    Employees’ emotional awareness as an antecedent of organizational commitment—The mediating role of affective commitment to the leader

    Commitment has been perceived as a strategic topic in organizations due to its positive effect on retaining talent, increasing performance, or boosting employees’ innovative behavior. However there are many focis of commitment in the workplace, which has represented a challenge to human resources management, who need implement measures to improve the employee’s commitment. Recent research has suggested a need to conduct studies about commitment, namely antecedents and the relationship between different focis, to understand the dynamic and directionality between them. Hence, the purpose of this work is to analyze how employees’ emotional awareness relates with two focis of commitment (the leader and the organization), also assessing the mediating role of affective commitment to the leader. The study uses structural equation modeling and Lisrel to test the hypotheses considering the multidimensionality of organizational commitment (affective; normative; and continuance), employees emotional awareness (understanding self-emotions; self-control when facing criticism; and understanding others’ emotions), and the affective commitment to the leader, under the scope of Social Exchange Theory. The Mackinon’s Z Test was used to assess the mediation role of affective commitment to the leader. The sample is composed for 403 employees from two multinational companies. The results provide empirical evidence about the mediating role of affective commitment to the leader in the relationship between employees’ emotional awareness and organizational commitment, and the employees’ emotional awareness as an antecedent of commitment.     Value of α‑fetoprotein as an early biomarker for treatment response to sorafenib therapy in advanced hepatocellular carcinoma

    Sorafenib is an oral multikinase inhibitor with antiangiogenic and antiproliferative properties, and is used as the first‑line treatment for patients with advanced hepatocellular carcinoma (HCC). Previous studies have identified an improvement in overall survival and progression‑free survival in patients with a manageable toxicity profile. α‑fetoprotein (AFP) has been revealed to be of great diagnostic and predictive value for tumour staging in multiple studies; however, its role as a predictive factor of response to treatment with sorafenib is not entirely clear. The present study aimed to determine the effectiveness of sorafenib and investigate the value of AFP as a predictive factor of early response to sorafenib in patients with HCC. Effectiveness was analysed based on median overall survival (mOS) time, while to analyse the possible predictive value of AFP, patients were classified into two groups: Non‑responders (≤20% AFP reduction) and responders (>20% AFP reduction) at 6‑8 weeks of treatment when compared with basal AFP level. For assessment of toxicity, any adverse effects were recorded. A total of 167 patients were included, who collectively exhibited a mOS time of 11 months with a median treatment duration of 5 months. The mOS time was significantly higher for patients with better hepatic function (12 months in cases of Child‑Pugh score A vs. 8 months in cases of Child‑Pugh score B; P=0.03) and with basal AFP values ≤200 ng/ml (14 months vs. 8 months in patients with AFP levels >200 ng/ml; P=0.01). A >20% reduction of AFP at 6‑8 weeks was determined to be a positive predictive factor upon multivariate analysis (P=0.002), obtaining, for the responder patients, an mOS of 18 months compared with 10 months (P=0.004) for the non‑responders. The main adverse reactions were hand‑foot syndrome (35/167; 21%), diarrhoea (39/167; 23.4%), anorexia (29/167; 17.4%) and arterial hypertension (30/167; 18%). In conclusion, a >20% drop in AFP at 6‑8 weeks may be useful as a predictive factor of response to sorafenib, as indicated by its association with longer survival times in patients with advanced HCC following treatment with sorafenib in the present study

    Análisis de las barreras percibidas por los deportistas de élite españoles para acceder a los estudios

