Cigarette-Smoke-Induced Dysregulation of MicroRNA Expression and Its Role in Lung Carcinogenesis
Dysregulation of microRNAs (miRNAs), particularly their downregulation, has been widely shown to be associated with the development of lung cancer. Downregulation of miRNAs leads to the overactivation of their oncogene targets, while upregulation of some miRNAs leads to inhibition of important tumor suppressors. Research has implicated cigarette smoke in miRNA dysregulation, leading to carcinogenesis. Cigarette smoke may lead to genetic or epigenetic damage to miRNAs, many of which map to fragile sites and some of which contain single nucleotide polymorphisms. Cigarette smoke may also cause dysregulation by affecting regulatory mechanisms controlling miRNA expression. Researchers have shown a correlation between smoke-exposure-induced dysregulation of miRNAs and age. Furthermore, dysregulation seems to be associated with intensity and duration of smoke exposure and duration of cessation. Longer exposure at a threshold level is needed for irreversibility of changes in expression. Better understanding of miRNA dysregulation may allow for improved biomonitoring and treatment regimens for lung cancer

Association of chronic central serous chorioretinopathy with subclinical Cushing\u27s syndrome.
Purpose: To report the clinical course of a patient with central serous chorioretinopathy (CSCR) secondary to subclinical hypercortisolism before and after adrenalectomy.
Observations: A 50-year-old female patient with multifocal, chronic CSCR was found to have an adrenal incidentaloma and was diagnosed with subclinical hypercortisolism. Patient elected to undergo minimally-invasive adrenalectomy and presented at 3 months after surgery without subretinal fluid.
Conclusions and importance: Subclinical Cushing\u27s Syndrome (SCS) may present an underrecognized risk factor for developing chronic CSCR. Further investigation is needed to determine the threshold of visual comorbidity that may influence surgical management
Pretransplant HLA typing revealed loss of heterozygosity in the major histocompatibility complex in a patient with acute myeloid leukemia
Introduction
Chromosomal abnormalities are frequent events in hematological malignancies. The degree of HLA compatibility between donor and recipient in hematopoietic stem cell transplantation is critical.
Purpose of the study
In this report, we describe an acute myeloid leukemia case with loss of heterozygosity (LOH) encompassing the entire HLA.
Materials and methods
HLA molecular typing was performed on peripheral blood (PB) and buccal swabs (BS). Chromosomal microarray analysis (CMA) was performed using a whole genome platform.
Results
Typing results on PB sample collected during blast crisis demonstrated homozygosity at the -A, -B, -C, -DR, and -DQ loci. A BS sample demonstrated heterozygosity at all loci. A subsequent PB sample drawn after count recovery confirmed heterozygosity. The CMA performed on PB samples collected during and after blast crisis revealed a large terminal region of copy-neutral LOH involving chromosome region 6p25.3p21.31, spanning approximately 35.9 Mb. The results of the CMA assay on sample collected after count recovery did not demonstrate LOH.
Conclusions
LOH at the HLA gene locus may significantly influence the donor search resulting in mistakenly choosing homozygous donors. We recommend confirming the HLA typing of recipients with hematological malignancies when homozygosity is detected at any locus by using BS samples, or alternatively from PB when remission is achieved
Recreational Fishing Impacts in an Offshore and Deep-Water Marine Park: Examining Patterns in Fished Species Using Hybrid Frequentist Model Selection and Bayesian Inference
No-take marine reserves are often located in remote locations far away from human activity, limiting perceived impact on extractive users but also reducing their use for investigating impacts of fishing. This study aimed to establish a benchmark in the distribution of fished species across the Ningaloo Marine Park – Commonwealth (NMP-Commonwealth), and adjacent comparable habitats within the Ningaloo Marine Park - State (NMP-State), in Western Australia to test if there was evidence of an effect of recreational fishing, as no commercial fishing is allowed within either marine park. We also examined whether the remote location of the newly established (2018) No-take Zone (NTZ), in NMP-Commonwealth, limits its use for studying the effects of fishing. Throughout the NMP-Commonwealth and NMP-State, where recreational fishing is permitted, we expected the abundance of recreationally fished fish species to increase with increasing distance to the nearest boat ramp, as a proxy of recreational fishing effort. Conversely, we did not expect the abundance of non-fished species and overall species richness to vary in response to the proxy for human activity. Distance to the nearest boat ramp was found to be a strong predictor of fished species abundance, indicating that the effect of recreational fishing can be detected across the NMP-Commonwealth. The effect of the NTZ on fished species abundance was weakly positive, but this difference across the NTZ is expected to increase over time. Habitat composition predictors were only found to influence species richness and non-fished species abundance. This study suggests a clear footprint of recreational fishing across the NMP-Commonwealth and as a result the new NTZ, despite its remote location, can act as a control in future studies of recreational fishing effects.publishedVersio
The Building Blocks of Interoperability. A Multisite Analysis of Patient Demographic Attributes Available for Matching.
