    Adolescents and Young Adults: Targeting the Unique Challenges of This High Risk Group

    Adolescents, particularly sexual minority youth, account for a disproportionate number of sexually transmitted infections and pelvic inflammatory disease cases. This population is at increased risk of infection and presents unique challenges when it comes to screening and treatment. This chapter will discuss these challenges and suggest some evidence based solutions. Challenges to be discussed include lack of physician comfort in discussing sexual activity with adolescents, issues of confidentiality, difficulties with contacting adolescent patients with their test results, and arranging for treatment of both patients and their partners. A discussion about expedited partner therapy will be included—the legal implications and the role it plays in sexually transmitted infection reduction efforts

    High Quality, Equity, and Assessment: An Analysis of Variables Impacting English Learner Standardized Science Test Performance and Implications for Construct Validity

    In the United States, assessment is seen as a lever that can facilitate high-quality education. This study on English learners, students whose native language is not English, was based on data from eighth-grade English learners’ performance on science general and content-specific (physical, life, and earth science) standardized exams and an English language proficiency exam. The researchers utilized regression analysis to examine factors (i.e., socioeconomic status, home language, English language proficiency, and receptive and productive elements of language) that are predictive of English learner performance in general and content-specific science standardized assessments to identify implications for construct validity of high-stakes science assessments. The research question is as follows: What factors influence the performance of English learners on a standardized science assessment, including overall performance and content-specific domains? Three main findings emerged from this research study. First, this study confirms previous research indicating that socioeconomic status and English language proficiency are predictive of English learners’ achievement on content-based standardized tests. Second, this study adds to current research by providing evidence that productive language scores are the most significant predictors of English learner science achievement, in comparison to receptive language scores, overall English language proficiency scores, home language, and socioeconomic status. Third, this study adds to the body of evidence needed to challenge the validity of standardized science tests for English learner populations. The findings of this study challenge the construct validity of science content-based assessments for English learners, emphasizing the importance of productive language in academic performance

    Infection percolation: A dynamic network model of disease spreading

    Models of disease spreading are critical for predicting infection growth in a population and evaluating public health policies. However, standard models typically represent the dynamics of disease transmission between individuals using macroscopic parameters that do not accurately represent person-to-person variability. To address this issue, we present a dynamic network model that provides a straightforward way to incorporate both disease transmission dynamics at the individual scale as well as the full spatiotemporal history of infection at the population scale. We find that disease spreads through a social network as a traveling wave of infection, followed by a traveling wave of recovery, with the onset and dynamics of spreading determined by the interplay between disease transmission and recovery. We use these insights to develop a scaling theory that predicts the dynamics of infection for diverse diseases and populations. Furthermore, we show how spatial heterogeneities in susceptibility to infection can either exacerbate or quell the spread of disease, depending on its infectivity. Ultimately, our dynamic network approach provides a simple way to model disease spreading that unifies previous findings and can be generalized to diverse diseases, containment strategies, seasonal conditions, and community structures.Comment: In press, Frontiers in Physics (2021

    Cutaneous Adverse Events of Systemic Melanoma Treatments: A Retrospective Single-Center Analysis

    Recent progress in the treatment of advanced melanoma has led to the improved survival of affected patients. However, novel treatments also lead to considerable and distinct skin toxicity. To further characterize cutaneous adverse events (AE) of systemic treatments, we conducted a single-center retrospective study of biopsy-proven cutaneous adverse events of melanoma treatment over a period of 10 years at the University Hospital of Zurich, Switzerland. In 102 identified patients, 135 individual skin AEs developed. Immune checkpoint blockade (ICB) was causal for 81 skin AEs, and 54 were related to targeted therapies (TT). Recorded types of skin AEs included lichenoid, maculopapular, acneiform, urticarial, panniculitis, folliculitis, psoriasiform, granulomatous, eczematous, and others. The incidence of skin AEs was higher with TT (18.54%) than with ICB (9.64%, p = 0.0029). Most AEs were low-grade, although 19.21% of AEs were common terminology criteria for adverse events (CTCAE) Grades 3 or 4. A large spectrum of skin AEs was documented during treatment of advanced melanoma, and distinct phenotypes were observed, depending on treatment classes. AEs occurred earlier during treatment with TT than with ICB, and distinct types of skin AEs were associated with respective treatment classes.
