Increased risk of malignancies in a population-based study of 818 soft-tissue sarcoma patients
Soft-tissue sarcomas (STS) have been associated with various rare cancer syndromes and occur at increased frequencies in survivors of childhood cancer. Also adult patients with STS have been suggested to be at an increased risk of additional malignancies. After exclusion of syndrome-associated and radiation-induced sarcomas, we studied multiple primary malignancies in a population-based cohort of 818 patients with primary STS of the extremities and the trunk wall. In total, 203 other malignancies developed in 164 (20%) patients median 10 (0–32) years before and median 4 (0–35) years after the sarcoma diagnosis. Standardised morbidity ratios (SMRs) were determined for primary malignancies following a STS. Hereby individuals who had developed a STS were identified to be at increased risk of second primary malignancies (SMR for all malignant tumours=1.3; 95% CI=1.0–1.5; P=0.02) with STS being the only specific tumour type that occurred at an increased risk (SMR=17.6; 95% CI=8.1–33.5; P<0.001). Hence, this population-based series demonstrates a high frequency of second primary tumours among STS patients and indicates a particularly increased risk of developing a new STS
Intervening to improve health indicators among Australian farm families
The Sustainable Farm Families project (http://www.sustainablefarmfamilies.org.au/) was a 3-year demonstration and education project designed to influence farmer behavior with respect to family health and well-being among cropping and grazing farmers in Victoria, New South Wales, and South Australia, Australia. The project was conducted by the Western District Health Service, Hamilton, Australia, in partnership with farmers; Farm Management 500 (peer discussion group); the Victorian Farmers Federation; Royal Melbourne Institute of Technology; and Land Connect. During the 3 years of the project, 128 farmers—men (70) and women (58)—were enrolled. The project utilized a combination of small group workshops, individualized health action plans, and health education opportunities to encourage farm safety and health behavior changes and to elicit sustained improvements in the following health indicators: body mass index (BMI), total cholesterol, fasting blood glucose, and blood pressure. Mean changes in these health indicators were analyzed using repeated measures analysis of variance (ANOVA) and McNemar\u27s test compared the proportion of individuals with elevated indicators. Among participants with elevated values at baseline, the following average reductions were observed: BMI 0.44 kg/m2 (p = .0034), total cholesterol 48.7 mg/dl (p < .0001), blood glucose 10.1 mg/dl (p = .0016), systolic blood pressure 12.5 mm Hg (p < .0001), and diastolic blood pressure 5.0 mm Hg (p = .0007). The proportion of participants with elevated total cholesterol at baseline decreased after 24 months (p < .001). Such findings suggest that proactive intervention by farmer associations, rural health services, and government agencies may be an effective vehicle for promoting voluntary farm safety and health behavior change while empowering farm families to achieve measurable reductions in important health risk factors. <br /
Measurement of migration of a humeral head resurfacing prosthesis using radiostereometry without implant marking: An experimental study
Today, the shoulder joint is the third most commonly replaced joint after the hip and knee joints and the incidence is increasing. In Sweden, 1863 primary Shoulder Arthroplasties and 195 revisions were performed in 2017. The most common diagnoses are Osteoarthritis and irreparable tears of the rotator cuff, with or without arthropathy, often referred to as cuff tear arthropathy.
Different Shoulder Arthroplasty (SA) concepts include anatomical total shoulder arthroplasty (TSA), hemiarthroplasty (HSA) and reversed shoulder arthroplasty, but also humeral head resurfacing (HHR) and stemless arthroplasties. All concepts offer pain relief, improvement of function and in quality of life for the different diagnoses. Unfortunately, there are sometimes complications after SA. They involve periprosthetic joint infection, humeral and glenoid fractures, stress shielding, loosening of the glenoid and humeral component but also glenoid erosion and cuff rupture. Some of these complications are most common within 1 year after operation, some after several years, both may lead to a revision. This, together with the fact that new designs of implants and methods of fixation of SA continues to develop, stresses the importance of continuous monitoring of implant survival and follow-up.
The overall aim of this thesis was to describe clinical examples of different methods to assess the outcome after Shoulder Arthroplasty. The most common methods are clinical examination, radiographic assessment, Patient Reported Outcome Measure (PROM), National Joint registries, where revisions are an important outcome, but also Clinical Trials. All of these methods are used in one or more of the 4 papers in this thesis and shows the complexity of the topic and the practical work.
In paper I we used Radio Stereometric Analysis (RSA) in an experimental set-up and concluded that marker-free RSA can be used for a humeral head resurfacing arthroplasty. In paper II we used data from the Swedish Shoulder Arthroplasty Registry (SSAR) with PROM and revisions to conclude that age is the only factor that affects revision when comparing HSA and HHR. Paper III is a long-time follow-up of a Randomized controlled study where we used radiological assessment, PROM and revisions. The conclusion was that both TSA and HSA develop severe radiological changes 10 year after primary operation. Paper IV is a prospective RSA cohort study where we also evaluated PROM and revisions. The conclusion is that HHR seems to obtain a secure fixation in the humerus, after an initial migration. But also that the prostheses shows continuous glenoid wear.
