    Calculation of the interventilatory threshold area: a method for examining the aerobic-anaerobic transition

    El objetivo fue determinar la relación entre el área interumbrales (ITA) [la zona comprendida entre el primer y el segundo umbral ventilatorio (VT1 y VT2) en la función VO2/VE, Carga/VO2 y Carga/VE] y las variables ergoespirométricas. Treinta y tres hombres realizaron un test incremental. El ITA se calculó: 1) como la integral definida por el área entre VT1 y VT2 bajo las curvas de VO2/VE, Carga/VO2 y Carga/VE y 2) como la suma de las áreas descritas por el triángulo y rectángulo entre los mismos puntos. El ITA para la función Carga/VE se correlacionó positivamente (p<0,01) con la carga en VT2 (r = 0,831) y la ventilación en VT2 (r = 0,799). El ITA para la función VO2/VE fue significativamente mayor en los ciclistas que en los estudiantes. La determinación del ITA es un método simple para evaluar la transición aeróbica-anaeróbica durante las pruebas de esfuerzo incremental.The aim was to determine the relationship between the interthreshold area (ITA) [the area between the first and second ventilatory threshold (VT1 and VT2) for the function VO2/VE, load/VO2 and load/VE] and the traditional variables measured. Thirty-three men underwent an incremental test. The ITA was calculated: 1) as the integral defined by the area between VT1 and VT2 under the curves for the functions VO2/VE, load/VO2 and load/VE and 2) as the simple sum of the areas described by the triangle and rectangle between the same points. The mean ITA for the function load/VE was positively correlated (p<0.01) with load at VT2 (r=0.831) and ventilation at VT2 (r=0.799). The mean ITA for the function VO2/VE was significantly greater in the cyclists than in the students. The ITA for the function load/VE differed between March and July as training progressed.     Circulating neutrophil counts and mortality in septic shock

    Producción CientíficaPolynuclear neutrophils can play dual roles in sepsis: on the one hand they mediate major antimicrobial activities and on the other hand they can contribute to the development of multiple organ failure [1]. Nonetheless, in spite of the importance of these cells in sepsis, the influence of the circulating neutrophil count (CNC) on the prognosis of septic patients with this pathology has not been properly evaluated. We analyzed the association between CNC and outcome in two cohorts of patients with diagnostic criteria of septic shock (SS) [2]: the first was recruited in the context of a single center study (EXPRESS study, discovery cohort, n = 195; Table 1), and the second in the context of a multi-centric study (GRECIA study, validation cohort, n = 194; Table 2). Written informed consent was obtained from each patient or their legal representative.     Neste artigo, resultado de uma pesquisa de tipo qualitativo, hermenêutico e documental, procuramos mostrar como as Ciências Cognitivas, desde a sua constituição, têm contribuído para a compreensão do ser humano. Fazemos referência às visões mais racionalistas da mente, que a entendem semelhante a um sistema computacional, até à visão menos (não) racionalista que lhe confere valor cognitivo, envolta em emoções e afetos, que numa visão mais construtivista – ou pós-racionalista – se diria que traduz significados, traduz uma vivência ou uma história narrativa (e) pessoal. Assim, apresentamos o modelo de terapia cognitiva pós-racionalista de Vittorio Guidano, refletindo sobre os fundamentos epistemológicos do Movimento da Auto-organização (MAO), nos quais ele se fundamenta. Descrevemos sumariamente os alicerces epistemológicos do MAO, os quais garantem a este modelo uma dimensão holista e explicativa do processo de construção da identidade humana, que pode ser descrita como processo de conhecimento, capaz de vivenciar e ao mesmo tempo perceber e avaliar a sua própria experiência (autoconsciência). O ser humano age na intersubjetividade, num mundo pluralista, com os seus congéneres, partilhando experiências e interpretando ações (complexificando-se através desses acoplamentos). Pretendemos, também, ilustrar que a matriz de funcionamento interno de cada ser humano, a qual lhe permite ordenar essa múltipla e facetada realidade, assim como formar/construir/ordenar significados pessoais, é o vínculo emotivo-afetivo.In this paper, which is the result of a qualitative, hermeneutical and documental research, we intend to show how the Cognitive Sciences, since their beginning, have contributed to the understanding of human beings. We refer to the rationalistic understanding of the mind (the mind is like a computer system), and also to the non-rationalistic understanding, which attributes to the mind a cognitive value, immersed in emotions and affections. In addition, according to a constructivist or post-rationalistic understanding, the mind translates meanings, an experience or a personal narrative story. Thus, we present the model of post-rationalistic cognitive therapy of Vittorio Guidano, reflecting on the epistemological foundations of the Self-Organization Movement, on which it is based. We describe briefly the epistemological principles of the Self-Organization Movement, which give to this model a holistic and explanatory dimension of the process of construction of human identity. This identity can be described as the human beings’ process of self-knowledge, as we are capable of experiencing and at the same time perceiving and evaluating, our own experiences (self-consciousness). The human being acts on intersubjectivity, in a pluralistic world with his peers, sharing experiences and interpreting actions. We also want to illustrate that the matrix of inner functioning of every human being, which allows us to sort this multi-faceted reality in a single personal order, is the emotional bond.(undefined

