Fetal programming and gestational diabetes mellitus
Gestational diabetes mellitus is defined by new-onset glucose intolerance during pregnancy. About 2–5% of all pregnant women develop gestational diabetes during their pregnancies and the prevalence has increased considerably during the last decade. This metabolic condition is manifested when pancreatic β-cells lose their ability to compensate for increased insulin resistance during pregnancy, however, the pathogenesis of the disease remains largely unknown. Gestational diabetes is strongly associated with adverse pregnancy outcome as well as with long-term adverse effects on the offspring which likely occurs due to epigenetic modifications of the fetal genome. In the current review we address gestational diabetes and the short and long term complications for both mothers and offspring focusing on the importance of fetal programming in conferring risk of developing diseases in adulthood
Gingival crevicular placental alkaline phosphatase is an early pregnancy biomarker for pre-eclampsia
Early and innovative diagnostic strategies are required to predict the risk of developing pre-eclampsia (PE). The purpose of this study was to evaluate the performance of gingival crevicular fluid (GCF) placental alkaline phosphatase (PLAP) concentrations to correctly classify women at risk of PE. A prospectively collected, retrospectively stratified cohort study was conducted, with 412 pregnant women recruited at 11–14 weeks of gestation. Physical, obstetrical, and periodontal data were recorded. GCF and blood samples were collected for PLAP determination by ELISA assay. A multiple logistic regression classification model was developed, and the classification efficiency of the model was established. Within the study cohort, 4.3% of pregnancies developed PE. GCF-PLAP concentration was 3-to 6-fold higher than in plasma samples. GCF-PLAP concentrations and systolic blood pressure were greater in women who developed PE (p = 0.015 and p \u3c 0.001, respectively). The performance of the multiparametric model that combines GCF-PLAP concentration and the levels of systolic blood pressure (at 11–14 weeks gestation) showed an association of systolic blood pressure and GCF-PLAP concentrations with the likelihood of developing PE (OR:1.07; 95% CI 1.01–1.11; p = 0.004 and OR:1.008, 95% CI 1.000–1.015; p = 0.034, respectively). The model had a sensitivity of 83%, a specificity of 72%, and positive and negative predictive values of 12% and 99%, respectively. The area under the receiver operating characteristic (AUC-ROC) curve was 0.77 and correctly classified 72% of PE pregnancies. In conclusion, the multivariate classification model developed may be of utility as an aid in identifying pre-symptomatic women who subsequently develop PE
Maternal exposure to a high-magnitude earthquake during pregnancy influences pre-reading skills in early childhood
Exposure to an adverse prenatal environment can influence fetal development and result in long-lasting changes in the offspring. However, the association between maternal exposure to stressful events during pregnancy and the achievement of pre-reading skills in the offspring is unknown. Here we examined the association between prenatal exposure to the Chilean high-magnitude earthquake that occurred on February 27th, 2010 and the development of early reading precursors skills (listening comprehension, print knowledge, alphabet knowledge, vocabulary, and phonological awareness) in children at kindergarten age. This multilevel retrospective cohort study including 3280 children, of whom 2415 were unexposed and 865 were prenatally exposed to the earthquake shows substantial evidence that maternal exposure to an unambiguously stressful event resulted in impaired pre-reading skills and that a higher detrimental effect was observed in those children who had been exposed to the earthquake during the first trimester of gestation. In addition, females were more significantly affected by the exposure to the earthquake than their male peers in alphabet knowledge; contrarily, males were more affected than females in print knowledge skills. These findings suggest that early intervention programs for pregnant women and/or children exposed to prenatal stress may be effective strategies to overcome impaired pre-reading skills in children
NFAT5 Is Activated by Hypoxia: Role in Ischemia and Reperfusion in the Rat Kidney
The current hypothesis postulates that NFAT5 activation in the kidney's inner medulla is due to hypertonicity, resulting in cell protection. Additionally, the renal medulla is hypoxic (10–18 mmHg); however there is no information about the effect of hypoxia on NFAT5. Using in vivo and in vitro models, we evaluated the effect of reducing the partial pressure of oxygen (PO2) on NFAT5 activity. We found that 1) Anoxia increased NFAT5 expression and nuclear translocation in primary cultures of IMCD cells from rat kidney. 2) Anoxia increased transcriptional activity and nuclear translocation of NFAT5 in HEK293 cells. 3) The dose-response curve demonstrated that HIF-1α peaked at 2.5% and NFAT5 at 1% of O2. 4) At 2.5% of O2, the time-course curve of hypoxia demonstrated earlier induction of HIF-1α gene expression than NFAT5. 