    Loophole-free Bell test for continuous variables via wave and particle correlations

    We derive two classes of multi-mode Bell inequalities under local realistic assumptions, which are violated only by the entangled states negative under partial transposition in accordance with the Peres conjecture. Remarkably, the failure of local realism can be manifested by exploiting wave and particle correlations of readily accessible continuous-variable states, with very large violation of inequalities insensitive to detector-efficiency, which makes a strong case for a loophole-free test.Comment: 4 pages, published versio

    Purpose: To compare the clinical outcomes of reduced fluence (time-reduced) photodynamic therapy (PDT) and half-dose PDT in treating chronic central serous chorioretinopathy (CSC). Methods: Medical records of patients who underwent half-dose or reduced fluence PDT for chronic CSC were reviewed. The clinical outcomes of 34 eyes that underwent half-dose PDT and 39 eyes that underwent reduced fluence PDT were compared. Results: Six months after treatment, complete absorption of subretinal fluid was observed in 29 of 34 eyes (85.3%) in the half-dose PDT group and 36 of 39 eyes (92.3%) in the reduced fluence PDT group (p = 0.46). Twenty-one of 34 (61.8%) eyes in the half-dose PDT group and 27 of 39 (69.2%) eyes in the reduced fluence PDT group achieved visual improvement of more than one line in the Snellen chart (p = 0.62). Focal retinal pigment epithelium degeneration after treatment was noted in 7 of 27 eyes (25.9%) in the half-dose PDT group and 10 of 30 (33.3%) eyes in the reduced fluence PDT group (p = 0.58). Conclusions: Reduced fluence PDT showed a favorable outcome, similar to that of half-dose PDT, in treating chronic CSC. Because of its convenient preparation and application, reduced fluence (low-fluence) PDT might be an alternative treatment modality in treating chronic CSC

    Anti-Obesity and Anti-Adipogenic Effects of Chitosan Oligosaccharide (GO2KA1) in SD Rats and in 3T3-L1 Preadipocytes Models

    Excess body weight is a major risk factor for type 2 diabetes (T2D) and associated metabolic complications, and weight loss has been shown to improve glycemic control and decrease morbidity and mortality in T2D patients. Weight-loss strategies using dietary interventions produce a significant decrease in diabetes-related metabolic disturbance. We have previously reported that the supplementation of low molecular chitosan oligosaccharide (GO2KA1) significantly inhibited blood glucose levels in both animals and humans. However, the effect of GO2KA1 on obesity still remains unclear. The aim of the study was to evaluate the anti-obesity effect of GO2KA1 on lipid accumulation and adipogenic gene expression using 3T3-L1 adipocytes in vitro and plasma lipid profiles using a Sprague-Dawley (SD) rat model. Murine 3T3-L1 preadipocytes were stimulated to differentiate under the adipogenic stimulation in the presence and absence of varying concentrations of GO2KA1. Adipocyte differentiation was confirmed by Oil Red O staining of lipids and the expression of adipogenic gene expression. Compared to control group, the cells treated with GO2KA1 significantly decreased in intracellular lipid accumulation with concomitant decreases in the expression of key transcription factors, peroxisome proliferator-activated receptor gamma (PPARγ) and CCAAT/enhancer-binding protein alpha (CEBP/α). Consistently, the mRNA expression of downstream adipogenic target genes such as fatty acid binding protein 4 (FABP4), fatty acid synthase (FAS), were significantly lower in the GO2KA1-treated group than in the control group. In vivo, male SD rats were fed a high fat diet (HFD) for 6 weeks to induced obesity, followed by oral administration of GO2KA1 at 0.1 g/kg/body weight or vehicle control in HFD. We assessed body weight, food intake, plasma lipids, levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) for liver function, and serum level of adiponectin, a marker for obesity-mediated metabolic syndrome. Compared to control group GO2KA1 significantly suppressed body weight gain (185.8 ± 8.8 g vs. 211.6 ± 20.1 g, p \u3c 0.05) with no significant difference in food intake. The serum total cholesterol, triglyceride, and low-density lipoprotein (LDL) levels were significantly lower in the GO2KA1-treated group than in the control group, whereas the high-density lipoprotein (HDL) level was higher in the GO2KA1 group. The GO2KA1-treated group also showed a significant reduction in ALT and AST levels compared to the control. Moreover, serum adiponectin levels were significantly 1.5-folder higher than the control group. These in vivo and in vitro findings suggest that dietary supplementation of GO2KA1 may prevent diet-induced weight gain and the anti-obesity effect is mediated in part by inhibiting adipogenesis and increasing adiponectin level

    Erectile dysfunction in patients with liver disease related to chronic hepatitis B

