Role of Biomarkers in Hepatocellular Carcinoma and Their Disease Progression
Hepatocellular carcinoma (HCC) is one of the third leading and common lethal cancers worldwide. Early detection of tumorigenesis of hepatocellular carcinoma is through ultrasonography, computerized tomography (CT) scans, and magnetic resonance imaging (MRI) scans; however, these methods are not up to the mark, so a search for an efficient biomarker for early diagnosis and treatment of hepatocarcinogenesis is important. Proteomic and genomic approaches aid to develop new promising biomarkers for the diagnosis of HCC at the early stages. These biomarkers not only help in prognosis but also provide better therapeutic intervention against HCC. Among the different biomarker candidates, liquid biopsy [including circulating tumor cells (CTCs) and circulating tumor DNA (ctDNA)] has recently emerged as a noninvasive detection technique for the characterization of circulating cells, providing a strong basis and early diagnosis for the individualized treatment of patients. Conservation agriculture with optimum fertilizer nitrogen rate reduces GWP for rice cultivation in floodplain soils
Wetland rice cultivation contributes significantly to global warming potential (GWP), an effect which is largely attributed to emissions of methane (CH4). Emerging technologies for wetland rice production such as conservation agriculture (CA) may mitigate greenhouse gas (GHG) emissions, but the effects are not well defined. Investigations were carried out in an irrigated rice (Boro rice) field in the fifth crop after conversion of conventional tillage (CT) to strip tillage (ST). Two crop residue levels (low versus high, LR versus HR) and three nitrogen (N) application rates (N1 = 108, N2 = 144, and N3 = 180 kg N ha−1) were laid out in a split-plot experiment with three replicates. Yield-scaled GHG emissions and GWP were estimated to evaluate the impacts of CA on mitigating CH4 and N2O emissions in the rice paddy field. There was a 55% higher N2O emission in ST with HR coupled with N3 than that in CT with LR coupled with N1. The N2O emission factors ranged from 0.43 to 0.75% in ST and 0.45 to 0.59% in CT, irrespective of the residue level and N rate. By contrast, CH4 emissions were significantly lower in CA than in the conventional practices (CT plus LR). The ST with LR in N2 reduced the GWP by 39% over the GWP in CT with HR in N1 and 16% over the conventional practices. Based on our investigation of the combination of tillage, residue, and N rate treatments, the adoption of CA with high and low residue levels reduced the GWP by 10 and 16%, respectively, because of lower CH4 and N2O emissions than the current management practices. The relatively high N2O emission factors suggest that mitigation of this GHG in wetland rice systems needs greater attention
Clinical Evaluation of Serum Alpha Feto-Protein (AFP) and Interlukin-6 in Hepatocellular Carcinoma
Background: Hepatocellular carcinoma (HCC) is one of the important cancers worldwide and predominant in Asia and Africa. A number of evidence suggests a possible role of interleukin-6 (IL-6) and α-Fetoprotein (AFP) in the pathogenesis of hepatocellular carcinoma (HCC).Patients and methods: We studied both IL-6 and AFP in patients with HCC or in healthy controls. Serum IL-6, AFP was measured by enzyme linked immunosorbent assay and Chemiluminescent immunoassay respectively in 30 patients with primary hepatocellular carcinoma and 30 normal subjectsResults:IL-6 and AFP was found in high levels in the serum of patients initially diagnosed with HCC (18±9.8), and (315.99±594.62) respectively compared with healthy subjects (4.29±2.10).and (3.13±1.27).A significant positive correlation was found between mean levels of IL- 6 & AFP in HCC (P < 0.05), Combination of IL-6 and AFP improved the sensitivity in diagnosing HCC or predicting future HCC development. Conclusions: IL-6 along with AFP could be considered a promising tumor marker for HCC. In particular, the diagnostic value of the test is significantly increased when combined with AFP
Long - term conservation agriculture increases nitrogen use efficiency by crops, land equivalent ratio and soil carbon stock in a subtropical rice - based cropping system