The main conclusion of this thesis is that patient’s operated with SA needs continuous monitoring and several methods may be used to evaluate the outcome

What can comparative genomics tell us about species concepts in the genus Aspergillus?
Understanding the nature of species” boundaries is a fundamental
question in evolutionary biology. The availability of genomes from several
species of the genus Aspergillus allows us for the first time to
examine the demarcation of fungal species at the whole-genome level. Here, we
examine four case studies, two of which involve intraspecific comparisons,
whereas the other two deal with interspecific genomic comparisons between
closely related species. These four comparisons reveal significant variation
in the nature of species boundaries across Aspergillus. For example,
comparisons between A. fumigatus and Neosartorya fischeri
(the teleomorph of A. fischerianus) and between A. oryzae
and A. flavus suggest that measures of sequence similarity and
species-specific genes are significantly higher for the A. fumigatus
- N. fischeri pair. Importantly, the values obtained from the
comparison between A. oryzae and A. flavus are remarkably
similar to those obtained from an intra-specific comparison of A.
fumigatus strains, giving support to the proposal that A. oryzae
represents a distinct ecotype of A. flavus and not a distinct
species. We argue that genomic data can aid Aspergillus taxonomy by
serving as a source of novel and unprecedented amounts of comparative data, as
a resource for the development of additional diagnostic tools, and finally as
a knowledge database about the biological differences between strains and
species
A Randomized Community-based Intervention Trial Comparing Faith Community Nurse Referrals to Telephone-Assisted Physician Appointments for Health Fair Participants with Elevated Blood Pressure
To measure the effect of faith community nurse referrals versus telephone-assisted physician appointments on blood pressure control among persons with elevated blood pressure at health fairs.
Randomized community-based intervention trial conducted from October 2006 to October 2007 of 100 adults who had an average blood pressure reading equal to or above a systolic of 140 mm Hg or a diastolic of 90 mm Hg obtained at a faith community nurse-led church health event. Participants were randomized to either referral to a faith community nurse or to a telephone-assisted physician appointment. The average enrollment systolic blood pressure (SBP) was 149 ± 14 mm Hg, diastolic blood pressure (DBP) was 87 ± 11 mm Hg, 57% were uninsured and 25% were undiagnosed at the time of enrollment.
The follow-up rate was 85% at 4 months. Patients in the faith community nurse referral arm had a 7 ± 15 mm Hg drop in SBP versus a 14 ± 15 mm Hg drop in the telephone-assisted physician appointment arm (p = 0.04). Twenty-seven percent of the patients in the faith community nurse referral arm had medication intensification compared to 32% in the telephone-assisted physician appointment arm (p = 0.98).
Church health fairs conducted in low-income, multiethnic communities can identify many people with elevated blood pressure. Facilitating physician appointments for people with elevated blood pressure identified at health fairs confers a greater decrease in SBP than referral to a faith community nurse at four months
Polyphasic taxonomy of Aspergillus section Fumigati and its teleomorph Neosartorya
The taxonomy of Aspergillus section Fumigati with its
teleomorph genus Neosartorya is revised. The species concept is based
on phenotypic (morphology and extrolite profiles) and molecular
(β-tubulin and calmodulin gene sequences) characters in a polyphasic
approach. Four new taxa are proposed: N. australensis N. ferenczii, N.
papuaensis and N. warcupii. All newly described and accepted
species are illustrated. The section consists of 33 taxa: 10 strictly
anamorphic Aspergillus species and 23 Neosartorya species.
Four other Neosartorya species described previously were not
available for this monograph, and consequently are relegated to the category
of doubtful species
A barrier to homologous recombination between sympatric strains of the cooperative soil bacterium Myxococcus xanthus
The bacterium Myxococcus xanthus glides through soil in search of prey microbes, but when food
sources run out, cells cooperatively construct and sporulate within multicellular fruiting bodies.
M. xanthus strains isolated from a 16 × 16-cm-scale patch of soil were previously shown to have
diversified into many distinct compatibility types that are distinguished by the failure of swarming
colonies to merge upon encounter. We sequenced the genomes of 22 isolates from this population
belonging to the two most frequently occurring multilocus sequence type (MLST) clades to trace
patterns of incipient genomic divergence, specifically related to social divergence. Although
homologous recombination occurs frequently within the two MLST clades, we find an almost
complete absence of recombination events between them. As the two clades are very closely related
and live in sympatry, either ecological or genetic barriers must reduce genetic exchange between
them. We find that the rate of change in the accessory genome is greater than the rate of amino-acid
substitution in the core genome. We identify a large genomic tract that consistently differs between
isolates that do not freely merge and therefore is a candidate region for harbouring gene(s)
responsible for self/non-self discrimination
