    Emergent and delayed hybrid external fixation management of tibial pilon fractures: A multicentric retrospective analysis of 80 patients

    Objective: To report our experience with the hybrid external fixator in emergency.Methods: We assessed 80 cases of pilon fracture treated with the external fixator during the period of January 2009 and December 2016:55 men (69%) and 30 women (33%) with a mean age of 40 years (range between 16 and 70). About 45 occurred as isolated trauma, 35 instead were politrauma. Each patient underwent standard radiographic examination and a CT examination. There were 28 open fractures (35%), (Gustilo type 1, 2 and 3) while closed fractures showed soft tissue involvement of various grade (2-3 Tscherne classification). In all cases, the external fixation, sometimes associated with other reduction and synthesis techniques, was used. The timing of surgery was dictated by the condition of the soft tissues. For clinical evaluation, the Mazur score with mean follow-ups at 12 months was utilized.Results: The final range of ankle motion was 15 dorsal and 10 plantar flexion. In about 80 cases there was an average Mazur score of83. The mean score was 90; in open fractures 85 to 72. Radiographic healing of fractures in 60 patients occurred in 120 days (mean 105 days), at the time when the external fixator was removed.Conclusion: Pilon fractures are complex and often present complications; the definitive treatment, in emergency or delayed, with hybrid external fixator permits a stable synthesis with minimal soft tissue damage. Weight bearing maybe allowed early and functional recovery is generally good

    Gamma-Ray Burst observations by the high-energy charged particle detector on board the CSES-01 satellite between 2019 and 2021

    In this paper we report the detection of five strong Gamma-Ray Bursts (GRBs) by the High-Energy Particle Detector (HEPD-01) mounted on board the China Seismo-Electromagnetic Satellite (CSES-01), operational since 2018 on a Sun-synchronous polar orbit at a \sim 507 km altitude and 97^\circ inclination. HEPD-01 was designed to detect high-energy electrons in the energy range 3 - 100 MeV, protons in the range 30 - 300 MeV, and light nuclei in the range 30 - 300 MeV/n. Nonetheless, Monte Carlo simulations have shown HEPD-01 is sensitive to gamma-ray photons in the energy range 300 keV - 50 MeV, even if with a moderate effective area above \sim 5 MeV. A dedicated time correlation analysis between GRBs reported in literature and signals from a set of HEPD-01 trigger configuration masks has confirmed the anticipated detector sensitivity to high-energy photons. A comparison between the simultaneous time profiles of HEPD-01 electron fluxes and photons from GRB190114C, GRB190305A, GRB190928A, GRB200826B and GRB211211A has shown a remarkable similarity, in spite of the different energy ranges. The high-energy response, with peak sensitivity at about 2 MeV, and moderate effective area of the detector in the actual flight configuration explain why these five GRBs, characterised by a fluence above \sim 3 ×\times 105^{-5} erg cm2^{-2} in the energy interval 300 keV - 50 MeV, have been detected.Comment: Accepted for publication in The Astrophysical Journal (ApJ