    Background/AimsDespite sexual function making an important contribution to the quality of life, data on erectile function are relatively scant in patients with chronic liver disease. We evaluated the prevalence of and risk factors for erectile dysfunction (ED) in patients with liver disease related to hepatitis B, especially among those with chronic hepatitis B (CHB) or early-stage cirrhosis.MethodsIn total, 69 patients (35 with CHB and 34 with hepatitis-B-related liver cirrhosis [HBV-LC]) aged 40-59 years were analyzed. Child-Pugh classes of A and B were present in 30 (88.2%) and 4 (11.8%) of the patients with HBV-LC, respectively. The erectile function of the patients was evaluated using the Korean version of IIEF-5.ResultsThe prevalence of any ED was 24.6% for all patients, and 8.6% and 41.2% for those with CHB and HBV-LC, respectively (P=0.002). While there was only one (2.9%) CHB patient for each stage of ED, mild, moderate, and severe ED stages were seen in three (8.8%), one (2.9%), and ten (29.4%) of the HBV-LC patients, respectively. Multiple regression analysis identified the type of liver disease (P=0.010), hypertension (P=0.022), score on the Beck Depression Inventory (P =0.044), and the serum albumin level (P=0.014) as significant independent factors for the presence of ED.ConclusionsThe prevalence of ED was significantly higher in patients with early-stage HBV-LC than in those with CHB. Therefore, screening male patients with early viral cirrhosis for ED and providing appropriate support are needed, especially when the cirrhosis is accompanied by hypertension, depression, or a depressed level of serum albumin

    Nanomorphology dependence of the environmental stability of organic solar cells

    Previous studies have reported contradictory effects of small-molecule acceptors on the environmental stability of polymer:small-molecule blends, with one showing that a small-molecule acceptor stabilizes and another showing that it destabilizes the polymer donor. In this work, to investigate the origin of these contradictory results, the effects of the nanomorphologies of small-molecule acceptors on the environmental stability of polymer:small-molecule blends are demonstrated. Investigations on the environmental stabilities of polymer:fullerene blends of poly[[4, 8-bis[(2-ethylhexyl)oxy]benzo[1, 2-b:4, 5-b′]dithiophene-2, 6-diyl][3-fluoro-2-[(2-ethylhexyl)carbonyl]thieno[3, 4-b]thiophenediyl]] (PTB7):phenyl-C₆₁-butyric acid methyl ester (PCBM) with contrasting nanomorphologies of PCBM reveal that dispersed PCBM in a mixed phase is the critical factor that causes triplet-mediated singlet oxygen generation and, hence, the severe photooxidation of PTB7, whereas an aggregated PCBM phase stabilizes PTB7 by reducing the formation of PTB7 triplet excitons. In addition, the photooxidation of PTB7 substantially degrades hole transport in the PTB7:PCBM blends by destroying the crystalline PTB7 phases within the films; this effect is strongly correlated with the efficiency losses of the PTB7:PCBM organic solar cells. These conclusions are also extended to polymer:nonfullerene blends of PTB7:ITIC and PTB7:Y6, thereby confirming the generality of this phenomenon for polymer:small-molecule organic solar cells

    Long-term Results of Primary Total Knee Arthroplasty with and without Patellar Resurfacing

    Among patients that underwent total knee arthroplasty from June, 1990 to January, 1999, 61 cases (44 patients) that could be followed for more than 10 years were included in this study. The patients were divided into a patellar retention group and a patellar resurfacing group, and were compared with regard to their clinical and radiological outcomes. In patients undergoing primary TKA, a selective patellar resurfacing protocol was used. The indications for patellar retention were a small patella, nearly normal articular cartilage, minimal preoperative patellofemoral pain, poor patellar bone quality, and young patient age. When patellar retention was performed, osteophytes of the patella were removed and marginal electrocauterization was carried out. There were 25 cases (20 patients) in the patellar retention group and 36 cases (29 patients) in the patellar resurfacing group. The mean follow-up period was 140.7 months in the patellar retention group and 149.0 months in the patellar resurfacing group. The selective patellar resurfacing with total knee arthroplasty had a favorable outcome;there were a significant difference noted between the 2 groups in the functional scores, which showed better outcomes in the patellar resurfacing group than in the patellar retention group

    Stroke is a rare but serious complication of acute myocardial infarction (AMI). Currently, glycoprotein (GP) IIb/IIIa inhibitor is used in clinical practice for acute coronary syndromes and percutaneous coronary interventions (PCIs). The incidence of stroke in patients receiving GP IIb/IIIa inhibitor during PCIs is very low. We report the case of a 47-year-old man who presented with AMI and suffered an acute cerebral infarction after infusion of a GP IIb/IIIa inhibitor following primary PCI

    Estrogen Antagonist and Development of Macular Hole

    To describe the clinical and optical coherence tomography (OCT) features of a macular hole (MH) or its precursor lesion in patients treated with systemic antiestrogen agents. We reviewed the medical history of the patient, ophthalmic examination, and both fundus and OCT findings. Three female patients receiving antiestrogen therapy sought treatment for visual disturbance. All of the patients showed foveal cystic changes with outer retinal defect upon OCT. Visual improvement was achieved through surgery for the treatment of MH in two patients. Antiestrogen therapy may result in MH or its precursor lesion, in addition to perifoveal refractile deposits. OCT examination would be helpful for early detection in such cases