Conservation Agriculture (CA) is still a relatively new approach for intensively cultivated (3 crops yr-1) rice-based cropping systems that produce high crop yield and amounts of residues annually. With the recent development of transplanting of rice into tilled strips on non-puddled soil, CA could become feasible for rice-based cropping patterns. However, the effect of increased retention of crop residues on crop response to nitrogen (N) fertilization rate in strip tilled systems with the transplanted rice and other crops grown in the annual rotation is yet to be determined. For nine years, we have examined the effects of soil disturbance levels - strip tillage (ST) and conventional tillage (CT), two residue retention levels –15% residue by height (low residue, LR) and 30% residue (high residue, HR) and five N rates (60%, 80%, 100%, 120%, and 140% of the recommended N fertilizer doses (RFD)) for a rice-wheat-mungbean cropping sequence. The 100% RFD was 75, 100 and 20 kg N ha-1for rice, wheat, and mungbean, respectively. Rice yields were comparable between the two tillage systems for up to year-6, wheat for up to year-3 but mungbean yield markedly increased in ST from year-1; however, the land equivalent ratio increased from year-1, principally because of higher mungbean yield. Introduction of ST increased land equivalent ratio by 26% relative to CT, N use efficiency and partial factor productivity. Nitrogen fertilizer demand for maximum yield in ST was increased by about 10% for rice and 5% for mungbean but decreased by 5% for wheat. Although fertilizer N demand had increased in ST system due to higher yield than CT, the N requirement declined by50–90% when the same yield goal is considered for ST as for CT. The soil organic carbon stock (0–15 cm) after 8 years increased from 21.5 to 30.5 t ha-1 due to the effect of ST plus high crop residue retention. Annual gross margin increased by 57% in ST over CT practice and 26% in HR over LR retention. In conclusion, after 9 years practicing CA with increased residue retention under strip tillage, the crops had higher N use efficiency, grain yield, land equivalent ratio and annual gross margin in the rice-wheat-mungbean cropping system while the N fertilizer requirement increased minimally
A homozygous AKNA frameshift variant is associated with microcephaly in a Pakistani family
Primary microcephaly (MCPH) is a prenatal condition of small brain size with a varying degree of intellectual disability. It is a heterogeneous genetic disorder with 28 associated genes reported so far. Most of these genes encode centrosomal proteins. Recently, AKNA was recognized as a novel centrosomal protein that regulates neurogenesis via microtubule organization, making AKNA a likely candidate gene for MCPH. Using linkage analysis and whole-exome sequencing, we found a frameshift variant in exon 12 of AKNA (NM_030767.4: c.2737delG) that cosegregates with microcephaly, mild intellectual disability and speech impairment in a consanguineous family from Pakistan. This variant is predicted to result in a protein with a truncated C-terminus (p.(Glu913Argfs*42)), which has been shown to be indispensable to AKNA’s localization to the centrosome and a normal brain development. Moreover, the amino acid sequence is altered from the beginning of the second of the two PEST domains, which are rich in proline (P), glutamic acid (E), serine (S), and threonine (T) and common to rapidly degraded proteins. An impaired function of the PEST domains may affect the intracellular half-life of the protein. Our genetic findings compellingly substantiate the predicted candidacy, based on its newly ascribed functional features, of the multifaceted protein AKNA for association with MCPH

Trends over time in the knowledge, attitude and practices of pregnant women related to COVID-19: A cross-sectional survey from seven low- and middle-income countries
Objective: To understand trends in the knowledge, attitudes and practices (KAP) of pregnant women related to COVID-19 in seven low- and middle-income countries. Design: Multi-country population-based prospective observational study. Setting: Study sites in Bangladesh, the Demographic Republic of Congo (DRC), Guatemala, India (two sites), Kenya, Pakistan and Zambia. Population: Pregnant women in the Global Network's Maternal and Neonatal Health Registry (MNHR). Methods: Pregnant women enrolled in the MNHR were interviewed to assess their KAP related to COVID-19 from September 2020 through July 2022 across all study sites. Main outcome measures: Trends of COVID-19 KAP were assessed using the Cochran–Armitage test for trend. Results: A total of 52 297 women participated in this study. There were wide inter-country differences in COVID-19-related knowledge. The level of knowledge of women in the DRC was much lower than that of women in the other sites. The ability to name COVID-19 symptoms increased over time in the African sites, whereas no such change was observed in Bangladesh, Belagavi and Guatemala. All sites observed decreasing trends over time in women avoiding antenatal care visits. Conclusions: The knowledge and attitudes of pregnant women related to COVID-19 varied substantially among the Global Network sites over a period of 2 years; however, there was very little change in knowledge related to COVID-19 over time across these sites. The major change observed was that fewer women reported avoiding medical care because of COVID-19 across all sites over time
