    Methylprednisolone as Adjunct to Endovascular Thrombectomy for Large-Vessel Occlusion Stroke

    Importance It is uncertain whether intravenous methylprednisolone improves outcomes for patients with acute ischemic stroke due to large-vessel occlusion (LVO) undergoing endovascular thrombectomy. Objective To assess the efficacy and adverse events of adjunctive intravenous low-dose methylprednisolone to endovascular thrombectomy for acute ischemic stroke secondary to LVO. Design, Setting, and Participants This investigator-initiated, randomized, double-blind, placebo-controlled trial was implemented at 82 hospitals in China, enrolling 1680 patients with stroke and proximal intracranial LVO presenting within 24 hours of time last known to be well. Recruitment took place between February 9, 2022, and June 30, 2023, with a final follow-up on September 30, 2023.InterventionsEligible patients were randomly assigned to intravenous methylprednisolone (n = 839) at 2 mg/kg/d or placebo (n = 841) for 3 days adjunctive to endovascular thrombectomy. Main Outcomes and Measures The primary efficacy outcome was disability level at 90 days as measured by the overall distribution of the modified Rankin Scale scores (range, 0 [no symptoms] to 6 [death]). The primary safety outcomes included mortality at 90 days and the incidence of symptomatic intracranial hemorrhage within 48 hours. Results Among 1680 patients randomized (median age, 69 years; 727 female [43.3%]), 1673 (99.6%) completed the trial. The median 90-day modified Rankin Scale score was 3 (IQR, 1-5) in the methylprednisolone group vs 3 (IQR, 1-6) in the placebo group (adjusted generalized odds ratio for a lower level of disability, 1.10 [95% CI, 0.96-1.25]; P = .17). In the methylprednisolone group, there was a lower mortality rate (23.2% vs 28.5%; adjusted risk ratio, 0.84 [95% CI, 0.71-0.98]; P = .03) and a lower rate of symptomatic intracranial hemorrhage (8.6% vs 11.7%; adjusted risk ratio, 0.74 [95% CI, 0.55-0.99]; P = .04) compared with placebo. Conclusions and Relevance Among patients with acute ischemic stroke due to LVO undergoing endovascular thrombectomy, adjunctive methylprednisolone added to endovascular thrombectomy did not significantly improve the degree of overall disability.Trial RegistrationChiCTR.org.cn Identifier: ChiCTR210005172

    Integration of GIS and a Lagrangian Particle-Tracking Model for Harmful Algal Bloom Trajectories Prediction

    Harmful algal bloom (HAB) is a major environmental problem in coastal waters around the world. The technologies and approaches for short-term forecasting of the HABs trajectories have obtained increasing attention from researchers. In this paper, we present a straightforward physical-based model based on a non-Fickian Lagrangian particle-tracking scheme for understanding the movement of detected HABs. The model adopts the fractional Brownian motion (fBm) technology, and is coupled with the Delft3D and WRF models and GIS. The fBm based Lagrangian particle-tracking model can flexibly control the scale of the particle clouds diffusion through Hurst value, which can be used to account for uncertainties and adjust for better representing the trajectories of HABs. Simulation results demonstrate that the presented model can successfully predict the trends and the main features of red tide drifting. The developed simulation tool enables users to create the model configuration, manage data inputs, run the model, and generate model maps and animations within a GIS environment. It is believed that the model and the tool outlined herein can be very useful for rapidly evaluating potential areas at risk from the HABs events

    A Data Quality Control Method for Seafloor Observatories: The Application of Observed Time Series Data in the East China Sea

    With the construction and deployment of seafloor observatories around the world, massive amounts of oceanographic measurement data were gathered and transmitted to data centers. The increase in the amount of observed data not only provides support for marine scientific research but also raises the requirements for data quality control, as scientists must ensure that their research outcomes come from high-quality data. In this paper, we first analyzed and defined data quality problems occurring in the East China Sea Seafloor Observatory System (ECSSOS). We then proposed a method to detect and repair the data quality problems of seafloor observatories. Incorporating data statistics and expert knowledge from domain specialists, the proposed method consists of three parts: a general pretest to preprocess data and provide a router for further processing, data outlier detection methods to label suspect data points, and a data interpolation method to fill up missing and suspect data. The autoregressive integrated moving average (ARIMA) model was improved and applied to seafloor observatory data quality control by using a sliding window and cleaning the input modeling data. Furthermore, a quality control flag system was also proposed and applied to describe data quality control results and processing procedure information. The real observed data in ECSSOS were used to implement and test the proposed method. The results demonstrated that the proposed method performed effectively at detecting and repairing data quality problems for seafloor observatory data

