    Investigations that were conducted with the help of overhead distribution line model reflected such situations as wire swinging cross, their contact with fallen trees and other earthed objects have shown that the overhead lines may operate rather long under such conditions. However leakage current may lead to dangerous high potentials on these objects and around them.The results of calculations and experimental research have revealed that in the rain leakage current on an earthed object and insulation resistance depends on the width of moistened zone and practically does not depend on an object size and shape. Leakage current does not exceed 3 mA at an operating voltage and 15mA at possible internal overvoltage and full insulation resistance is not less than 3…4 MW for actual overhead distribution line spans.Исследования, проведенные на макете ВЛП при схлестывании проводов и попадании на них упавших деревьев и других заземленных предметов, показали, что в таком режиме ВЛП может работать длительное время. Однако за счет токов утечки опасные высокие потенциалы могут появиться на этих предметах и вокруг них. Приводятся результаты расчетов и экспериментальных исследований, которые показывают, что под дождем ток утечки на заземленный предмет и сопротивление изоляции зависят от ширины увлажненной зоны и практически не зависят от размеров и формы предмета. Для реальных пролетов ВЛП ток утечки не превышает 3 м × А при рабочем напряжении и 15 м × А – при возможных внутренних перенапряжениях, а полное сопротивление изоляции – не менее 3…4 м × Ом

    ABSTRACT. The paper describes the first experience of successful small bowel transplantation in the Sklifosovsky Research Institute for Emergency Medicine. It contains a brief review of literature on essential aspects of small bowel transplantation, presents the donor’s case history, specific features of anesthesia, surgical procedure, and postoperative course, including the immunosuppression scheme, diagnosis and treatment of evolved complications. The paper provides in-detail results of morphological control over rejection development, presents the data of ultrasonographic observation of the changes in the transplanted intestine in the course of its adaptation to new requirements, covers the tactics of parenteral and enteral nutrition.РЕЗЮМЕ. Статья посвящена описанию первого опыта успешной трансплантации тонкой кишки (ТТК) в Институте скорой помощи им. Н.В. Склифосовского. Содержит краткий обзор литературы по основным проблемам ТТК, описание истории болезни донора, особенностей проведения наркоза, операции и течения послеоперационного периода, включая тактику иммуносупрессии, диагностики и лечения развившихся осложнений. Подробно описаны результаты морфологического контроля за развитием реакции отторжения, приведены данные УЗ-наблюдения за изменениями трансплантированной кишки по ходу ее адаптации к новым условиям, тактика парентерального и энтерального питания.

    Severe cardiovascular system involvement in a patient with Marfan’s syndrome

    The given clinical case of a 14-year-old child with Marfan’s syndrome demonstrates the early severe cardiovascular system diseases, such as dilated cardiomyopathy and aortic root dilatation, that required surgical treatment when he was 9 years old, and grades 2 and 3 insufficiency of the mitral and aortic valves, respectively. The paper presents the modern view of the involvement of the heart and vessels in Marfan’s syndrome (cardiomyopathy as a new sign of the disease, arterial dissection). It shows new possibilities for treating cardiovascular disorders in children with this disease, such as pathogenetic therapy with losartan, anti-transforming growth factor-β antibodies