    El trabajo de grado evalúa y propone un ejemplo para modernizar la arquitectura de una de las manzanas del barrio Quinta Paredes. Esta propuesta aborda la necesidad de preservar la arquitectura existente, al mismo tiempo que se busca aumentar la disponibilidad de viviendas, manteniendo la alta calidad que caracteriza a estas edificaciones. La estrategia consiste en realizar modificaciones en el interior de las casas a través de nuevas distribuciones espaciales y la actualización de las redes. Asimismo, se propone intervenir los antejardines, integrándolos para crear un espacio público-privado que fomente la vida en comunidad y que sea gestionado de manera especial para satisfacer las necesidades de los residentes. En relación con las fachadas, se persigue la conservación estética del conjunto mediante ajustes en los materiales, reparaciones o modificaciones que resalten el lenguaje arquitectónico presente en las edificaciones.The thesis appreciates and illustrates an approach to modernize the architecture within one of the blocks in the Quinta Paredes neighborhood. This initiative addresses the imperative to both conserve the existing architectural character and augment the housing capacity, all while preserving the inherent quality of these structures. This is achieved through alterations to the interior layouts, introducing new spatial arrangements, and upgrading networks. Additionally, the front gardens are to be reimagined, integrated to establish a public-private space conducive to community life. The houses requiring special management are adapted to fulfill the unique needs of the block's residents, transforming into facilities that enhance the overall quality of life in the neighborhood.     El presente estudio tuvo como objetivo describir la experiencia de salir del hogar de jóvenes universitarios y su regreso forzoso a causa de la pandemia del Covid 19. Se implementó un método de tipo cualitativo con estudio de caso múltiple, en cuatro jóvenes con edades entre los 22 y 25 años provenientes de la ciudad de Villavicencio que residen en Bogotá debido a sus estudios universitarios. La elección de los participantes se realizó de manera no probabilística y por cuotas, ya que se buscó intencionalmente seleccionar dos personas de ambos sexos. Las entrevistas evidenciaron que, tanto la salida del hogar de los jóvenes, como su regreso forzoso, constituyen transiciones en las cuales los jóvenes atraviesan un momento inicial de desequilibrio que conlleva a un ajuste conductual, cognitivo y emocional y a un crecimiento personal en el proceso de desarrollo de los universitarios. Adicionalmente, a pesar de los momentos difíciles y los desacuerdos que en un principio los jóvenes pudieron mantener con sus padres al regresar al hogar, luego de cierto lapso de tiempo, existe una adaptación en la reciente convivencia en condiciones de confinamiento obligatorio.Estudiantes universitariosThe present study aimed to describe the experience of young university students of leaving home and their forced return due to the Covid 19 pandemic. A qualitative method with a multiple case study was implemented in four young people aged between 22 and 25 years from the city of Villavicencio who reside in Bogota due to their university studies. The choice of participants was carried out in a non-probabilistic quota sampling, since it was intentionally sought to select two people of both sexes. The interviews showed that leaving home and the forced return of young people are transitions which go through an initial moment of imbalance that lead to a behavioral, cognitive and emotional adjustment and personal growth in the development process of the university students. Additionally, despite the difficult moments and the disagreements that the young people maintained with their parents at first, it is possible to observe that, after a certain period of time, there is an adaptation in the recent coexistence at home.Psicólogo (a)Pregrad

    Los objetivos a nivel mundial de disminución de la temperatura global a 2°C o, inclusive a 1,5° en comparación con la temperatura registrada en la época pre-industrial fijados a través del Acuerdo de París, llevaron a Colombia a comprometerse a reducir en un 51% las emisiones de Gases Efecto Invernadero para el año 2030 a través de las medidas de Contribución Nacionalmente Determinada, dentro de las que se encuentran los proyectos de movilidad sostenible en ciudades y que corresponden a medidas con metas claras y cuantitativas de implementación. El presente artículo busca, en consecuencia, analizar la efectividad de las políticas públicas de movilidad sostenible adoptadas en los diferentes Planes de Gobierno de la ciudad de Bogotá en los últimos 5 años y su aporte para la reducción de las emisiones de Gases Efecto Invernadero en cumplimiento de las metas a las cuales se obligó Colombia en virtud del Acuerdo de París. El anterior análisis se realizará a través del uso de la metodología mixta que integra el enfoque cualitativo y cuantitativo.The worldwide objectives of reducing global temperature to 2°C or even 1.5° compared to the temperature recorded in the pre-industrial era, set through the Paris Agreement, led Colombia to commit to a 51% reduction of Greenhouse Gas emissions by 2030 through the Nationally Determined Contribution measures, among which are sustainable mobility projects in cities and which correspond to measures with clear and quantitative implementation goals. This article seeks to analyze the effectiveness of public policies on sustainable mobility adopted in the different Government Plans of the city of Bogota in the last 5 years and their contribution to the reduction of Greenhouse Gas emissions in compliance with the goals to which Colombia is obliged under the Paris Agreement.     Continuous Femoral Catheter for Postoperative Analgesia After Total Knee Arthroplasty