COVID-19 symptoms and antibody positivity among unvaccinated pregnant women: An observational study in seven countries from the Global Network
Objective: To determine the relation of COVID-19 symptoms to COVID-19 antibody positivity among unvaccinated pregnant women in low- and middle-income countries (LMIC). Design: COVID-19 infection status measured by antibody positivity at delivery was compared with the symptoms of COVID-19 in the current pregnancy in a prospective, observational cohort study in seven LMICs. Setting: The study was conducted among women in the Global Network for Women's and Children's Health's Maternal and Newborn Health Registry (MNHR), a prospective, population-based study in Kenya, Zambia, the Democratic Republic of the Congo (DRC), Bangladesh, Pakistan, India (Belagavi and Nagpur sites) and Guatemala. Population: Pregnant women enrolled in the ongoing pregnancy registry at study sites. Methods: Data on COVID-19 symptoms during the current pregnancy were collected by trained staff between October 2020 and June 2022. COVID-19 antibody testing was performed on samples collected at delivery. The relation between COVID-19 antibody positivity and symptoms was assessed using generalised linear models with a binomial distribution adjusting for site and symptoms. Main outcome measures: COVID-19 antibody status and symptoms of COVID-19 among pregnant women. Results: Among 19 218 non-vaccinated pregnant women who were evaluated, 14.1% of antibody-positive women had one or more symptoms compared with 13.4% in antibody-negative women. Overall, 85.3% of antibody-positive women reported no COVID-19 symptoms during the present pregnancy. Reported fever was significantly associated with antibody status (relative risk [RR] 1.10, 95% CI 1.03–11.18; P = 0.008). A multiple variable model adjusting for site and all eight symptoms during pregnancy showed similar results (RR 1.13, 95% CI 1.04–1.23; P = 0.012). None of the other symptoms was significantly related to antibody positivity. Conclusions: In a population-based cohort in LMICs, unvaccinated pregnant women who were antibody-positive had slightly more symptoms during their pregnancy and a small but significantly greater increase in fever. However, for prevalence studies, evaluating COVID-19-related symptoms does not appear to be useful in differentiating pregnant women who have had a COVID-19 infection
Knowledge, attitudes, and practices of pregnant women regarding COVID-19 vaccination in pregnancy in 7 low- and middle-income countries: An observational trial from the Global Network for Women and Children’s Health Research
Objectives: We sought to determine the knowledge, attitudes and practices of pregnant women regarding COVID-19 vaccination in pregnancy in seven low- and middle-income countries (LMIC). Design: Prospective, observational, population-based study. Settings: Study areas in seven LMICs: Bangladesh, India, Pakistan, Guatemala, Democratic Republic of the Congo (DRC), Kenya and Zambia. Population: Pregnant women in an ongoing registry. Methods: COVID-19 vaccine questionnaires were administered to pregnant women in the Global Network's Maternal Newborn Health Registry from February 2021 through November 2021 in face-to-face interviews. Main outcome measures: Knowledge, attitude and practice regarding vaccination during pregnancy; vaccination status. Results: No women were vaccinated except for small proportions in India (12.9%) and Guatemala (5.5%). Overall, nearly half the women believed the COVID-19 vaccine is very/somewhat effective and a similar proportion believed that the COVID-19 vaccine is safe for pregnant women. With availability of vaccines, about 56.7% said they would get the vaccine and a 34.8% would refuse. Of those who would not get vaccinated, safety, fear of adverse effects, and lack of trust predicted vaccine refusal. Those with lower educational status were less willing to be vaccinated. Family members and health professionals were the most trusted source of information for vaccination. Conclusions: This COVID-19 vaccine survey in seven LMICs found that knowledge about the effectiveness and safety of the vaccine was generally low but varied. Concerns about vaccine safety and effectiveness among pregnant women is an important target for educational efforts to increase vaccination rates
