systematic review and meta-analysis to assess the status of pesticides in European waters. We will search for original studies in the PubMed/Medline, Scopus, Web of Science, EMBASE, ScienceDirect databases. Prevalence studies of emerging contaminants (pesticides) in water resources (watersheds, aquifers, rivers, marine and springs), wastewaters (influent and effluent), and drinking water should be included. Two reviewers will independently screen and assess the included studies, with any disagreements being resolved by a third reviewer. We will summarize the findings using a narrative approach and, if possible, conduct a quantitative synthesis (meta-analysis). We will conduct the protocol following the Preferred Reporting Items for Systematic Review and Meta-Analyses Protocols (PRISMA-P) guidelines. The review will summarize the current evidence on the presence of pesticides in European waters such as glyphosate, chlorpyrifos, pyrethroid pesticides, neonicotinoid pesticides, and/or fungicides, in samples of different water resources like wastewaters and drinking water. We expect that this systematic review will establish preventive mechanisms that minimize the presence of pesticides in water in the environment.Ciencias Ambientale

    Initiation and Single Dispensing in Cardiovascular and Insulin Medications: Prevalence and Explanatory Factors

    : Background: Adherence problems have negative effects on health, but there is little information on the magnitude of non-initiation and single dispensing. Objective: The aim of this study was to estimate the prevalence of non-initiation and single dispensation and identify associated predictive factors for the main treatments prescribed in Primary Care (PC) for cardiovascular disease (CVD) and diabetes. Methods: Cohort study with real-world data. Patients who received a first prescription (2013-2014) for insulins, platelet aggregation inhibitors, angiotensin-converting enzyme inhibitors (ACEI) or statins in Catalan PC were included. The prevalence of non-initiation and single dispensation was calculated. Factors that explained these behaviours were explored. Results: At three months, between 5.7% (ACEI) and 9.1% (antiplatelets) of patients did not initiate their treatment and between 10.6% (statins) and 18.4% (ACEI) filled a single prescription. Body mass index, previous CVD, place of origin and having a substitute prescriber, among others, influenced the risk of non-initiation and single dispensation. Conclusions: The prevalence of non-initiation and single dispensation of CVD medications and insulin prescribed in PC in is high. Patient and health-system factors, such as place of origin and type of prescriber, should be taken into consideration when prescribing new medications for CVD and diabetes

The majority of the adult population engages two 01 more risk behaviours, that is why a multiple intervention might be more effective and efficient The primary objectives are to evaluate the effectiveness, the cost effectiveness and an implementation strategy of a complex multiple risk intervention to promote healthy behaviours in people between 45 to 75 years attended in PHC. CDATA[CDATA[Methods: This study is a cluster randomised controlled hybrid type 2 trial with two parallel groups comparing a complex multiple risk behaviour intervention with usual care It will be carried out in 26 PHC centres in Spam The study focuses on people between 45 and 75 years who carry out two or more of the following unhealthy behaviours tobacco use, low adherence to the Mediterranean dietary pattern or insufficient physical activity level The intervention is based on the Transtheoretical Model and it will be made by physicians and nurses in the routine care of PHC practices according to the conceptual framework of the ''5A''s" It will have a maximum duration of 12 months and it will be carried out to three different levels (individual, group and community) Incremental cost per quality adjusted life year gamed measured by the tanffs of the EuioQo! 5D questionnaire will be estimated. The implementation strategy is based on the ''Consolidated Framework for Implementation Research, a set of discrete implementation strategies and an evaluation framework.     Fish Oil Enriched Intravenous Lipid Emulsions Reduce Triglyceride Levels in Non-Critically Ill Patients with TPN and Type 2 Diabetes. A Post-Hoc Analysis of the INSUPAR Study

