ESSENCE-Q - dash; a first clinical validation study of a new screening questionnaire for young children with suspected neurodevelopmental problems in south Japan
Background: Early identification of autism spectrum disorder, intellectual developmental disorder, attention-deficit/hyperactivity disorder, and other neurodevelopmental disorders/problems is crucial, yet diagnosis is often delayed for years under the often misguided “wait-and-see” paradigm. The early symptomatic syndromes eliciting neurodevelopmental clinical examinations-questionnaire (ESSENCE-Q) is a brief (12-item) screening questionnaire developed specifically for the purpose of speeding up the identification process of a wide variety of neurodevelopmental problems. The aims were to 1) estimate the reliability of the ESSENCE-Q, 2) evaluate the clinical cutoff levels suggested by the author of the ESSENCE-Q, and 3) propose optimal cutoff levels based on receiver operating characteristic analysis.
Methods: The ESSENCE-Q was used for 1 year by a psychiatrist in Kochi, Japan, assessing children under the age of 6 years referred for developmental problems. The children were also clinically assessed with regard to whether or not they met criteria for a developmental disorder (diagnosis positive and diagnosis negative groups). We contrasted the results of the ESSENCE-Q and those of clinical diagnostic assessments in 130 cases.
Results: Cronbach’s alpha was 0.82, sensitivity was 0.94 (95% confidence interval [CI]: [0.88, 0.98]), and specificity 0.53 (95% CI: [0.28, 0.77]), which are reasonable psychometrics for a first-step screening tool. Based on receiver operating characteristic analysis, we recommended an optimal cutoff level of yes ≥2 or maybe/a little ≥3 on the ESSENCE-Q (0.87 (95% CI: [0.79, 0.92]) sensitivity and 0.77 (95% CI: [0.50, 0.93]) specificity).
Conclusion and implication: The ESSENCE-Q can be a good instrument for use as a screening tool for aiding in the process of early identification of neurodevelopmental disorders in clinical settings. To establish the broader validity and reliability of the ESSENCE-Q, case–control studies and general population studies of children in different age groups are needed

Strategies to enhance the excitation energy-transfer efficiency in a light-harvesting system using the intra-molecular charge transfer character of carotenoids
Fucoxanthin is a carotenoid that is mainly found in light-harvesting complexes from brown algae and diatoms. Due to the presence of a carbonyl group attached to polyene chains in polar environments, excitation produces an excited intra-molecular charge transfer. This intra-molecular charge transfer state plays a key role in the highly efficient (∼95%) energy-transfer from fucoxanthin to chlorophyll a in the light-harvesting complexes from brown algae. In purple bacterial light-harvesting systems the efficiency of excitation energy-transfer from carotenoids to bacteriochlorophylls depends on the extent of conjugation of the carotenoids. In this study we were successful, for the first time, in incorporating fucoxanthin into a light-harvesting complex 1 from the purple photosynthetic bacterium, Rhodospirillum rubrum G9+ (a carotenoidless strain). Femtosecond pump-probe spectroscopy was applied to this reconstituted light-harvesting complex in order to determine the efficiency of excitation energy-transfer from fucoxanthin to bacteriochlorophyll a when they are bound to the light-harvesting 1 apo-proteins
転移性脳腫瘍の倍量造影剤投与における、分割投与と単回投与の病変描出能の比較
PURPOSE: As stereotactic radiotherapy (SRT) becomes widespread, precise information including number, location, and margin of lesions is required when magnetic resonance (MR) imaging of brain metastasis is performed. We compare methods using 2 separate injections and a single injection for the administration of a double dose of contrast medium for contrastenhanced MR imaging. MATERIALS AND METHODS: We divided 40 patients with brain metastasis into 2 groups of 20 patients. Group A received 2 separate injections (0.2 + 0.2 mL/kg) of contrast medium (gadoteridol); Group B received a single injection of the same total dose (0.4 mL/kg). Group A underwent spin echo (SE) T1-weighted imaging (T1WI) and magnetization prepared rapid acquisition with gradient echo sequence (MPRAGE) after each injection, and Group B underwent the same MR studies at the same timing as Group A. We evaluated the number, signal-to-noise ratio (SNR), diameter, margin delineation, and volume of lesions and compared them between early and delayed studies by the 2 methods. RESULTS: The number of detected lesions was largest in delayed studies of MPRAGE in both groups. The SNR of the lesions was statistically lower in early studies of Group A than other studies. Delayed studies of Group B showed statistically better margin delineation than other studies on both SE-T1WI and MPRAGE studies. Diameter and enhanced volume were statistically significantly larger on delayed phase than early phase in both groups. CONCLUSION: Use of a single injection of double-dose contrast medium and longer delay time may improve margin delineation of lesions for the study of brain metastasis. Oscillator neural network model with distributed native frequencies
We study associative memory of an oscillator neural network with distributed
native frequencies. The model is based on the use of the Hebb learning rule
with random patterns (), and the distribution function of
native frequencies is assumed to be symmetric with respect to its average.
Although the system with an extensive number of stored patterns is not allowed
to get entirely synchronized, long time behaviors of the macroscopic order
parameters describing partial synchronization phenomena can be obtained by
discarding the contribution from the desynchronized part of the system. The
oscillator network is shown to work as associative memory accompanied by
synchronized oscillations. A phase diagram representing properties of memory
retrieval is presented in terms of the parameters characterizing the native
frequency distribution. Our analytical calculations based on the
self-consistent signal-to-noise analysis are shown to be in excellent agreement
with numerical simulations, confirming the validity of our theoretical