BackgroundPatient matching is a key barrier to achieving interoperability. Patient demographic elements must be consistently collected over time and region to be valuable elements for patient matching.ObjectivesWe sought to determine what patient demographic attributes are collected at multiple institutions in the United States and see how their availability changes over time and across clinical sites.MethodsWe compiled a list of 36 demographic elements that stakeholders previously identified as essential patient demographic attributes that should be collected for the purpose of linking patient records. We studied a convenience sample of 9 health care systems from geographically distinct sites around the country. We identified changes in the availability of individual patient demographic attributes over time and across clinical sites.ResultsSeveral attributes were consistently available over the study period (2005-2014) including last name (99.96%), first name (99.95%), date of birth (98.82%), gender/sex (99.73%), postal code (94.71%), and full street address (94.65%). Other attributes changed significantly from 2005-2014: Social security number (SSN) availability declined from 83.3% to 50.44% (p<0.0001). Email address availability increased from 8.94% up to 54% availability (p<0.0001). Work phone number increased from 20.61% to 52.33% (p<0.0001).ConclusionsOverall, first name, last name, date of birth, gender/sex and address were widely collected across institutional sites and over time. Availability of emerging attributes such as email and phone numbers are increasing while SSN use is declining. Understanding the relative availability of patient attributes can inform strategies for optimal matching in healthcare
Lessons for COVID-19 vaccination from eight federal government direct communication evaluations
We discuss eight randomized evaluations intended to increase vaccination uptake conducted by the US General Services Administration’s Office of Evaluation Sciences (OES). These evaluations had a median sample size of 55,000, deployed a variety of behaviorally-informed direct communications, and used administrative data to measure vaccination uptake. The confidence interval from an internal meta-analysis shows changes in vaccination rates ranging from -0.004 to 0.394 percentage points. Two studies yielded statistically significant increases, of 0.59 and 0.16 percentage points. The other six were not statistically significant, although the studies were powered to detect effect sizes in line with published research. This work highlights the likely effects of government communications and demonstrates the value of conducting rapid evaluations to support COVID-19 vaccination efforts
Aurora: A Software Radio for Electromagnetic Vector Sensors in Space
The AERO (Auroral Emission Radio Observer) and VISTA (Vector Interferometry Space Technology using AERO) missions will advance auroral radio science and radio interferometry technology. AERO is intended to qualify and validate electromagnetic vector sensor technology in space while also answering key scientific questions about the nature and sources of auroral radio emissions. These questions cannot be addressed from the ground due to shielding by the ionosphere. VISTA, together with AERO, will provide the first demonstration of interferometric imaging, beamforming, and nulling using electromagnetic vector sensors at low frequencies (100 kHz – 15 MHz) using Space based sensors.
A key component of the AERO-VISTA joint mission is the Aurora software radio system which forms the primary mission payload when combined with an electromagnetic vector sensor antenna (VSA). This radio combines the analog, digital, and signal processing necessary to detect and digitize the signals associated with the radio aurora. We provide a detailed discussion of the radio design, implementation, and performance results from early testing of our engineering model units