    Los objetivos del presente estudio fueron determinar si la formación académica de los deportistas de élite, la carga de entrenamiento, la dificultad para conciliar estudios y deporte y las barreras percibidas para estudiar son diferentes en función del tipo de deporte practicado y del género. Se utilizó un estudio descriptivo transversal mediante encuestas, con muestreo intencional, administrando un cuestionario de preguntas cerradas, elaborado ad hoc. Participaron un total de 648 deportistas de élite, de ellos, 418 eran deportistas de deportes individuales y 230 deportes colectivos. La carga de entrenamiento fue superior entre deportistas que practicaba deportes individuales. El nivel académico fue superior en los deportistas de deportes colectivos frente a los de deportes individuales. Los deportistas de deportes individuales percibieron una mayor di$cultad para conciliar su vida deportiva y los estudios. Así mismo, también mostraron en mayor grado barreras de tipo individual (estoy cansado habitualmente, me da pereza y pierdo el ritmo de los cursos) que los deportistas de deportes colectivos. Las mujeres mostraron en mayor medida que los hombres barreras relacionadas con la gestión del tiempo (no tengo tiempo, los horarios de los estudios no son %exibles).     Learning context effects : study abroad, formal instruction and international immersion classrooms

    This book deals with the effects of three different learning contexts mainly on adult, but also on adolescent, learners' language acquisition. The three contexts brought together in the monograph include i) a conventional instructed second language acquisition (ISLA) environment, in which learners receive formal instruction in English as a Foreign Language (EFL); ii) a Study Abroad (SA) context, which learners experience during mobility programmes, when the target language is no longer a foreign but a second language learnt in a naturalistic context; iii) the immersion classroom, also known as an integrated content and language (ICL) setting, in which learners are taught content subjects through the medium of the target language-more often than not English, used as the Lingua Franca (ELF). The volume examines how these contexts change language learners' linguistic performance, and also non-linguistic, that is, it throws light on how motivation, sense of identity, interculturality, international ethos, and affective factors develop. To our knowledge, no publication exists which places the three contexts on focus in this monograph along a continuum, as suggested in Pérez-Vidal (2011, 2014), with SA as 'the most naturalistic' context on one extreme, ISLA on the other, and ICL somewhere in between, while framing them all as international classrooms. Concerning target languages, the nine chapters included in the volume analyze English, and one chapter deals with Spanish, as the target language. As for target countries in SA programmes, data include England, Ireland, France, Germany, and Spain in Europe, but also Canada, China, and Australia. While the main bulk of the chapters deal with tertiary level language learners, a language learning population which has received less attention by research thus far, one chapter deals with adolescent learners

    The Control of Zoonotic Soil-Transmitted Helminthoses Using Saprophytic Fungi

    Soil-transmitted helminths (STHs) are parasites transmitted through contact with soil contaminated with their infective eggs/larvae. People are infected by exposure to human-specific species or animal species (zoonotic agents). Fecal samples containing eggs of Ascaris suum or Lemurostrongylus sp. were sprayed with spores of the soil saprophytic filamentous fungi Clonostachys rosea (CR) and Trichoderma atrobrunneum (TA). The antagonistic effect was assessed by estimating the viability of eggs and their developmental rate. Compared to the controls (unexposed to fungi), the viability of the eggs of A. suum was halved in CR and decreased by two thirds in TA, while the viability of the eggs of Lemurostrongylus sp. was reduced by one quarter and one third in CR and TA treatments, respectively. The Soil Contamination Index (SCI), defined as the viable eggs that attained the infective stage, reached the highest percentages for A. suum in the controls after four weeks (66%), with 21% in CL and 11% in TA. For Lemurostrongylus sp., the values were 80%, 49%, and 41% for control, CR and TA treatments, respectively.     We present the case of a 35-year-old patient with a diagnosis of congenital middle aortic syndrome corrected in childhood with aortic bypass interposition of a Dacron conduit. During follow-up, she was diagnosed with arterial hypertension, for which antihypertensive treatment was started and a stent was implanted in the conduit. A control CT of the aorta revealed a rib lesion suggestive of osteochondroma that compressed the duct. Cardiac catheterization confirmed hemodynamic stenosis due to extrinsic compression of the rib lesion at the level of the previously implanted stent. The CT imaging study helped to diagnose a complication at the level of the aorta that was  undetected in other imaging techniques.Presentamos el caso de una paciente de 35 años con diagnóstico de síndrome de aorta media congénita corregida en la infancia con bypass aórtico con interposición de un conducto de dacrón. Durante el seguimiento fue diagnosticada de hipertensión arterial por lo que se inició tratamiento antihipertensivo y se implantó un stent en el conducto. En un TC de aorta de control se apreció una lesión costal sugestiva de osteocondroma que comprimía el conducto. Se realizó un cateterismo cardiaco que confirmó la estenosis hemodinámica por compresión extrínseca de la lesión costal a nivel del stent implantado previamente. El estudio con imagen con TC ayudó a diagnosticar una complicación a nivel de la aorta que había pasado desapercibida en otras técnicas de imagen