5) siRNA knockdown of NFAT5 increased the hypoxia-induced cell death. 6) siRNA knockdown of HIF-1α did not affect the NFAT5 induction by hypoxia. Additionally, HIF-1α was still induced by hypoxia even when NFAT5 was knocked down. 7) NFAT5 and HIF-1α expression were increased in kidney (cortex and medulla) from rats subjected to an experimental model of ischemia and reperfusion (I/R). 7) Experimental I/R increased the NFAT5-target gene aldose reductase (AR). 8) NFAT5 activators (ATM and PI3K) were induced in vitro (HEK293 cells) and in vivo (I/R kidneys) with the same timing of NFAT5. 8) Wortmannin, which inhibits ATM and PI3K, reduces hypoxia-induced NFAT5 transcriptional activation in HEK293 cells. These results demonstrate for the first time that NFAT5 is induced by hypoxia and could be a protective factor against ischemic damage
Preterm labour: Association between labour physiology, tocolysis and prevention
Introduction: In developed countries, preterm birth is the major cause of perinatal morbidity, mortality and the most important public health problem in the obstetric field. In the past decades, an increasing trend has been observed regardless of the great efforts focussed on the improvement of our understanding of the physiopathological mechanisms behind preterm labour (PTL) and the improvement in the use of tocolytic drugs. Areas covered: In this review, the authors focus on some points of the physiopathology of labour in order to understand the rationality behind the different management approaches developed for the PTL syndrome. Expert opinion: There is a need to develop new tools for the treatment of patients with PTL. Research focussed on improving tocolysis, the physiology of labour and pathological processes involved in PTL would afford new approaches for the treatment of PTL, allowing clinicians to provide integrative solutions for this multifactorial disease. Recently, the prophylactic use of progesterone pessary and cerclage in women with high risk of premature labour has been reported to reduce the incidence of premature births and improve neonatal outcomes. These results highlight the importance of prediction models in order to establish preventative strategies early in pregnancy.</p
Suplementación prenatal con omega 3 y su efecto en complicaciones durante el embarazo
During pregnancy, omega 3 supplementation has raised its popularity due to evidence that it would show not only benefits in the neural and visual development of the unborn child, but also in the prevention of obstetrical pathologies associated with of perinatal morbidity and mortality. Omega 3 polyunsaturated fatty acids (PUFAs), specifically, docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA), possess anti-inflammatory, vasodilatory and anti-aggregating properties, which have led to the use of PUFAs in the prevention of cardiovascular diseases. In this review, we detail the effects of omega 3 supplementation on different aspects of pregnancy such as prevention of preterm birth, pre-eclampsia, postpartum depression, and improved metabolism during gestational diabetes. Although there are several randomized clinical trials using omega-3 supplementation during pregnancy, the evidence remains inconclusive, due to variability in dosage and administration time. Certainly, a greater number of high-quality studies including randomized clinical trials are necessary to determine the impact of omega 3 supplementation during pregnancy in the prevention of obstetric pathologiesDurante los últimos años se ha incentivado la suplementación con omega 3 durante el embarazo principalmente debido a la evidencia que mostraría beneficios en el desarrollo neuronal y visual del hijo en gestación, y a la prevención de patologías obstétricas asociadas a un aumento de la morbi-mortalidad perinatal. Los ácidos grasos poliinsaturados (PUFAs) omega 3, específicamente el ácido eicosapentaenoico (EPA) y el ácido docosahexaenoico (DHA), poseen propiedades antiinflamatorias, vasodilatadoras, además de propiedades anti-agregantes, las cuales han estimulado el uso de PUFAs en la prevención de enfermedades cardiovasculares. En esta revisión detallamos los efectos de la suplementación con omega 3 en diferentes aspectos del embarazo tales como la prevención del parto prematuro, preeclampsia, depresión post-parto y mejora del metabolismo durante la diabetes gestacional. Si bien existen diversos ensayos clínicos randomizados que estudian la suplementación con omega 3 durante la gestación, la evidencia sigue siendo no concluyente, debido a la variabilidad de las dosis y tiempo de administración. Ciertamente, un mayor número de estudios de calidad son necesarios para determinar el real impacto de la suplementación con omega 3 durante la gestación en la prevención de patologías obstétrica
Increased Circulating Levels of PCSK9 and Pro-Atherogenic Lipoprotein Profile in Pregnant Women with Maternal Supraphysiological Hypercholesterolemia
