    Poor written and oral text comprehension in third grade children. A multiple case study

    In this multiple case study we analyzed oral text comprehension, reading profiles and underlying cognitive abilities (attention, executive functions, working memory, narrative memory, rapid automatized naming and vocabulary) of 9 children identified as poor written text comprehenders after a school screening on 75 third grade children. Four out of the 9 children were named Language-Minority (L-M) children, since they had immigrant parents. The remaining 5 children were born in Italy from Italian parents. The comparisons of the two subgroups suggested that the lexical route of reading was particularly impaired in the L-M subgroup and that written text comprehension was weakened by restricted vocabulary which, in turn, was not supported by efficient phonological short-term memory. In a second type of data analysis we examined the individual profiles of the 9 children, irrespective of their belonging to the L-M or Italian subgroups, and identified different patterns of associations among reading performance, written text comprehension and oral text comprehension. The findings showed that poor text comprehension always co-occurred with word and/or text reading difficulties which, in turn, were associated to slow naming and weak verbal working memory. Moreover, when children had both written and oral text comprehension difficulties, not only verbal working memory was impaired but also narrative memory, suggesting a weakness in the episodic buffer (Baddeley 2000; 2010).     La diffusione tra i bambini di dispositivi elettronici interattivi (in particolare smartphone e tablet) Ăš un fenomeno recente ma in crescente espansione. In Italia le ricerche si sono concentrate finora sui bambini in etĂ  scolare e sugli adolescenti, mentre sono pochi gli studi sui prescolari. Nel nostro studio 473 genitori di bambini di 5-6 anni hanno compilato un questionario che indagava caratteristiche sociodemografiche e familiari, problematiche del bambino, tipologia dei dispositivi tecnologici domestici, caratteristiche dell’uso di smartphone e tablet da parte del bambino, opinioni dei genitori su tale uso. I risultati indicano che metĂ  dei bambini del campione utilizza uno smartphone ed il 48,5% un tablet. Confrontando bambini che usano i dispositivi mobili per almeno 2 ore al giorno (utilizzatori abituali, n=45) con bambini che li usano occasionalmente (utilizzatori occasionali, n=69) emerge che gli utilizzatori abituali hanno padri piĂč giovani, vivono in contesti dove sono maggiormente presenti dispositivi digitali, in cui i dispositivi mobili sono considerati strumenti di conoscenza e di promozione dell’intelligenza e vengono impiegati soprattutto per intrattenere il bambino in ambienti piĂč adatti agli adulti (es. ristoranti). Inoltre, negli utilizzatori abituali sono piĂč frequenti problematiche emotive, di autoregolazione e relative al sonno ed all’alimentazione. Parole chiave: bambini prescolari; tecnologie digitali; dispositivi mobili; problematiche cognitive ed emotive; questionario genitori.The spread of interactive electronic devices (especially smartphones and tablets) among children is a recent but increasingly expanding phenomenon. Research conducted so far in Italy has focused on school-aged children and adolescents, while there are still very few studies on preschoolers. In our study, 473 parents of 5-6 year old children completed a questionnaire on socio-demographic and familiar features, problems of the child, typology of home-based devices, characteristics of the use of smartphones and tablets by the child, opinions of parents on such use. The results indicate that the smartphone is used by half of the children and the tablet by 48,5% of them. Children using mobile devices for at least 2 hours a day (regular users, n=45) were compared with children who only occasionally use them (occasional users, n=69). The results show that, compared to the other group, regular users: a) have younger fathers; b) live in contexts where digital devices are more present; c) have parents who consider mobile devices as instruments of knowledge and promotion of intelligence and allow their children to use them mainly in environments more suited to adults (eg restaurants). Moreover, emotional, selfregulating, sleep and feeding problems are more frequent among regular users.     Training verbal working memory in children with mild intellectual disabilities: effects on problem-solving

    This multiple case study explores the effects of a cognitive training program in children with mild to borderline intellectual disability. Experimental training effects were evaluated comparing pre-post-test changes after (a) a baseline phase versus a training phase in the same participant, (b) an experimental training versus either a no intervention phase or a control training in two pairs of children matched for cognitive profile. Key elements of the training program included (1) exercises and card games targeting inhibition, switching, and verbal working memory, (2) guided practice emphasizing concrete strategies to engage in exercises, and (3) a variable amount of adult support.     Executive functions and symptom severity in an Italian sample of intellectually able preschoolers with autism spectrum disorder

