    Translation to practice: a randomised controlled study of an evidenced based booklet targeted at breast care nurses in the United Kingdom

    BACKGROUND: In the United Kingdom (UK), it was documented that a problem of knowledge transfer existed within the speciality of breast-cancer care, thus depriving patients of receiving optimal care. Despite increasingly robust research evidence indicating recommendation of whole body exercise for people affected by breast cancer, commensurate changes to practice were not noted amongst breast-care nurses (BCNs). AIM: To evaluate the effect of a targeted booklet, Exercise and Breast Cancer: A Booklet for Breast-Care Nurses, on changes in knowledge, reported practice, and attitudes of BCNs in the UK. METHOD: A prospective, experimental approach was used for designing a pre- and post-test randomised controlled study. Comparisons of knowledge, reported practice, and attitudes based on responses to a questionnaire were made at two time-points in two groups of BCNs (control and experimental). The unit of randomisation and analysis was hospital clusters of BCNs. The sample comprised 92 nurses from 62 hospitals. Analysis consisted of descriptive statistics and clustered regression techniques: clustered logistic regression for knowledge items, clustered linear regression for knowledge scores, ologit for attitude and reported practice items, and clustered multiple regression for paired and multiple variable analysis. RESULTS: A statistically significant increase in knowledge and changes in reported practice and attitudes were found. Robust variables affecting knowledge acquisition were: promotion of health, promotion of exercise, and understanding how exercise can reduce cancer-related fatigue. DISCUSSION: The study has shown that evidence-based printed material, such as an information booklet, can be used as an effective research dissemination method when developed for needs, values, and context of a target audience. CONCLUSIONS: This practical approach to research dissemination could be replicated and applied to other groups of nurses.</p

    The Effects of Sugars Intake and Frequency of Ingestion on Dental Caries Increment in a Three-year Longitudinal Study

    A three-year longitudinal study was carried out with a group of children, initially aged 11-15, residing in non-fluoridated rural communities in south-central Michigan. This report analyzes the relation between caries increment and consumption of sugars from all sources to see if accepted relationships have changed with the caries decline in the United States. There were 499 children who provided three or more 24-hour dietary recall interviews, and who received dental examinations at baseline and after three years. Caries increment averaged 2.91 DMFS over the three years, with 81 % of new lesions on pit-and-fissure surfaces. Consumption of sugars from all sources averaged 156 g per day for males and 127 g per day for females, an average of 52 kg per person per year. Sugars constituted one-quarter of total caloric intake for both boys and girls, and the average number of eating occasions per day was 4.3. Children who consumed a higher proportion of their total energy intake as sugars had a higher increment of approximal caries, though there was little relation to pit-and-fissure caries. The average number of daily eating occasions was not related to caries increment, nor was the average number of sugary snacks (defined as foods with 15% or more of sugars) consumed between meals, but the average consumption of between-meal sugars was related to the approximal caries increment. When children were categorized by high caries increment compared with no caries increment, a tendency toward more frequent snacks was seen in the high-caries children. In an age of generally declining caries, it was concluded that higher average daily consumption of sugars, and higher between-meal consumption of sugars, was still a risk factor for children susceptible to approximal caries. Overall frequency of eating and frequency of ingestion of sugary foods between meals, however, were both poorly related to approximal caries increment.     Effects of manure history and nitrogen fertilizer rate on sugar beet production in the northwest U.S.

    Past manure applications effects on sugarbeet production needs to be assessed in the areas where manure applications to crop land are common. A study was conducted in Kimberly, Idaho in 2014 and 2016 to assess the effects of manure application history and N rates on sugarbeet production on a Portneuf silt loam (coarse-silty mixed mesic Durixerollic Calciorthid) soil. From 2004 to 2009, manure was applied to plots every two years (M1, total application = 60 tons per acre), every year (M2 total application = 106 tons per acre), or no manure (F, commercial fertilizer only). In spring 2014, the manure main plots were split in half with one half receiving a commercial fertilizer N rate treatment superimposed on the main plots in 2014 and the other half receiving the superimposed N rate treatments in 2016. In 2014 and 2016, the commercial fertilizer N rates were 0, 30, 56, 77, 100, 141, 180, and 202 pounds per acre. The study design was a randomized block split-plot with manure history as the main plot and N rate as the subplot. During both years of the study, N rate did not affect sugarbeet yields, but M1 and M2 treatments had higher sugarbeet root yields compared to the F treatment. Averaged across all N rates, root yields from both manured treatments were 12% and 36% greater than the F treatment in 2014 and 2016, respectively, although sugar yield was only significantly greater in 2016.