    In this paper we analize the role of the university library as a provider of quality information services that collaborate with the basic functions of the University and its commitment with society: teaching well prepared professionals in vast areas of knowledge who possess the capacity of acquiring all the new information that is produced in their field and in their working environment. Skills and abilities in the search and retrieval of information is a must in the formation of our students, if we want them to be immersed, not only in the informational context and in the new methodologies of information, but also in the steady process of scientific research that a good university education implies, process that any country needs for getting a sustainable development. Our proposal of informational literacy includes required application forms, interviews with professors and instructors of classroom courses, an interactive manual for the development of competence in the use of digital resources the university offers, and one on line course presenting the manual.Se analiza el rol de la biblioteca como prestadora de servicios de información de calidad que coadyuve en las funciones sustantivas de la universidad, y del compromiso que ésta tiene con la sociedad, en formar profesionistas sólidamente preparados en sus respectivas áreas de conocimiento y como poseedores de la información, que les permita una constante actualización en sus campos de conocimiento y en su entorno laboral. Los recursos y habilidades en la búsqueda y recuperación de información son herramientas que no debemos soslayar en la formación de nuestros educandos, para lograr una educación de altura en el contexto informacional y en las nuevas tecnologías de información, así como en el continuo proceso de la investigación científica que todo país requiere para su propio desarrollo. En este entorno la biblioteca debe constituirse como un bastión fundamental para este proceso de formación que haga posible a la Universidad cumplir sus fines de docencia, investigación y extensión de la cultura.     Rebleeding prophylaxis improves outcomes in patients with hepatocellular carcinoma. A multicenter case-control study

    Outcome of variceal bleeding (VB) in patients with hepatocellular carcinoma (HCC) is unknown. We compared outcomes after VB in patients with and without HCC. All patients with HCC and esophageal VB admitted between 2007 and 2010 were included. Follow-up was prolonged until death, transplantation, or June 2011. For each patient with HCC, a patient without HCC matched by age and Child-Pugh class was selected. A total of 292 patients were included, 146 with HCC (Barcelona Classification of Liver Cancer class 0-3 patients, A [in 25], B [in 29], C [in 45], and D [in 41]) and 146 without HCC. No differences were observed regarding previous use of prophylaxis, clinical presentation, endoscopic findings, and initial endoscopic treatment. Five-day failure was similar (25% in HCC versus 18% in non-HCC; P = 0.257). HCC patients had greater 6-week rebleeding rate (16 versus 7%, respectively; P = 0.025) and 6-week mortality (30% versus 15%; P = 0.003). Fewer patients with HCC received secondary prophylaxis after bleeding (77% versus 89%; P = 0.009), and standard combination therapy was used less frequently (58% versus 70%; P = 0.079). Secondary prophylaxis failure was more frequent (50% versus 31%; P = 0.001) and survival significantly shorter in patients with HCC (median survival: 5 months versus greater than 38 months in patients without HCC; P < 0.001). Lack of prophylaxis increased rebleeding and mortality. On multivariate analysis Child-Pugh score, presence of HCC, portal vein thrombosis, and lack of secondary prophylaxis were predictors of death. Conclusions: Patients with HCC and VB have worse prognosis than patients with VB without HCC. Secondary prophylaxis offers survival benefit in HCC patient

    ALL-268 genetic classification of B-Cell precursor adult acute lymphoblastic leukemia patients enrolled in LAL19 trial from the pethema group: response to treatment and survival