    A novel battery (BAFE; Valeri etal. 2015) was used in order to assess three executive function (EF) abilities (working memory, inhibition and shifting) in a sample of 27 intellectually able preschoolers with autism spectrum disorder (ASD) compared with 27 typically developing children matched on age and nonverbal IQ. Differences in EF skills were analyzed in participants with distinct ASD symptom severity. Children with ASD performed worse than typical controls on both set-shifting and inhibition, but not on visuo-spatial working memory. Additionally, children with more severe ASD symptoms showed a worse performance on inhibition than children with milder symptoms. These results confirm the presence of EF deficits and highlight a link between ASD symptoms and EF impairments in preschool age

    Amniotic fluid stem cell-derived extracellular vesicles educate type 2 conventional dendritic cells to rescue autoimmune disorders in a multiple sclerosis mouse model

    Dendritic cells (DCs) are essential orchestrators of immune responses and represent potential targets for immunomodulation in autoimmune diseases. Human amniotic fluid secretome is abundant in immunoregulatory factors, with extracellular vesicles (EVs) being a significant component. However, the impact of these EVs on dendritic cells subsets remain unexplored. In this study, we investigated the interaction between highly purified dendritic cell subsets and EVs derived from amniotic fluid stem cell lines (HAFSC-EVs). Our results suggest that HAFSC-EVs are preferentially taken up by conventional dendritic cell type 2 (cDC2) through CD29 receptor-mediated internalization, resulting in a tolerogenic DC phenotype characterized by reduced expression and production of pro-inflammatory mediators. Furthermore, treatment of cDC2 cells with HAFSC-EVs in coculture systems resulted in a higher proportion of T cells expressing the regulatory T cell marker Foxp3 compared to vehicle-treated control cells. Moreover, transfer of HAFSC-EV-treated cDC2s into an EAE mouse model resulted in the suppression of autoimmune responses and clinical improvement. These results suggest that HAFSC-EVs may serve as a promising tool for reprogramming inflammatory cDC2s towards a tolerogenic phenotype and for controlling autoimmune responses in the central nervous system, representing a potential platform for the study of the effects of EVs in DC subsets

    Cervical cancer screening in women vaccinated against human papillomavirus infection: Recommendations from a consensus conference

    In Italy, the cohorts of women who were offered Human papillomavirus (HPV) vaccination in 2007/08 will reach the age (25 years) for cervical cancer (CC) screening from 2017. The simultaneous shift from cytology-based screening to HPV test-based screening gives the opportunity for unprecedented reorganisation of CC prevention. The ONS (National Screening Monitoring Centre) Directive and the GISCi (Italian Group for Cervical Screening) identified the consensus conference as the most suitable method for addressing this topic. A summary of consensus recommendations is reported here. The main objective was to define the best screening methods in girls vaccinated against HPV and the knowledge required for defining evidence-based screening strategies. A Jury made recommendations about questions and proposals formulated by a panel of experts representative of Italian scientific societies involved in CC prevention and based on systematic reviews of literature and evidence. The Jury considered changing the screening protocols for girls vaccinated in their twelfth year as appropriate. Tailored screening protocols based on vaccination status could be replaced by \u201cone size fits all\u201d protocols only when a herd immunity effect has been reached. Vaccinated women should start screening at age 30, instead of 25, with HPV test. Furthermore, there is a strong rationale for applying longer intervals for re-screening HPV negative women than the currently recommended 5 years, but research is needed to determine the optimal screening time points. For non-vaccinated women and for women vaccinated in their fifteenth year or later, the current protocol should be kept

    Peri-operative red blood cell transfusion in neonates and infants: NEonate and Children audiT of Anaesthesia pRactice IN Europe: A prospective European multicentre observational study

    BACKGROUND: Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12 g dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (≄week 3) onwards. OBJECTIVE: To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN: A multicentre observational study. SETTING: The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS: The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES: The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS: Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g dl-1 in week 2 and 8.0 [7.3 to 9.0] g dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS: Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies.
