    The influence of optimal medical treatment on the 'obesity paradox', body mass index and long-term mortality in patients treated with percutaneous coronary intervention: A prospective cohort study

    Objective: To assess whether the obesity paradox persists in the long term and to study the effect of optimal medical treatment on this phenomenon. Design: A retrospective cohort study. Setting: A tertiary care centre in Rotterdam. Participants: From January 2000 to December 2005, 6332 patients undergoing percutaneous coronary intervention for coronary artery disease were categorised into underweight (body mass index (BMI)30). Primary outcome measure: Mortality. Secondary outcome measures: Cardiac death and non-fatal myocardial infarction. Results: Optimal medical treatment was more common in obese patients as compared with normal weight patients (85% vs 76%; p<0.001). At a mean of 6.1 years, overweight and obese patients had a lower risk of all-cause mortality (HR: 0.75, 95% CI 0.66 to 0.86 and HR: 0.72, 95% CI 0.60 to 0.87, respectively). After adjusting for OMT in the multivariate analysis, BMI did not remain an independent predictor of longterm mortality (HR: 0.90, 95% CI 0.72 to 1.12 and HR: 1.07, 95% CI: 0.80 to 1.43, respectively). Conclusion: BMI is inversely related to long-term mortality in patients treated with percutaneous coronary intervention. Patients with a normal BMI are on suboptimal medical treatment when compared with those with a high BMI. A more optimal medical treatment in the obese group may explain the observed improved outcome in these patients

    Grazing in a megagrazer-dominated savanna does not reduce soil carbon stocks, even at high intensities

    Recent studies suggest that wild animals can promote ecosystem carbon sinks through their impacts on vegetation and soils. However, livestock studies show that intense levels of grazing reduce soil organic carbon (SOC), leading to concerns that rewilding with large grazers may compromise ecosystem carbon storage. Furthermore, wild grazers can both limit and promote woody plant recruitment and survival on savanna grasslands, with both positive and negative impacts on SOC, depending on the rainfall and soil texture contexts. We used grazing lawns in one of the few African protected savannas that are still dominated by megagrazers (> 1000 kg), namely white rhinoceros Ceratotherium simum, as a model to study the impact of prolonged and intense wild grazing on SOC stocks. We contrasted SOC stocks between patches of varying grazing intensity and woody plant encroachment in sites across different rhino habitat types. We found no differences in SOC stocks between the most- and least grazed plots in any of the habitats. Intermediately grazed plots, however, had higher SOC stocks in the top 5 cm compared to most and least grazed plots, but only in the closed-canopy woodland habitat and not in the open habitats. Importantly, we found no evidence to support the hypothesis that wild grazing reduces SOC, even at high grazing intensities by the world's largest megagrazer. Compared to the non-encroached reference plots, woody encroached plots had higher SOC stocks in soils with low clay content and lower SOC stocks in soils with high clay content, although only in the top 5 cm. Accordingly, our study highlights that wild grazers may influence SOC indirectly through their impact on tree-grass ratios in grassy ecosystems. Our study thus provides important insights for future natural climate solutions that focus on wild grazer conservation and restoration.     Solid phase speciation controls copper mobilisation from marine sediments by methanobactin

    Although marine environments represent huge reservoirs of the potent greenhouse gas methane, they currently contribute little to global net methane emissions. Most of the methane is oxidized by methanotrophs, minimizing escape to the atmosphere. Aerobic methanotrophs oxidize methane mostly via the copper (Cu)-bearing enzyme particulate methane monooxygenase (pMMO). Therefore, aerobic methane oxidation depends on sufficient Cu acquisition by methanotrophs. Because they require both oxygen and methane, aerobic methanotrophs reside at oxic-anoxic interfaces, often close to sulphidic zones where Cu bioavailability can be limited by poorly soluble Cu sulphide mineral phases. Under Cu-limiting conditions, certain aerobic methanotrophs exude Cu-binding ligands termed chalkophores, such as methanobactin (mb) exuded by Methylosinus trichosporium OB3b. Our main objective was to establish whether chalkophores can mobilise Cu from Cu sulphide-bearing marine sediments to enhance Cu bioavailability. Through a series of kinetic batch experiments, we investigated Cu mobilisation by mb from a set of well-characterized sulphidic marine sediments differing in sediment properties, including Cu content and phase distribution. Characterization of solid-phase Cu speciation included X-ray absorption spectroscopy and a targeted sequential extraction. Furthermore, in batch experiments, we investigated to what extent adsorption of metal-free mb and Cu-mb complexes to marine sediments constrains Cu mobilisation. Our results are the first to show that both solid phase Cu speciation and chalkophore adsorption can constrain methanotrophic Cu acquisition from marine sediments. Only for certain sediments did mb addition enhance dissolved Cu concentrations. Cu mobilisation by mb was not correlated to the total Cu content of the sediment, but was controlled by solid-phase Cu speciation. Cu was only mobilised from sediments containing a mono-Cu-sulphide (CuSx) phase. We also show that mb adsorption to sediments limits Cu acquisition by mb to less compact (surface) sediments. Therefore, in sulphidic sediments, mb-mediated Cu acquisition is presumably constrained to surface-sediment interfaces containing mono-Cu-sulphide phases

Research has shown a lack of clinimetric knowledge and clinical application of measurement instruments in daily practice may hamper implementation of these guidelines.