    SIRT1 mediates obesity- and nutrient-dependent perturbation of pubertal timing by epigenetically controlling Kiss1 expression

    Puberty is regulated by epigenetic mechanisms and is highly sensitive to metabolic and nutritional cues. However, the epigenetic pathways mediating the effects of nutrition and obesity on pubertal timing are unknown. Here, we identify Sirtuin 1 (SIRT1), a fuel-sensing deacetylase, as a molecule that restrains female puberty via epigenetic repression of the puberty-activating gene, Kiss1. SIRT1 is expressed in hypothalamic Kiss1 neurons and suppresses Kiss1 expression. SIRT1 interacts with the Polycomb silencing complex to decrease Kiss1 promoter activity. As puberty approaches, SIRT1 is evicted from the Kiss1 promoter facilitating a repressive-to-permissive switch in chromatin landscape. Early-onset overnutrition accelerates these changes, enhances Kiss1 expression and advances puberty. In contrast, undernutrition raises SIRT1 levels, protracts Kiss1 repression and delays puberty. This delay is mimicked by central pharmacological activation of SIRT1 or SIRT1 overexpression, achieved via transgenesis or virogenetic targeting to the ARC. Our results identify SIRT1-mediated inhibition of Kiss1 as key epigenetic mechanism by which nutritional cues and obesity influence mammalian puberty

    Spontaneous reperfusion enhances succinate concentration in peripheral blood from stemi patients but its levels does not correlate with myocardial infarct size or area at risk

Succinate levels might have a prognostic value, as they may correlate with edema volume or myocardial infarct size. However, blood from the coronary sinus is not routinely obtained in the CathLab. As succinate might be also increased in peripheral blood, we aimed to investigate whether peripheral plasma concentrations of succinate and other metabolites obtained during coronary revascularization correlate with edema volume or infarct size in STEMI patients. Plasma samples were obtained from peripheral blood within the first 10 min of revascularization in 102 STEMI patients included in the COMBAT-MI trial (initial TIMI 1) and from 9 additional patients with restituted coronary blood flow (TIMI 2). Metabolite concentrations were analyzed by 1H-NMR. Succinate concentration averaged 0.069 ± 0.0073 mmol/L in patients with TIMI flow ≤ 1 and was significantly increased in those with TIMI 2 at admission (0.141 ± 0.058 mmol/L, p < 0.05). However, regression analysis did not detect any significant correlation between most metabolite concentrations and infarct size, extent of edema or other cardiac magnetic resonance (CMR) variables. In conclusion, spontaneous reperfusion in TIMI 2 patients associates with enhanced succinate levels in peripheral blood, suggesting that succinate release increases overtime following reperfusion. However, early plasma levels of succinate and other metabolites obtained from peripheral blood does not correlate with the degree of irreversible injury or area at risk in STEMI patients, and cannot be considered as predictors of CMR variables.     Effect of COMBinAtion therapy with remote ischemic conditioning and exenatide on the Myocardial Infarct size: a two-by-two factorial randomized trial (COMBAT-MI)