    Context: B-cell precursor acute lymphoblastic leukemia (BCP ALL) is a genetically heterogeneous neoplasm with >20 biologic subtypes. Each subtype shows specific genetic traits that determine relapse risk and patients' survival. Objectives: To establish the genetic subtype (primary alteration) of adult BCP ALL patients enrolled in the PETHEMA LAL19 trial (NCT 04179929) and to correlate them with measurable residual disease (MRD) level and survival. Patients and Methods: In the LAL19 trial (NCT04179929), Ph-negative patients (18–65 y) with MRD≥0.01% at day+35 or high-risk genetics receive alloHSCT and MRD<0.01% patients with standard-risk genetics receive maintenance chemotherapy. The genetic analyses are centralized: FISH and NGS DNA panel (Hospital de Salamanca), RNAseq panel (Hospital 12 de Octubre), FISH panel (Hospital La Fe), and SNP array (Josep Carreras Institute/ICO-Hospital Germans Trias i Pujol). MRD determinations are centrally done by next-generation flow cytometry in the Cytometry Service, NUCLEUS, University of Salamanca. Results: The genetic subtype was identified in 54% (82/152) of patients. The most recurrent subtypes were KMT2Ar (11%), Ph-like (mostly CRLF2::IGH, 11%), low-hypodiploid (7%), PAX5 P80R (7%), high-hyperdiploid (6%), and t(1;19)/TCF3::PBX1 (6%). In addition, t(12;21)/ETV6::RUNX1, ZNF384r, and iAMP21 subtypes (1.5% each) and MEF2Dr, MYCr, IDH1 R132 subtypes (<1% each) were found. Regarding secondary alterations, NRAS (15%), TP53 (13%), PAX5 (13%), and KRAS (10%) mutations were the most frequently observed. Twelve patients were refractory (mainly low-hypodiploid, Ph-like, MYCr, and B-other/unclassified patients). Statistically significant differences were observed for day+35 MRD levels between genetic subtypes. Ph-like, low-hypodiploid, and KMT2Ar showed lower frequencies of MRD<0.01% (17%, 33%, and 57%, respectively) than patients with PAX5P80R (100%), t(1;19)/TCF3::PBX1 (83%), and high-hyperdiploid (75%) (P=0.006). Despite the short median follow-up (11 months), differences in response to treatment were reflected in patients' survival. Significant differences in survival were observed between poor-response subtypes (Ph-like, KMT2Ar, and low-hypodiploid) and good-response subtypes (PAX5 P80R, t(1;19)/TCF3::PBX1, and high-hyperdiploid). Conclusions: Knowing the genetic subtype of each ALL is crucial to better predict relapse risk and offer the best (personalized) treatment for each patient

    Use of eltrombopag for patients 65 years old or older with immune thrombocytopenia

    Background Eltrombopag is useful for immune thrombocytopenia (ITP). However, results of clinical trials may not accurately mirror clinical practice reality. Here we evaluated eltrombopag for primary and secondary ITP in our ≥65‐year‐old population. Methods A total of 106 primary ITP patients (16 with newly diagnosed ITP, 16 with persistent ITP, and 74 with chronic ITP) and 39 secondary ITP patients (20 with ITP secondary to immune disorders, 7 with ITP secondary to infectious diseases, and 12 with ITP secondary to lymphoproliferative disorders [LPD]) were retrospectively evaluated. Results Median age of our cohort was 76 (interquartile range, IQR, 70‐81) years. 75.9% of patients yielded a platelet response including 66.2% complete responders. Median time to platelet response was 14 (IQR, 8‐21) days. Median time on response was 320 (IQR, 147‐526) days. Sixty‐three adverse events (AEs), mainly grade 1‐2, occurred. The most common were hepatobiliary laboratory abnormalities (HBLAs) and headaches. One transient ischemic attack in a newly diagnosed ITP and two self‐limited pulmonary embolisms in secondary ITP were the only thrombotic events observed. Conclusion Eltrombopag showed efficacy and safety in ITP patients aged ≥65 years with primary and secondary ITP. However, efficacy results in LPD‐ITP were poor.