    Introduction: Postoperative pain management in the total knee replacement (TKR) represent a fundamental step for a positive outcome, allowing rapid mobilization, already on the first day. Further, continuous peripheral nerve block techniques have been reported to allow effective and safe control of acute postoperative pain, ensuring the implementation and completion of an accurate and intensive joint rehabilitation program. Aim: The aim of this study was to assess early mobility and compliance of patients that underwent TKR surgery using the femoral block. Methods: For the study, all patients that underwent TKR from 2015 to 2018 with ASA score between II-III was evaluated. Patients underwent vital parameters monitoring and were treated initially with midazolam (0.05mg / kg) e.v. combined sciatic block + femoral perineural catheter positioning using a peripheral nerve stimulation-assisted technique. Results: Intraoperatively, satisfactory analgesia was guaranteed in all patients, associated with a complete muscle relaxation of the affected limb. High patient compliance, associated with good control of acute postoperative pain was obtained in the first 24 hours after surgery (VAS 0- 1). The ROM outcomes were good in all rehabilitation stages, managing to reach 90 degrees bending on the seventh day with an average KSS score of 88-90. Patient satisfaction and impact on quality of life were assessed with the SF-36 showing average scores of 78. Conclusion: Continuous femoral nerve blocking ensures good postoperative analgesia in TKR allowing an early joint mobilization, a rapid functional recovery of the knee and increasing patient compliance during the post-operative rehabilitation program

    6 páginasBackground. Nowadays the importance of lifestyles in the prevention of type 2 diabetes and the metabolic syndrome has been largely accertained. Objective. The purpose of our work is to implement programs that promote a nutritional culture in adolescents and young adults of the La Sabana University. Methods. The methodology entailed, after the corresponding informed consent, taking measures of the triceps and supraescapular skinfolds, waist circumference, body mass index (BMI), lean mass, and fat mass. Fasting blood samples were also taken to quantify cholesterol, triglycerides, high density lipoprotein (HDL) and low density lipoprotein (LDL). Results. The results obtained show that of the 165 students, 10.3% were underweight, 13.93% were overweight and 0.6% were obese. With regards to gender, 4.8% of the men and 9% of the women were overweight, 3% of the men and 7.2% of the women were underweight, and 0.6% of the women were obese. The blood chemistry showed that 30% had hypercholesterolemia, 18% hypertriglyceridemia, 17% reported low HDL levels and 67% reported high LDL levels. Of all the cases studied, 40% are at risk of a metabolic syndrome. 60% claimed not to practice any physical activity - especially women who reported 44.70%. Conclusions. These findings have allowed us at the institution to implement a culture of healthy habits. The have also allowed us to identify students with risk factors for type 2 diabetes and metabolic syndrome. This is why the cardiometabolic monitoring and control based on healthy eating and physical activity are important.Fondo. En la actualidad se ha evidenciado la importancia de los estilos de vida en la prevención de la diabetes tipo 2 y el síndrome metabólico. sido en gran parte acertada. Objetivo. El propósito de nuestro trabajo es implementar programas que promuevan una cultura nutricional en adolescentes y jóvenes. adultos de la Universidad de La Sabana. Métodos. La metodología consistió, previo consentimiento informado correspondiente, en la toma de medidas del tríceps y pliegues cutáneos supraescapulares, circunferencia de la cintura, índice de masa corporal (IMC), masa magra y masa grasa. Muestras de sangre en ayunas también se tomaron para cuantificar colesterol, triglicéridos, lipoproteínas de alta densidad (HDL) y lipoproteínas de baja densidad (LDL). Resultados. Los resultados obtenidos muestran que de los 165 estudiantes, el 10,3% presentaba bajo peso, el 13,93% sobrepeso y 0,6% eran obesos. En cuanto al género, el 4,8% de los hombres y el 9% de las mujeres tenían sobrepeso, el 3% de los hombres y El 7,2% de las mujeres tenían bajo peso y el 0,6% de las mujeres eran obesas. La química sanguínea mostró que el 30% tenía hipercolesterolemia, 18% hipertrigliceridemia, 17% reportaron niveles bajos de HDL y 67% reportaron niveles altos de LDL. De todo de los casos estudiados, el 40% tiene riesgo de síndrome metabólico. El 60% afirmó no practicar ninguna actividad física, especialmente mujeres que reportaron el 44.70%. Conclusiones. Estos hallazgos nos han permitido en la institución implementar una cultura de hábitos saludables. El tener también permitió identificar a los estudiantes con factores de riesgo para diabetes tipo 2 y síndrome metabólico. Por eso es importante el seguimiento y control cardiometabólico basado en la alimentación saludable y la actividad física
