    ObjectiveEndobronchial ultrasound-guided transbronchial needle aspiration could be performed better with computer-based preparation.MethodsThree-dimensional virtual bronchoscopy was used to develop 2 modes of computer-based “virtual endobronchial ultrasound.” “Virtual endobronchial ultrasound standard” used conventional virtual bronchoscopy to determine the spot and angle for transbronchial needle aspiration, which was further evaluated by virtual bronchoscopy. “Virtual endobronchial ultrasound advanced” used multiple layers of 3-dimensional images of the target lesions and associated vascular structures in combination with virtual bronchoscopy. Target lesions and associated vascular structures (eg, pulmonary artery) were visualized through half-transparent bronchial walls.ResultsBoth methods required 5 to 15 minutes of preparation per case. Virtual endobronchial ultrasound standard required only basic computer software for virtual bronchoscopy, whereas virtual endobronchial ultrasound advanced required an advanced computer application. Virtual endobronchial ultrasound advanced allowed for a more intuitive recognition of the target. Both methods were useful in evaluating the feasibility of transbronchial needle aspiration, especially when the target was out of regular mediastinal lymph nodes, or in targeting a lesion located at a high upper angle (eg, 4L lymph node). Because the puncture spot was predetermined, bronchoscopists did not have to search for the target using ultrasound at the time of actual endobronchial ultrasound-guided transbronchial needle aspiration; rather, ultrasound was used only for confirmation of the target location and visualization of transbronchial needle aspiration.ConclusionsBoth computer-based preparation methods of virtual endobronchial ultrasound were useful in predetermining the puncture spot of transbronchial needle aspiration, suggesting their potential complementary role to the conventional technique of endobronchial ultrasound-guided transbronchial needle aspiration

    Melting of excitonic insulator phase by an intense terahertz pulse in Ta2_2NiSe5_5

    In this study, the optical response to a terahertz pulse was investigated in the transition metal chalcogenide Ta2_2NiSe5_5, a candidate excitonic insulator. First, by irradiating a terahertz pulse with a relatively weak electric field (0.3 MV/cm), the spectral changes in reflectivity near the absorption edge due to third-order optical nonlinearity were measured and the absorption peak characteristic of the excitonic phase just below the interband transition was identified. Next, by irradiating a strong terahertz pulse with a strong electric field of 1.65 MV/cm, the absorption of the excitonic phase was found to be reduced, and a Drude-like response appeared in the mid-infrared region. These responses can be interpreted as carrier generation by exciton dissociation induced by the electric field, resulting in the partial melting of the excitonic phase and metallization. The presence of a distinct threshold electric field for carrier generation indicates exciton dissociation via quantum-tunnelling processes. The spectral change due to metallization by the electric field is significantly different from that due to the strong optical excitation across the gap, which can be explained by the different melting mechanisms of the excitonic phase in the two types of excitations.Comment: 66 pages, 11 figures, 2 table

    Formation of a community of practice in the watershed scale, with integrated local environmental knowledge

    金沢大学地域連携推進センターRural communities around the world face formidable problems such as resource depletion, environmental degradation and economic decline. While the term \u27community\u27 is often used without clear definition or context, it can be viewed as a group of people emerging through social interaction. Through a series of collaborative action toward a shared goal, a community of practice can be formed. This paper proposes a hypothetical framework of integrated local environmental knowledge (ILEK), and applies it to analyze the processes of collaborative actions in the case of the Nishibetsu Watershed in Hokkaido, Japan. The case study identified several phases of actions, all initiated by a group of local residents on a grassroots and voluntary basis. These local resident-initiated collaborative actions had a particular confluence of elements to facilitate gradual strengthening of formal and informal institutions in the watershed scale beyond jurisdictional boundaries, making this a worthy case to study. The local residents used diverse types of knowledge, including livelihood-based technologies and skills of working as a group and with local governments, for establishing and strengthening various institutions for collaborative actions, with such knowledge being used in the manner of tools in a box of bricolage for community formation. © 2018 by the authors

    A generalization of determinant formulas for the solutions of Painlev\'e II and XXXIV equations

    A generalization of determinant formulas for the classical solutions of Painlev\'e XXXIV and Painlev\'e II equations are constructed using the technique of Darboux transformation and Hirota's bilinear formalism.     Solitary mandibular metastasis as an initial manifestation of hepatocellular carcinoma.

    Oral metastases from hepatocellular carcinoma are very rare. We encountered a case of hepatocellular carcinoma with a solitary metastasis to the mandible as an initial manifestation. The patient was a 76-year-old man who was admitted for left mandibular swelling. A biopsy specimen of mandible was suspected to be a metastatic tumor. The histological findings, abdominal computed tomography, bone scintigraphy, and F-18 fluorodeoxyglucose-positron emission tomography (FDG-PET) revealed it to be a solitary metastasis from hepatocellular carcinoma. As a result, he was diagnosed to have liver cirrhosis due to a hepatitis C virus infection and hepatocellular carcinoma with a solitary metastasis to the mandible. The primary lesion was treated with transcatheter arterial embolization (TAE), and the metastasis to the mandible was surgically resected. The patient survived for 9 months after treatment without recurrence