    There are no studies that have specifically assessed the role of intravenous lipid emulsions (ILE) enriched with fish oil in people with diabetes receiving total parenteral nutrition (TPN). The objective of this study was to assess the metabolic control (glycemic and lipid) and in-hospital complications that occurred in non-critically ill inpatients with TPN and type 2 diabetes with regard to the use of fish oil emulsions compared with other ILEs. We performed a post-hoc analysis of the Insulin in Parenteral Nutrition (INSUPAR) trial that included patients who started with TPN for any cause and that would predictably continue with TPN for at least five days. The study included 161 patients who started with TPN for any cause. There were 80 patients (49.7%) on fish oil enriched ILEs and 81 patients (50.3%) on other ILEs. We found significant decreases in triglyceride levels in the fish oil group compared to the other patients. We did not find any differences in glucose metabolic control: mean capillary glucose, glycemic variability, and insulin dose, except in the number of mild hypoglycemic events that was significantly higher in the fish oil group. We did not observe any differences in other metabolic, liver or infectious complications, in-hospital length of stay or mortality

    Realizar una propuesta de perfeccionamiento docente a partir de los resultados recogidos con el cuestionario de evaluación del profesorado de la Universidad de Murcia (CEPUM). Conseguir que los resultados de evaluación del profesorado sirvan como recurso formativo de enriquecimiento profesional.. 3335 evaluaciones de profesor/asignatura realizadas durante los bienios 1991-92 y 1993-94. 26 profesores de Psicología, 112 profesores de Veterinaria, 32 profesores de Informática, todos ellos de la Universidad de Murcia.. Se realiza una revisión de la situación de la efectividad en la enseñanza desde el punto de vista de los alumnos. Se presenta el cuestionario para la evaluación del profesorado de la Universidad de Murcia (CEPUM) y se realiza un análisis psicométrico que muestra su validez y fiabilidad. Para conocer las dimensiones de la efectividad docente según los alumnos se realiza un análisis exploratorio por ciclo evaluativo, empleando el paquete informático SYSTAT. El modelo general de efectividad docente obtenido se contrasta con modelos alternativos mediante un análisis factorial confirmatorio. Se realiza un análisis de perfiles multivariable sobre una muestra de profesores de distintos centros (Psicología, Veterinaria, Informática) que imparten una misma asignatura en momentos diferentes.. Cuestionario de evaluación del profesorado de la Universidad de Murcia (CEPUM), paquete informático SYSTAT 5.0.. Coeficiente Alpha de Cronbach.. En el análisis de la dimensionalidad se comprueba la existencia de tres dimensiones: 1. Dominio conceptual, preparación y recursos pedagógicos, 2. Interacción profesor/alumno y motivación, 3. Planificación y organización de la docencia.     Origen anómalo de la arteria coronaria derecha del seno aórtico izquierdo

    Se presenta el caso de un paciente de 32 años que presentó síntomas de fibrilación ventricular mientras estaba practicando una maratón. El estudio diagnóstico puso de manifiesto la existencia de un origen anómalo de la arteria coronaria derecha en el seno coronario izquierdo, con trayecto no intramural entre la aorta y la arteria pulmonar. La reparación quirúrgica consistió en la realización de una anastomosis laterolateral de la coronaria derecha al seno aórtico derecho. El postoperatorio del paciente transcurrió con normalidad. El origen aórtico anómalo de las coronarias es una alteración congénita rara asociada a un mayor riesgo de muerte súbita. En muchos casos la enfermedad es asintomática hasta producirse el infarto o la arritmia ventricular. Cuando la coronaria sigue un trayecto intramural la corrección mediante el unroofing es la técnica más empleada. Si no hay trayecto intramural y la translocación no es posible, la técnica descrita constituye una alternativa válida

    MINECO/CGL-2012-40043-C02-01Our study focuses on the Iberian ibex (Capra pyrenaica) from the Sierra Nevada Natural Space (southern Spain), where sarcoptic mange is an endemic disease and animals are affected by a highly seasonal environment. Our aim was to distinguish between disease and environmental influences on seasonal variation in body weight, hematology, and serum biochemistry in Iberian ibex. We sampled 136 chemically immobilized male ibexes. The single effect of mange influenced hemoglobin, hematocrit, mean corpuscular volume, leukocytes, band neutrophils, monocytes, cholesterol, urea, creatine, and aspartate aminotransferase. Both mange and the period of the year also affected values of mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration, neutrophils, glucose, and serum proteins. Scabietic animals showed a marked reduction in body weight (21.4 kg on average), which was more pronounced in winter. These results reveal that 1) infested animals are anemic, 2) secondary infections likely occur, and 3) sarcoptic mange is catabolic
