    On Discreteness of Commensurators

    We begin by showing that commensurators of Zariski dense subgroups of isometry groups of symmetric spaces of non-compact type are discrete provided that the limit set on the Furstenberg boundary is not invariant under the action of a (virtual) simple factor. In particular for rank one or simple Lie groups, Zariski dense subgroups with non-empty domain of discontinuity have discrete commensurators. This generalizes a Theorem of Greenberg for Kleinian groups. We then prove that for all finitely generated, Zariski dense, infinite covolume discrete subgroups of Isom(H3)Isom ({\mathbb{H}}^3), commensurators are discrete. Together these prove discreteness of commensurators for all known examples of finitely generated, Zariski dense, infinite covolume discrete subgroups of Isom(X)Isom(X) for XX a symmetric space of non-compact type.Comment: 14 pages, 1 figur

    Modeling Ephraim Chambers' Knowledge Structure from a NaĂŻve Standpoint

    In the preface to his Cyclopaedia published in 1728 Ephraim Chambers offers readers a systematized structure of his attempt to produce a universal repository of human knowledge. Divided into an interconnected taxonomic tree and domain vocabulary, this structure forms the basis of one effort from the Metadata Research Center to study historical ontologies. The knowledge structure is being encoded into a Simple Knowledge Organization System (SKOS) form as well as a Web Ontology Language (OWL) version. This paper explores the expressive and functional differences between these SKOS and OWL versions of Chambers’ knowledge structure. As part of this goal, the paper research focused on the construction and application of rules in each system to produce a more computationally ready representation of Chambers’ structure in SKOS, which is more thesaurus-like, and OWL, which represents additional ontological nuances. First, studying the various textual aspects at the semantic, syntactic, and typographic levels allowed for the relationships between terms to manifest from which rules governing expression of the connections between elements developed. Second, because each language, SKOS and OWL, functionally expresses different logical relationships, their possibilities and limitations offer a ground for further analyzing the resultant knowledge structures; although, each stemmed from the same basic source of Chambers’ text. Lastly this paper will examine rule making and expression in light of Paul Grice’s theory of conversational implicature to understand how a naïve agent formulates and applies these rules to a knowledge structure

    The Enhanced Brief Structured Observation Model: Efficiently Assess Trainee Competence and Provide Feedback.

    Introduction: The regular observation of trainees is essential to ascertain trainee proficiency in competency-based assessments. Unfortunately, observation of residents is not frequent enough to facilitate entrustment decisions, and the busy clinician-educator may not have the tools or time to conduct effective and efficient observations. Methods: We created a hands-on faculty development workshop utilizing an enhanced variation of the brief structured observation (BSO) technique to train both primary care and subspecialty pediatric faculty on how to effectively and efficiently observe trainees. The workshop has provided faculty a practical approach to observing trainees in a focused fashion and providing effective feedback on clinical skills based on their observation. In the workshop, faculty had an opportunity to observe residents taking an unrehearsed history from a medical student simulating an acutely ill patient, culminating in feedback on the residents\u27 performance using the subjective, objective, assessment, and plan (SOAP) format. Results: This faculty development workshop has been presented to more than 100 faculty both locally and nationally, and feedback has been uniformly positive, with three institutions incorporating this model into their programs to date. Discussion: This enhanced BSO workshop promotes a model that streamlines the observations of trainees and provides faculty with the tools to encourage more observations

    Nanocrystal budesonide (nanobudesonide) is a suspension for nebulization in patients with steroid-responsive pulmonary diseases such as asthma. The pharmacokinetics and safety of the product were compared to those of Pulmicort Respules. Sixteen healthy volunteers were administered nanobudesonide 0.5 and 1.0 mg, Pulmicort Respules 0.5 mg, and placebo in a four-way, randomized crossover design. All nebulized formulations were well tolerated, with no evidence of bronchospasm. Nebulization times were significantly shorter for nanobudesonide compared to Pulmicort Respules. Because of a low oral bioavailability, plasma concentration of budesonide is a good marker of lung-delivered dose. The pharmacokinetics of nanobudesonide 0.5 and 1.0 mg were approximately dose proportional with respect to Cmax, AUC(0-t), and AUC(0-infinity). Nanobudesonide 0.5 mg and Pulmicort Respules 0.5 mg exhibited similar AUCs, suggesting a similar extent of pulmonary absorption. A higher Cmax was noted with nanobudesonide 0.5 mg, and the tmax was significantly different, suggesting a more rapid rate of drug delivery of nanobudesonide 0.5 mg than Pulmicort Respules. In conclusion, nebulized nanobudesonide 0.5 mg was safe in healthy volunteers, with a similar extent of absorption as Pulmicort Respules

    Resident Wellness in US Ophthalmic Graduate Medical Education: The Resident Perspective

    Importance: Wellness programs have become important strategies to combat burnout and depression among residents. However, the resident perspective on wellness in ophthalmic graduate medical education has not been solicited on a national level. Objectives: To report on residents\u27 views of wellness initiatives in ophthalmic graduate medical education and identify potential strategies for promoting resident wellness. Design, Setting, and Participants: In this national survey of ophthalmology residents in the United States, conducted from September 21 to November 3, 2017, all 1048 ophthalmology residents listed on the websites of ophthalmology residency programs accredited by the Accreditation Council for Graduate Medical Education were emailed an anonymous online survey consisting of 12 multiple-choice questions with options for free-text answers. Residents also received a mailed letter with a survey link and a $1 incentive, as well as 2 reminder emails. Survey responses were analyzed using descriptive statistics, and the free-text answers were categorized. Main Outcomes and Measures: Main outcomes include residents\u27 reports regarding their personal experiences with wellness during residency, support systems provided by their programs, and opportunities for improving wellness in ophthalmic graduate medical education. Results: Of 1048 residents, 241 (23.0%) responded to the survey. Most respondents (121 of 177 [68.4%]) reported that their programs faced an issue involving depression, burnout, or suicide among residents within the past year; 26.3% of respondents (61 of 232) reported being involved in a case when resident fatigue, burnout, or depression adversely affected a medical outcome or judgment. Fewer than half of the respondents (110 of 241 [45.6%]) reported that their residency programs placed moderate or major emphasis on promoting a culture of resident wellness, and only 26.7% (63 of 236) reported that their department had a formal resident wellness program. The most commonly cited barrier to resident wellness (59 of 236 [25.0%]) was a lack of time to attend wellness programs. Conclusions and Relevance: These results suggest that there is a substantial burden of burnout and depression among US ophthalmology residents and that there are opportunities to boost wellness in ophthalmic graduate medical education by making wellness curricula more accessible to residents and ensuring that residents have time to attend wellness programs
