    Changes in left atrial function following two regimens of combined exercise training in patients with ischemic cardiomyopathy: a pilot study

    PurposeLeft atrial dysfunction has shown to play a prognostic role in patients with ischemic cardiomyopathy (ICM) and is becoming a therapeutic target for pharmacological and non-pharmacological interventions. The effects of exercise training on the atrial function in patients with ICM have been poorly investigated. In the present study, we assessed the effects of a 12-week combined training (CT) program on the left atrial function in patients with ICM.MethodsWe enlisted a total of 45 clinically stable patients and randomly assigned them to one of the following three groups: 15 to a supervised CT with low-frequency sessions (twice per week) (CTLF); 15 to a supervised CT with high-frequency sessions (thrice per week) (CTHF); and 15 to a control group following contemporary preventive exercise guidelines at home. At baseline and 12 weeks, all patients underwent a symptom-limited exercise test and echocardiography. The training included aerobic continuous exercise and resistance exercise. The analysis of variance (ANOVA) was used to compare within- and inter-group changes.ResultsAt 12 weeks, the CTLF and CTHF groups showed a similar increase in the duration of the ergometric test compared with the control (ANOVA p < 0.001). The peak atrial longitudinal strain significantly increased in the CTHF group, while it was unchanged in the CTLF and control groups (ANOVA p = 0.003). The peak atrial contraction strain presented a significant improvement in the CTHF group compared with the CTLF and control groups. The left ventricular global longitudinal strain significantly increased in both the CTHF and the CTLF groups compared with the control group (ANOVA p = 0.017). The systolic blood pressure decreased in the CTHF and CTLF groups, while it was unchanged in the control group. There were no side effects causing the discontinuation of the training.ConclusionsWe demonstrated that a CT program effectively improved atrial function in patients with ICM in a dose–effect manner. This result can help with programming exercise training in this population

    Advances in Ku-Band GaN Single Chip Front End for Space SARs: From System Specifications to Technology Selection

    In this paper, a single-chip front-end (SCFE) operating in Ku-band (12–17 GHz) is presented. It is designed exploiting a GaN on SiC technology featured by 150 nm gate length provided by UMS foundry. This MMIC integrates high power and low noise amplification functions enabled by a single-pole double-throw (SPDT) switch, occupying a total area of 20 mm2. The transmitting chain (Tx) presents a 39 dBm output power, a power added efficiency (PAE) higher than 30% and a 22 dB power gain. The receive path (Rx) offers a low noise figure (NF) lower than 2.8 dB with 25 dB of linear gain. The Rx port output power leakage is limited on chip to be below 15 dBm even at high compression levels. Finally, a complete characterization of the SCFE in the Rx and Tx modes is presented, also showing the measurement of the recovery time in the presence of large-signal interferences

    Analysis of Volatiles in <i>Senecio anteuphorbium</i> Essential Oil with a Focus on Its Allelopathic Effect by Means of Gas Chromatography

    The present study aimed to investigate Senecio anteuphorbium, an endemic plant growing in West Morocco and widely used in local folk medicine. The essential oil (EO) extracted from the aerial parts was analyzed by gas chromatography and tested for allelopathic activity. The quantitation of the volatiles was carried out by means of GC-FID with response factors, which were validated through reliable calibration procedures, based on external and internal standardization. This analytical approach allowed to define the real concentration of each constituent (weight%, g/100 g) alongside the conventional relative percent. On the other hand, the identification process was supported by a dual matching based on both mass spectra and retention indices. The essential oil resulted in being rich in sesquiterpenes, with the predominant constituents being bicyclogermacrene (22.75 g/100 g), spathulenol (25.26 g/100 g), epi-Îł-eudesmol (6.8 g/100 g), and selina-4,11-diene (5.08 g/100 g). The allelopathic effect was evaluated by studying the inhibition of the germination and growth of Lactuca sativa seeds. A potent allelopathic effect was recorded by the essential oil at a dose of 0.281 mg/mL, with almost a total inhibition of germination

    Simultaneous Integrated Boost Radiotherapy in Unresectable Stage IV (M0) Head and Neck Squamous Cell Cancer Patients: Daily Clinical Practice

    AimTo evaluate clinical outcome in locally-advanced stage IV (M0) head and neck cancer patients treated using intensity-modulated radiotherapy (IMRT) with simultaneous integrated boost (SIB) in daily clinical practice.BackgroundDespite SIB-IMRT has been reported as a feasible and effective advanced head and neck cancer treatment, there are few data about its concurrent use with systemic therapies.Material and MethodsWe reviewed 41 staged IV (M0) head and neck cancer patients treated in two radiotherapy units in the city of Messina (Italy) during the last six years, using intensity modulated techniques-SIB. 22/41 patients had concomitant chemotherapy or cetuximab. Acute and late toxicities, objective response (OR) rate, local control (LC) and overall survival (OS) have been evaluated.Results37/41 patients received the planned doses of radiotherapy, 2 patients died during the therapy. The major acute regional toxicities were skin reaction and mucositis. A case of mandibular osteoradionecrosis was recorded. At completion of treatment, OR was evaluated in 38 patients: 32/38 patients (84.2%) had complete (55.3%) and partial (28.9%) response. The 1- and 5-year LC rates were 73.4% and 69.73%, respectively. The 1-, 3-, and 5-year OS rates were 85.93%, 51.49% and 44.14%, respectively. No statistically significant differences in outcomes have been observed in patients treated with radiotherapy alone vs. irradiation concomitant to chemo/biotherapy. The median OS was 45 months.ConclusionSIB-IMRT is safeand can be used with concomitant chemotherapy/biotherapy in real-life daily clinical practice. SIB-IMRT alone is a valid alternative in patients unfit for systemic therapies
