    Effect of Application and Intensity of Bevacizumab-based Maintenance After Induction Chemotherapy With Bevacizumab for Metastatic Colorectal Cancer: A Meta-analysis

    BACKGROUND: The administration and intensity of bevacizumab-based maintenance therapy after induction treatment with bevacizumab is still a matter of debate. Thus, the present meta-analysis and an indirect comparison were performed to clarify these issues. PATIENTS AND METHODS: Trials evaluating a separately defined "maintenance phase," with randomization after the induction phase, were selected. Three trials of maintenance with bevacizumab with or without a fluoropyrimidine (CAIRO3, SAKK 41/06, and AIO KRK 0207) were analyzed regarding the effect on progression-free survival (PFS) and overall survival (OS) of any maintenance therapy compared with observation alone and different maintenance intensities (bevacizumab with or without fluoropyrimidine) compared with observation alone and between each other. RESULTS: Maintenance with bevacizumab with or without fluoropyrimidine after bevacizumab-based induction treatment for 4 to 6 months significantly improved PFS (hazard ratio [HR], 0.57; 95% confidence interval [CI], 0.43-0.75; P = .0004) and showed a trend toward prolonged OS (HR, 0.89; 95% CI, 0.78-1.02; P = .09) compared with observation alone. The effect on PFS increased with the intensity of the maintenance regimen (HR, 0.72; 95% CI, 0.60-0.85 for single-agent bevacizumab vs. HR, 0.45; 95%, CI 0.39-0.51 for combination therapy, both compared to observation alone). In contrast, the HRs for OS remained in the same range. A similarly improved PFS (HR, 0.63; 95% CI, 0.50-0.79) was shown for the more intensive maintenance therapy (bevacizumab and fluoropyrimidine) compared with bevacizumab alone. CONCLUSION: Bevacizumab-based maintenance therapy after induction chemotherapy with bevacizumab significantly improves PFS and showed a trend toward prolonged OS and should thus be considered, in particular, in patients with a response to induction treatment

    The Planetary Mass Companion 2MASS1207-3932 B: Temperature, Mass and Evidence for an Edge-On Disk

    We present J-band imaging and H+K-band low-resolution spectroscopy of 2MASS1207-3932 AB, obtained with VLT NACO. For the putative planetary mass secondary, we find J = 20.0+/-0.2 mag. The HK spectra of both components imply low gravity, and a dusty atmosphere for the secondary. Comparisons to synthetic spectra yield Teff_A ~ 2550+/-150K, and Teff_B ~ 1600+/-100K, consistent with their late-M and mid-to-late L types. For these Teff, and an age of 5-10 Myrs, evolutionary models imply M_A ~ 24+/-6 M_Jup and M_B ~ 8+/-2 M_Jup. Independent comparisons of these models to the observed colors, spanning ~I to L', also yield the same masses and temperatures. Our primary mass agrees with other recent analyses; however, our secondary mass, while still in the planetary regime, is 2-3 times larger than claimed previously. This discrepancy can be traced to the luminosities: while the absolute photometry and Mbol of the primary agree with theoretical predictions, the secondary is ~ 2.5+/-0.5 mag fainter than expected in all bands from I to L' and in Mbol. This accounts for the much lower secondary mass (and temperature) derived earlier. We argue that this effect is highly unlikely to result from a variety of model-related problems, and is instead real. This conclusion is bolstered by the absence of any luminosity problems in either the primary, or in AB Pic B which we also analyse. We therefore suggest grey extinction in 2M1207B, due to occlusion by an edge-on circum-secondary disk. This is consistent with the observed properties of edge-on disks around T Tauri stars, and with the known presence of a high-inclination evolved disk around the primary.     We demonstrate that a minor modification of the extrapolation proposed by Jensen [(2005): Theor Chem Acc 113:267] yields very reliable estimates of the Hartree-Fock limit in conjunction with correlation consistent basis sets. Specifically, a two-point extrapolation of the form EHF,L=EHF,+A(L+1)exp(9L)E_{HF,L}=E_{HF,\infty}+A(L+1)\exp(-9\sqrt{L}) yields HF limits EHF,E_{HF,\infty} with an RMS error of 0.1 millihartree using aug-cc-pVQZ and aug-cc-pV5Z basis sets, and of 0.01 millihartree using aug-cc-pV5Z and aug-cc-pV6Z basis sets.Comment: Theoretical Chemistry Accounts, in pres

    Expectancy effects in the EEG during joint and spontaneous word-by-word sentence production in German

    Our aim in the present study is to measure neural correlates during spontaneous interactive sentence production. We present a novel approach using the word-by-word technique from improvisational theatre, in which two speakers jointly produce one sentence. This paradigm allows the assessment of behavioural aspects, such as turn-times, and electrophysiological responses, such as event-related-potentials (ERPs). Twenty-five participants constructed a cued but spontaneous four-word German sentence together with a confederate, taking turns for each word of the sentence. In 30% of the trials, the confederate uttered an unexpected gender-marked article. To complete the sentence in a meaningful way, the participant had to detect the violation and retrieve and utter a new fitting response. We found significant increases in response times after unexpected words and – despite allowing unscripted language production and naturally varying speech material – successfully detected significant N400 and P600 ERP effects for the unexpected word. The N400 EEG activity further significantly predicted the response time of the subsequent turn. Our results show that combining behavioural and neuroscientific measures of verbal interactions while retaining sufficient experimental control is possible, and that this combination provides promising insights into the mechanisms of spontaneous spoken dialogue

    On the Development of a New Nonequilibrium Chemistry Model for Mars Entry

    This paper represents a summary of results to date of an on-going effort at NASA Ames Research Center to develop a physics-based non-equilibrium model for hypersonic entry into the Martian atmosphere. Our approach is to first compute potential energy surfaces based on accurate solutions of the electronic Schroedinger equation and then use quasiclassical trajectory calculations to obtain reaction cross sections and rate coefficients based on these potentials. We have presented new rate coefficients for N2 dissociation and CO dissociation and exchange reactions. These results illustrate shortcomings with some of the rate coefficients in Parks original T-Tv model for Mars entries and with some of the 30-45 year old shock tube data. We observe that the shock tube experiments of CO + O dissociation did not adequately account for the exchange reaction that leads to formation of C + O2. This reaction is actually the primary channel for CO removal in the shock layer at temperatures below 10,000 K, because the reaction enthalpy for exchange is considerably lower than the comparable value for dissociation
