Posttreatment Surveillance in Patients with Prolonged Disease-Free Survival After Resection of Colorectal Liver Metastasis
Introducing an innovative surgical technique:gluteal turnover flap for posterior vaginal wall reconstruction :a case series
Four patients with rectal cancer required reconstruction of a defect of the posterior vaginal wall. All patients received neoadjuvant (chemo)radiotherapy, followed by an en bloc (abdomino)perineal resection of the rectum and posterior vaginal wall. The extent of the vaginal defect necessitated closure using a tissue flap with skin island. The gluteal turnover flap was used for this purpose as an alternative to conventional more invasive myocutaneous flaps (gracilis, gluteus, or rectus abdominis). The gluteal turnover flap was created through a curved incision at a maximum width of 2.5 cm from the edge of the perineal wound, thereby creating a half-moon shape skin island. The subcutaneous fat was dissected toward the gluteal muscle, and the gluteal fascia was incised. Thereafter, the flap was rotated into the defect and the skin island was sutured into the vaginal wall defect. The contralateral subcutaneous fat was mobilized for perineal closure in the midline, after which no donor site was visible.The duration of surgery varied from 77 to 392 min, and the hospital stay ranged between 3 and 16 days. A perineal wound dehiscence occurred in two patients, requiring an additional VY gluteal plasty in one patient. Complete vaginal and perineal wound healing was achieved in all patients. The gluteal turnover flap is a promising least invasive technique to reconstruct posterior vaginal wall defects after abdominoperineal resection for rectal cancer.</p
Quality of life in patients with a perineal hernia
Introduction: Patients who develop a perineal hernia after abdominoperineal resection may experience discomfort during daily activities and urogenital dysfunction, but the impact on quality of life has never been formally assessed. Materials and methods: Patients who underwent abdominoperineal resection for rectal cancer between 2014 and 2022 in two prospective multicenter trials were included. Primary outcome was defined as median overall scores or scores on functional and symptom scales of the following quality of life questionnaires: 5-level version of the 5-dimensional EuroQol, Short Form-36, and European Organization for Research and Treatment of Cancer QoL Questionnaire Colorectal cancer 29 and 30, Urogenital Distress Inventory-6, Incontinence Impact Questionnaire-7. Results: Questionnaires were available in 27 patients with a perineal hernia and 62 patients without a perineal hernia. The 5-dimensional EuroQol score was significantly lower in patients with a perineal hernia (83 vs 87, p = 0.048), which implies a reduced level of functioning. The median scores of pain-specific domains were significantly worse in patients with a perineal hernia as measured by the SF-36 (78 vs. 90, p = 0.006), the EORTC-CR29 (17 vs. 11, p=<0.001) and EORTC-C30 (17 vs. 0, p = 0.019). Also, significantly worse physical (73 vs. 100, p = 0.049) and emotional (83 vs. 100, p = 0.048) functioning based on EORTC-C30 was observed among those patients. Minimally important differences were found for role, physical and social functioning of the SF-36 and EORTC-C30. The urological function did not differ between the groups. Conclusion: A symptomatic perineal hernia can significantly worsen quality of life on several domains, indicating the severity of this complication.</p
Relation between body composition and severe diarrhea in patients treated with preoperative chemoradiation with capecitabine for rectal cancer:a single-centre cohort study
BACKGROUND: Chemoradiation with capecitabine followed by surgery is standard care for locally advanced rectal cancer (LARC). Severe diarrhea is considered a dose-limiting toxicity of adding capecitabine to radiation therapy. The aim of this study was to describe the risk factors and the impact of body composition on severe diarrhea in patients with LARC during preoperative chemoradiation with capecitabine. METHODS: A single centre retrospective cohort study was conducted in a tertiary referral centre. All patients treated with preoperative chemoradiation with capecitabine for LARC from 2009 to 2015 were included. Patients with locally recurrent rectal cancer who received chemoradiation for the first time were included as well. Logistic regression analyses were performed to identify risk factors for severe diarrhea. RESULTS: A total of 746 patients were included. Median age was 64 years (interquartile range 57–71) and 477 patients (64%) were male. All patients received a radiation dosage of 25 × 2 Gy during a period of five weeks with either concomitant capecitabine administered on radiation days or continuously during radiotherapy. In this cohort 70 patients (9%) developed severe diarrhea. In multivariable logistic regression analyses female sex (OR: 4.42, 95% CI 2.54–7.91) and age ≥ 65 (OR: 3.25, 95% CI 1.85–5.87) were the only risk factors for severe diarrhea. CONCLUSIONS: Female patients and patients aged sixty-five or older had an increased risk of developing severe diarrhea during preoperative chemoradiation therapy with capecitabine. Treatment of Inguinal Lymph Node Metastases in Patients with Rectal Adenocarcinoma
Background
Inguinal lymph node metastases (ILNM) from rectal adenocarcinoma are rare and staged as systemic disease. This study aimed to provide insight into the treatment and prognosis of ILNM from rectal adenocarcinoma.
Methods
All patients with a diagnosis of synchronous or metachronous ILNM from rectal adenocarcinoma between January 2005 and March 2017 were retrospectively reviewed.
Result
Treatment and outcomes of local re-recurrence in rectal cancer:a multicentre observational cohort
Objective: This study aimed to assess patient, tumour and treatment characteristics in locally re-recurrent rectal cancer (LrRRC), and relate these to long-term oncological outcomes since limited literature on this topic is available. Methods: A retrospective study of locally recurrent rectal cancer (LRRC) patients treated with curative intent between 1994 and 2022 in two tertiary centres in the Netherlands. Data on patient, tumour and treatment characteristics were collected. Oncological outcomes of patients treated with curative and palliative intent were displayed. Results: Of 852 curatively treated LRRC patients, 277 (33 %) developed LrRRC. The median survival of this LrRRC group was 23 months (95 % CI 20–26). Treatment with curative intent was offered to 92/277 (33 %) patients, resulting in surgical resection in 81/277 (29 %) patients. Complete oncological clearance (R0) was achieved in 50/81 cases (62 %). Surgical resection resulted in a median survival of 35 months (95 % CI 27–43). Palliative treatment resulted in a median survival of 16 months (95 % CI 11–21). Median progression free survival (PFS) in patients treated with curative intent was 16 months (95 % CI 12–20). Median local re-re-recurrence free survival (LrrRFS) in patients who underwent surgery was 11 months (95 % CI 8–16). Conclusion: The median OS in 277 LrRRC patients was 23 months. Aggressive treatment in selected patients resulted in a median OS of 35 months. Furthermore, locoregional control was achieved for 11 months in patients undergoing surgery. These findings underscore the need for meticulous patient selection and tailored treatment strategies to improve survival while minimising unnecessary morbidity.</p