Maternal physiological hypercholesterolemia (MPH) occurs during pregnancy to assure fetal development. Some pregnant women develop maternal supraphysiological hypercholesterolemia (MSPH) characterized by increased levels of low-density lipoprotein (LDL). We aim to determine if proprotein convertase subtilisin/kexin type 9 (PCSK9) levels (a protein that regulate the availability of LDL receptor in the cells surface), as well as the composition and function of LDL, are modulated in MSPH women. This study included 122 pregnant women. Maternal total cholesterol (TC), LDL, triglycerides and PCSK9 increased from first (T1) to third trimester (T3) in MPH women. At T3, maternal TC, LDL, PCSK9 and placental abundances of PCSK9 were significantly higher in MPSH compared to MPH. Circulating PCSK9 levels were correlated with LDL at T3. In MSPH women, the levels of lipid peroxidation and oxidized LDL were significantly higher compared to MPH. LDL isolated from MSPH women presented significantly higher triglycerides and ApoB but lower levels of ApoAI compared to MPH. The formation of conjugated dienes was earlier in LDL from MSPH and in endothelial cells incubated with these LDLs; the levels of reactive oxygen species were significantly higher compared to LDL from MPH. We conclude that increased maternal PCSK9 would contribute to the maternal elevated levels of pro-atherogenic LDL in MSPH, which could eventually be related to maternal vascular dysfunction
Pathogenesis of Preeclampsia: The Genetic Component
Preeclampsia (PE) is
one of the main causes of maternal and fetal
morbidity and mortality in the world, causing
nearly 40% of births delivered before 35
weeks of gestation. PE begins with inadequate
trophoblast invasion early in pregnancy, which
produces an increase in oxidative stress
contributing to the development of systemic
endothelial dysfunction in the later phases of
the disease, leading to the characteristic
clinical manifestation of PE. Numerous methods
have been used to predict the onset of PE with
different degrees of efficiency. These methods
have used fetal/placental and maternal markers
in different stages of pregnancy. From an
epidemiological point of view, many studies have
shown that PE is a disease with a strong
familiar predisposition, which also varies
according to geographical, socioeconomic, and
racial features, and this information can be
used in the prediction process. Large amounts of
research have shown a genetic association with a
multifactorial polygenic inheritance in the
development of this disease. Many biological
candidate genes and polymorphisms have been
examined in their relation with PE. We will
discuss the most important of them, grouped
by the different pathogenic mechanisms involved
in PE
¿MEJORA EL USO DE ÁCIDO URSODEOXICÓLICO EL PRONÓSTICO PERINATAL EN MUJERES EMBARAZADAS CON COLESTASIA INTRAHEPÁTICA DEL EMBARAZO?
La colestasia intrahepática del embarazo (CIÉ) es un cuadro clínico caracterizado por prurito palmo plantar de predominio nocturno y elevación de ácidos biliares conjugados séricos en el tercer trimestre del embarazo. Esta patología puede asociarse a ictericia y complicaciones como la muerte fetal. Aunque el manejo obstétrico de la CIÉ es eminentemente clínico, el ácido ursodeoxicólico (UDCA) ha sido utilizado efectivamente en el tratamiento sintomático de esta patología y en la corrección de los marcadores bioquímicos de la enfermedad. Además se ha sugerido que su uso estaría asociado a una disminución de las complicaciones fetales. Esta revisión tiene por objeto verificar la validez del uso de UDCA para mejorar el pronóstico fetal. Para esto se realizó una búsqueda detallada de la bibliografía médica en diferentes bases de datos. Aplicando distintos criterios se seleccionaron finalmente 4 artículos que constituyen la base de esta revisión. Fundamentados en la evidencia, se concluye que no existen datos suficientes en la literatura que apoyen el uso de UDCA para mejorar en forma efectiva los resultados perinatales. A pesar de que algunos estudios evaluados proponen un mejor desenlace fetal en pacientes tratados con UDCA, éstos no cuentan con la validez y el poder estadístico necesario para modificar la conducta actual frente a esta patologíaIntrahepatic cholestasis of pregnancy (ICP) is a disease of the third trimester of pregnancy involving pruritus and elevated bile acid levels. Once thought to be benign for both mother and fetus, ICP has been associated with maternal jaundice and increased rates of fetal morbidity and mortality. However ursodeoxycholic acid (UDCA) has proved to be effective and safe in patients with ICP, attenuating pruritus and correcting some biochemical abnormalities in the mothers. The fetal outcome has also been suggested to improve in patients receiving UDCA. This review intends to verify the validity of the UDCA uses to reduce fetal morbidity and mortality. For this a detailed search of the medical bibliography was done in different data bases. Applying different criteria, 4 articles were finally selected and this constitutes the base of our review. Based on the research, one concludes that there is not enough evidence in the literature to support the use of UDCA to improve perinatal outcome. Although some studies propose an improved fetal outcome in patient using UDCA, these do not count with the validity and the statistical power to modify the present management of IC