    Variation of Suspended Particles in the Bottom Layer of the East China Sea with Data from Seafloor Observatory

    The in situ scattering and transmissometry laser (LISST-100X), equipped with an acoustic wave and current (AWAC) meter and conductivity, temperature, and depth (CTD) instruments over the seabed in the East China Sea, was used to monitor the variation in suspended particles in the bottom sea layer, including particle size distribution (PSD) and volume concentration. The power law approximation was tested to describe the variability in PSD based on the field data. The results show that the power law was robust in processing continuous data, accompanied with the same optimal reference particle size (~63 μm) and little change in the corresponding exponent (~3.4) in both periods. Suspended particles were divided into three types: macroflocs (>133 μm), microflocs (36–133 μm), and single grains (<36 μm). Particle sizes were coarse during the two seasons, with macroflocs representing more than 60% of all the suspended particles, especially in February, when the particle size spectra were usually open-ended. Results from the harmonic analysis method indicate that tidal-induced resuspension and advection are the major reasons for the diurnal dynamics of sediments. Due to the tidal asymmetry in the region, we only found one mode in volume concentration at the moment of maximum velocity. However, the ratios of macroflocs were bimodal, with maximum floods and ebbs in one tidal cycle in February, when the higher mode at the maximum ebbs may be contributed to by the flocculation of finer particles considering the decreasing ratios of finer particles. Due to the enhanced stratification and the clean barrier built up by the Taiwan Warm Current in the southeast corner, the significant daily variation in suspended particles observed in February weakened in September. The influence of waves was uncertain, although the correlation coefficient between significant wave height and volume concentration was about 64% in February

    Numerical Modeling of a Green Tide Migration Process with Multiple Artificial Structures in the Western Bohai Sea, China

    Green tides have increasingly become a nuisance worldwide in recent years, and especially in China. Since 2015, green tides have started recurring in Jinmeng Bay, Qinhuangdao, western Bohai Sea of China, and have severely deteriorated the tourism environment there. In order to investigate the migration process of the green tides in Jinmeng Bay, a hydrodynamic model and a particle-tracking model were applied based on the latest green tide event in August 2021. The hydrodynamic model was applied with triple-level 2DH meshes with different refinements and scales, which provided the hydrodynamics to drive the green macroalgae into the particle-tracking model. From the model results, the semi-enclosed waters surrounded by multiple artificial structures are a low-energy hydrodynamic environment, which is not helpful for water exchange and thus the dispersal of nutrients. The green macroalgae are distributed substantially within the semi-enclosed waters, and few are transported out with low biomass. The effects of wind and artificial structures both increase the coverage of the green macroalgae trajectories; the effect of wind plays a more important role. A sensitivity analysis of the effect of wind showed that 6 m/s wind in ENE led to the maximum coverage of the green macroalgae trajectories in the cases of different magnitudes and directions of winds. This study can provide references for the pre-warning and mitigation of green tides in Jinmeng Bay and other similar places

    Implementation of an Attribute-Based Encryption Scheme Based on SM9

    In recent years, attribute-based encryption (ABE) has been widely applied in mobile computing, cloud computing, and the Internet of things, for supporting flexible and fine-grained access control of sensitive data. In this paper, we present a novel attribute-based encryption scheme that is based on bilinear pairing over Barreto and Naehrig curves (BN-curves). The identity-based encryption scheme SM9, which is a Chinese commercial cryptographic standard and a forthcoming part of ISO/IEC11770-3, has been used as the fundamental building block, and thus we first introduce SM9 and present our SM9 implementation in details. Subsequently, we propose the design and implementation of the ABE scheme. Moreover, we also develop a hybrid ABE for achieving lower ciphertext expansion rate when the size of access structure or plaintext is large. The performance and energy consumption of the implementation of the proposed ABE and its hybrid version are evaluated with a workstation, a PC, a smart phone, and an embedded device. The experimental results indicated that our schemes work well on various computing platforms. Moreover, the proposed schemes and their implementations would benefit developers in building applications that fulfill the regulatory compliance with the Chinese commercial cryptographic standard since there is no existing ABE scheme compatible with any Chinese cryptographic standard
