    To study the correlation of mean macular thickness using optical coherence tomography with distant and near visual acuity in patients of diabetic maculopathy

    Background: to study the correlation of mean macular thickness using optical coherence tomography with distant and near visual acuity in patients of diabetic maculopathy.Methods: A prospective, single centre study was conducted on 50 eyes of diabetic patients, with Diabetic Retinopathy with CSME in which patients macular thickness was measured on Ocular Coherence Tomography using fast macular thickness scan. The unaided and best corrected visual acuity was measured in all patients using Snellens distant vision and Jaggers near vision charts.Results: A linear correlation between the OCT measured macular thickness and both the distance visual acuity and the near visual acuity. That means that for a given level of macular thickness, we can predict visual acuity for it. In our series correlation coefficient was 0.921 for distance visual acuity and 0.899 for near visual acuity. Although the correlation value is high in our study, we did find a range of visual acuities for a given range of macular thickness. For every 100 micron change in mean macular thickness, best corrected visual acuity (BCVA) changed 0.3 LogMAR units, for distance as well as for near.Conclusions: Macular thickness and visual acuities (distance as well as near) are strongly correlated but there can be variations. And a wide range of visual acuities is possible for a given degree of macular edema. Macular thickness though a strong predictor of visual acuity; other factors might also play a role in determining visual acuity for a particular patient

    Prevalence of diabetes mellitus in rural population of Mullana, district Ambala, Haryana, India

    Background: Diabetes mellitus (DM) is a major cause of avoidable blindness in developing and developed countries. The International Diabetes Federation (IDF)’s Diabetes Atlas reports that India has the highest number of people with diabetes (nearly 25%) in the world, and hence considered to be the “Diabetes Capital of the World”.Methods: This is a hospital record based study was planned to estimate the prevalence of Diabetes Mellitus in various age groups in rural population of Mullana, District Ambala(Haryana).Results: Out of 1050 patients screened 50 were found to be having Diabetes mellitus (DM) 1000 were non-diabetic i.e. prevalence of Diabetes mellitus (DM) was found to be 4.76%. Out of 362 males screened 22 were suffering from Diabetes mellitus (DM) i.e. prevalence of 6.07%. Whereas out of 688 females screened 28 were suffering from Diabetes mellitus (DM) i.e. prevalence of 4.06%. Prevalence of Diabetes mellitus (DM) in males was found to be maximum in age group of more than 70 years i.e. 6.97% as compare to female i.e.5.29%.Conclusions: Thus, the current study recorded high prevalence of Diabetes mellitus (DM) among rural population which should be a cause of concern for health care providers.

    The stark malignancy of domestic violence which is also referred as intimate partner violence on the state of women hood can be determined by the prevalence of it in the present society. It bears to push women to the brink and fringes of deemed societal ladders.  The brunt of domestic violence lead to exploitation of women be it in the form of physical or be it mental and occurs chiefly in their matrimonial homes. The detrimental effect of this abuse and exploitation regrettably imprints itself not only on the suffering females but to the entire family including children which in turn causes monetary, health and societal issue and concerns.  What separates domestic violence from other crimes and abuses is that in Domestic violence there is familial interrelatedness and hence the victim is a known of the complainant. In contrast to the modern trends of raising the bar of rights for women the domestic violence is considered as a major road block and its waning as precursor for the society to bring female rights at par. However, on the other spectrum an alarming fact has been observed by the judiciary about the registration of sham cases against the husband or his family members or both for the purposes of harassment or extorting money. To carry off the said object the complaints are registered by overstating the facts, by addition of conjectures, overplaying the financial conditions of the husband or his family and misrepresenting relevant facts and materials. The supreme court opined in Rajesh Sharma v. State of U.P. that the bulk of the cases are filed impetuously and over trifling matters.   To comprehend the say of Indian judiciary on instances of false domestic violence and sham complaints registered by the complainant, secondary data-based research is conducted. Landmark Judgement from the apex court and different High Courts are also analyzed. Endeavor has been made to appreciate the liabilities and legal rights of both the parties in the cases of intimate partner violence which is perpetrated towards one another

    Delayed Repair of Infected Ruptured Patellar Tendon using Suture Anchors

    Patellar tendon rupture are rare injuries that are easily missed in acute phases if careful clinical examination is not carried out. The delayed condition is further difficult to treat and augmentation of end to end repair is generally required. However, literature presents no such case of delayed presentation with presence of infection. We here present one such case of delayed presentation of patellar tendon rupture at three weeks in a 52-year-old male patient. Usual techniques were not sufficient to allow early rehabilitation. Technique of suture anchors was planned for repair after thorough debridement. After this intervention, patient was put on aggressive rehabilitation protocol and he gained excellent range of motion. Patient was followed for one year and he showed no loss of movement or signs of infection. We thus recommend using anchor suture repair of patellar tendon that provides a stable and rigid fixation with possibility of early active rehabilitation even in delayed setting

    COVID-19 in Punjab, India: Epidemiological patterns, laboratory surveillance and contact tracing of COVID-19 cases, March–May 2020

    Background: In Punjab, first COVID-19 case was detected on March 5, 2020 followed by multiple clusters. Understanding the epidemiology of reported COVID-19 cases helps decision makers in planning future responses. We described the epidemiological patterns, laboratory surveillance and contact tracing of COVID-19 cases in Punjab. Methods: We analysed state's COVID-19 data from March–May 2020 to describe time, place and person distribution. We analysed the laboratory surveillance and contact tracing reports to calculate frequency of testing, sample positivity rate (PR) and contacts traced per case. Findings: A total of 2256 cases were reported from March–May 2020 (attack rate 75 cases/million and case fatality rate 2%). Attack rate was higher among males (81 cases/million males) and maximum affected age group was 60–69 years (164∙5 cases/million). Five of 22 districts reported almost half cases in May's first week. Mortality rate was highest among individuals >60 years (six deaths/million) and males (two deaths/million males). Of 45 deaths, 41 reported comorbidities [(hypertension (42%), diabetes (40%)]. COVID-19 testing increased from 46 samples/day (PR: 2%) in March's first week to 4000 samples/day (PR: 2∙5%) by May's end (2752 tests/million). Amritsar conducted 2035 tests/million (highest PR: 6∙5%) while Barnala conducted 4158 tests/million (lowest PR: 1%). For 2256 cases, 19,432 contacts were traced (nine contacts/case) with 11% positivity rate. Interpretation: COVID-19 in Punjab mostly affected males, >60 years of age and individuals with comorbid conditions. Many districts with less testing and contact tracing had higher positivity rate. We recommended to implement and ensure adequate testing and contact tracing in all the districts of Punjab

    Total upper and lower eyelid reconstruction remains a challenging task for plastic and reconstructive surgeons. Repair of full-thickness defects >50% of the horizontal length of the lids can be particularly complex. Here, we discuss a case of a 52-year-old male who presented to the emergency with avulsion of the right upper and lower eyelids associated with corneal injury and orbital floor fracture. Among all the possible choices of locoregional flap available, we chose the paramedian forehead flap for anterior lamella reconstruction of both upper and lower lids, and the posterior lamella was reconstructed with buccal mucosal graft while orbital floor reconstruction was carried out with prosthesis. This reconstructive modality resulted in satisfactory function and esthetic outcome