    Remote ischemic conditioning (RIC) and the GLP-1 analog exenatide activate different cardioprotective pathways and may have additive effects on infarct size (IS). Here, we aimed to assess the efficacy of RIC as compared with sham procedure, and of exenatide, as compared with placebo, and the interaction between both, to reduce IS in humans. We designed a two-by-two factorial, randomized controlled, blinded, multicenter, clinical trial. Patients with ST-segment elevation myocardial infarction receiving primary percutaneous coronary intervention (PPCI) within 6 h of symptoms were randomized to RIC or sham procedure and exenatide or matching placebo. The primary outcome was IS measured by late gadolinium enhancement in cardiac magnetic resonance performed 3–7 days after PPCI. The secondary outcomes were myocardial salvage index, transmurality index, left ventricular ejection fraction and relative microvascular obstruction volume. A total of 378 patients were randomly allocated, and after applying exclusion criteria, 222 patients were available for analysis. There were no significant interactions between the two randomization factors on the primary or secondary outcomes. IS was similar between groups for the RIC (24 ± 11.8% in the RIC group vs 23.7 ± 10.9% in the sham group, P = 0.827) and the exenatide hypotheses (25.1 ± 11.5% in the exenatide group vs 22.5 ± 10.9% in the placebo group, P = 0.092). There were no effects with either RIC or exenatide on the secondary outcomes. Unexpected adverse events or side effects of RIC and exenatide were not observed. In conclusion, neither RIC nor exenatide, or its combination, were able to reduce IS in STEMI patients when administered as an adjunct to PPCI

    Measurement of the cross-section and charge asymmetry of WW bosons produced in proton-proton collisions at s=8\sqrt{s}=8 TeV with the ATLAS detector

    This paper presents measurements of the W+μ+νW^+ \rightarrow \mu^+\nu and WμνW^- \rightarrow \mu^-\nu cross-sections and the associated charge asymmetry as a function of the absolute pseudorapidity of the decay muon. The data were collected in proton--proton collisions at a centre-of-mass energy of 8 TeV with the ATLAS experiment at the LHC and correspond to a total integrated luminosity of 20.2~\mbox{fb^{-1}}. The precision of the cross-section measurements varies between 0.8% to 1.5% as a function of the pseudorapidity, excluding the 1.9% uncertainty on the integrated luminosity. The charge asymmetry is measured with an uncertainty between 0.002 and 0.003. The results are compared with predictions based on next-to-next-to-leading-order calculations with various parton distribution functions and have the sensitivity to discriminate between them.Comment: 38 pages in total, author list starting page 22, 5 figures, 4 tables, submitted to EPJC.     Search for chargino-neutralino production with mass splittings near the electroweak scale in three-lepton final states in √s=13 TeV pp collisions with the ATLAS detector

    A search for supersymmetry through the pair production of electroweakinos with mass splittings near the electroweak scale and decaying via on-shell W and Z bosons is presented for a three-lepton final state. The analyzed proton-proton collision data taken at a center-of-mass energy of √s=13  TeV were collected between 2015 and 2018 by the ATLAS experiment at the Large Hadron Collider, corresponding to an integrated luminosity of 139  fb−1. A search, emulating the recursive jigsaw reconstruction technique with easily reproducible laboratory-frame variables, is performed. The two excesses observed in the 2015–2016 data recursive jigsaw analysis in the low-mass three-lepton phase space are reproduced. Results with the full data set are in agreement with the Standard Model expectations. They are interpreted to set exclusion limits at the 95% confidence level on simplified models of chargino-neutralino pair production for masses up to 345 GeV

    Search for new phenomena in final states with an energetic jet and large missing transverse momentum in pp collisions at √ s = 8 TeV with the ATLAS detector

    Results of a search for new phenomena in final states with an energetic jet and large missing transverse momentum are reported. The search uses 20.3 fb−1 of √ s = 8 TeV data collected in 2012 with the ATLAS detector at the LHC. Events are required to have at least one jet with pT > 120 GeV and no leptons. Nine signal regions are considered with increasing missing transverse momentum requirements between Emiss T > 150 GeV and Emiss T > 700 GeV. Good agreement is observed between the number of events in data and Standard Model expectations. The results are translated into exclusion limits on models with either large extra spatial dimensions, pair production of weakly interacting dark matter candidates, or production of very light gravitinos in a gauge-mediated supersymmetric model.