Linear stability analysis of retrieval state in associative memory neural networks of spiking neurons
We study associative memory neural networks of the Hodgkin-Huxley type of
spiking neurons in which multiple periodic spatio-temporal patterns of spike
timing are memorized as limit-cycle-type attractors. In encoding the
spatio-temporal patterns, we assume the spike-timing-dependent synaptic
plasticity with the asymmetric time window. Analysis for periodic solution of
retrieval state reveals that if the area of the negative part of the time
window is equivalent to the positive part, then crosstalk among encoded
patterns vanishes. Phase transition due to the loss of the stability of
periodic solution is observed when we assume fast alpha-function for direct
interaction among neurons. In order to evaluate the critical point of this
phase transition, we employ Floquet theory in which the stability problem of
the infinite number of spiking neurons interacting with alpha-function is
reduced into the eigenvalue problem with the finite size of matrix. Numerical
integration of the single-body dynamics yields the explicit value of the
matrix, which enables us to determine the critical point of the phase

Investigation of the outpatient chemotherapy for lung cancer patients in Tokushima University Hospital
Platinum-doublet regimens and docetaxel as first- and second-line chemotherapy,
respectively, are shown to prolong the survival of lung cancer patients in various
randomized phase III studies. However, the evidence for the efficacy of chemotherapy
for lung cancer in the clinical practice is still insufficient. In the present study, we
investigated the effectiveness and safety of outpatient chemotherapy for lung cancer in
the clinical practice. Ninety-four lung cancer cases were retrospectively analyzed. Among
these cases, 67 (71.3%) were non-small cell lung cancer (NSCLC) and 27 (28.7%) were
small cell lung cancer (SCLC). The response rates in SCLC and NSCLC patients were
55.6% (15/27) and 16.9% (11/65), respectively. Objective tumor response rates for the patients
were found to decrease substantially with each line of treatment as described previously.
All adverse events were well tolerated and no treatment-related death was observed.
Median time to treatment failures (TTFs) of first-line treatment were 10.1 months
and 4.8 months in SCLC and NSCLC, respectively. These findings indicate that even in
the setting of clinical practice, the efficacy and safety of chemotherapy is strictly insured
by the appropriate therapeutic management
Associative memory storing an extensive number of patterns based on a network of oscillators with distributed natural frequencies in the presence of external white noise
We study associative memory based on temporal coding in which successful
retrieval is realized as an entrainment in a network of simple phase
oscillators with distributed natural frequencies under the influence of white
noise. The memory patterns are assumed to be given by uniformly distributed
random numbers on so that the patterns encode the phase differences
of the oscillators. To derive the macroscopic order parameter equations for the
network with an extensive number of stored patterns, we introduce the effective
transfer function by assuming the fixed-point equation of the form of the TAP
equation, which describes the time-averaged output as a function of the
effective time-averaged local field. Properties of the networks associated with
synchronization phenomena for a discrete symmetric natural frequency
distribution with three frequency components are studied based on the order
parameter equations, and are shown to be in good agreement with the results of
numerical simulations. Two types of retrieval states are found to occur with
respect to the degree of synchronization, when the size of the width of the

Outcomes and factors influencing survival in cirrhotic cases with spontaneous rupture of hepatocellular carcinoma: a multicenter study
<p>Abstract</p> <p>Background</p> <p>Spontaneous rupture is rare complication of hepatocellular carcinoma (HCC) with high mortality rate in cirrhotic cases. The aim of this study was to determine the factors influencing prognosis in cases of spontaneously ruptured HCC and to investigate the outcomes of the treatments employed, especially transcatheter arterial embolization (TAE).</p> <p>Methods</p> <p>A retrospective multicenter study was conducted in 48 cirrhotic patients with spontaneous rupture of HCC. Conservative treatment was employed in 32 patients (ConT group) and TAE was performed in 16 patients (TAE group).</p> <p>Results</p> <p>The median survival time (MST) in the ConT group was only 13.1 days and the survival rate was extremely poor: 59.4% at 7 days, 37.5% at 14 days, and 6.3% at 30 days. On the other hand, the MST in the TAE group was 244.8 days and the survival rate was 87.5% at 1 month, 56.3% at 3 months, 23.4% at 12 months, and 15.6% at 24 months. According to the results of univariate analyses, factors associated with poor hepatic function and poor suitability for TAE was important determinants of short-term death (less than 3 weeks) among the patients (<it>p </it>< 0.05). On the other hand, among the patients in whom initial TAE was successfully performed (<it>n </it>= 15), a multivariate analysis showed that a maximum tumor size not exceeding 7 cm was the only independent factor determining long-term survival (<it>p </it>= 0.0130).</p> <p>Conclusion</p> <p>Despite the inherent limitations of this retrospective study, TAE appears to be a useful treatment strategy for cirrhotic patients with spontaneous HCC rupture, as it yielded a longer survival period compared with conservative treatment in patients with ruptured HCC. Among the patients with ruptured HCC in whom initial TAE was successfully performed, the maximum tumor size was an important factor influencing survival.</p
