Solitary pulmonary nodule — the role of imaging in the diagnostic process
A solitary pulmonary nodule is a round opacity less than 30 mm in diameter surrounded by normally aerated lung
tissue. Determination of the character of the lesion following its detection (particularly when the identification was incidental)
may require a complex diagnostic process. In most cases, nodules are benign in character; however, the probability
of malignancy increases significantly for part-solid lesions. The main features that describe the solitary pulmonary
nodule in computed tomography scans include their size, shape, density, presence of calcification and rate of growth.
PET-CT examination provides additional information on the metabolic activity of the lesions, and MRI is helpful in assessment of
local invasion of surrounding structures. Due to limited availability and highly specialized character, these examinations are not
routinely used. Therefore, despite development of other imaging modalities, computed tomography remains the most important
and crucial diagnostic tool.
Clinical risk factors such as age or smoking status are very important for evaluation of the likelihood of malignancy of a nodular
lesion. Due to the multidisciplinary nature of data required for complex assessment of a solitary nodular lesion, management
routines are needed in the diagnostic process such as those proposed by the Fleischner Society
The impact of timeliness of care on survival in non-small cell lung cancer patients
Wstęp: Nie jest w pełni ustalonym, na ile opóźnienie diagnostyki i leczenia niedrobnokomórkowego raka płuca (NDRP) ma
wpływ na przeżycie chorych. Celem badania było ustalenie wpływu opóźnienia zależnego od chorego i zależnego od lekarza
na przeżycie chorych na NDRP w niewyselekcjonowanej populacji.
Materiał i metody: W okresie od 1995 do 1998 roku 8705 chorych na raka płaskonabłonkowego i 1881 chorych na raka
gruczołowego płuca było zarejestrowanych w Rejestrze Instytutu Gruźlicy i Chorób Płuc.
Wyniki: Mediana czasu pomiędzy wystąpieniem pierwszych objawów a rozpoczęciem leczenia wynosiła 92 dni (śr. 138,5
dnia). Mediana czasu pomiędzy wystąpieniem pierwszych objawów a zgłoszeniem się do lekarza opieki podstawowej
wynosiła 30 dni (śr. 57 dni), a mediana czasu pomiędzy zgłoszeniem się do lekarza a wizytą u specjalisty chorób płuc —
17 dni (śr. 41 dni). Rozpoznanie raka płuca ustalone było średnio w ciągu 71dni (mediana 40 dni), Czas w jakim lekarze
chorób płuc rozpoznawali raka płuca wynosił średnio 51 dni (mediana 28 dni). Analiza wieloczynnikowa wykazała, że stan
sprawności 2 w skali ECOG (HR = 1,4), 3+4 (HR = 2,23), stopień zaawansowania klinicznego nowotworu II (HR = 1,32),
III (HR = 1,41), IV (HR = 1,82) są niezależnymi czynnikami wpływającymi na przeżycie. Brak leczenia chirurgicznego był niezależnym negatywnym czynnikiem rokowniczym (HR = 3,03). Brak opóźnienia ze strony chorego wpływał pozytywnie na
rokowanie (HR = 0,88), szczególnie u chorych w stanie sprawności 0+1 w skali ECOG (HR = 0,9) oraz 3+4 (HR = 0,9).
Brak opóźnienia zależnego od lekarza znacząco negatywnie korelował z przeżyciem chorych (HR = 1,14). Szczególnie
dotyczyło to chorych w stanie sprawności 2 (HR = 1,28).
Wnioski: Brak opóźnienia ze strony chorego wpływa pozytywnie na przeżywalność chorych na NDRP. Natomiast nie
obserwowano tego typu zależności dla opóźnienia zawiązanego z procesem diagnostycznym, a wręcz przeciwnie chorzy
dłużej diagnozowani charakteryzowali się lepszą przeżywalnością.Introduction: It is uncertain whether timeliness improves clinical outcomes in lung cancer patients. The goal of the study
was to analyse the influence of patient’s and doctor’s delays on survival of unselected population of NSCLC patients.
Material and methods: From 1995 to 1998, 8705 squamous cell lung cancer patient’s and 1881 adenocarcinoma patient’s
were registered in Pulmonary Outpatients Clinics in all parts of Poland and subsequently in National Tuberculosis and Lung
Diseases Research Institute Register (NTLDRIR).
Results: The median time from first symptom(s) to the beginning of a treatment was 92 days (mean —138.5 days).The median
waiting time between first symptom(s) and first visit to a doctor’s was 30 days (mean 57 days) and from first visit to a doctor’s to
referral to a chest physician — was 17 days (mean 41days). Diagnosis of the NSCLC was established in a mean time of 71 days
(median 40 days), but chest physician diagnosed patients in a mean time of 51days (median 28 days).
The multivariate analysis revealed that ECOG performance status (PS) 2 (HR = 1.4) and 3+4 (HR = 2.23), clinical stage of the
disease II (HR = 1.32), III (HR = 1.41), and IV (HR = 1.82) were independent negative predictors of survival. Non-surgically treated
patients had worse prognosis than patients treated surgically (HR = 3.03). Lack of patient’s delay had a significant positive impact on
survival (HR = 0.88), particularly for patients in PS 0+1 (HR = 0.9) and 3+4 (HR = 0.9). Lack of doctor’s delay was a negative
predictive factor of survival (HR = 1.14). It was observed particularly in patients in performance status 2 (HR = 1.28).
Conclusions: The patient’s delay and lack of doctor’s delay had a negative impact on survival of NSCLC patients
Epidemiological situation of tuberculosis in Poland: Part II. What are the causes of the different epidemiological situation in various regions of Poland?
Introduction: The different epidemiological situation of tuberculosis in various regions of Poland (higher and lower notification rates) was described previously by our group. The patients diagnosed with tuberculosis in the higher notification rate areas were younger and there were more cases of primary tuberculosis (tuberculous pleurisy and tuberculosis of chest lymph nodes) than in the patients diagnosed in the lower notification areas. The aim of the present study was to assess the possible causes of the different epidemiological situation of tuberculosis in various regions of Poland.Material and methods: Analysis was done at the same regions as in the previous paper. A comparison was made between two groups: Group I, which included three voivodeships with higher rates of notification, from 23.7 to 32.3/100,000 (mean rates in the analysed period of time); and Group II, which included five voivodeships with lower notification rates (mean rates from 12.2 to 18.6/100,000). The wealth of the regions (GDP, gross domestic product per capita), the level of unemployment, and social status of the patients were analysed. We compared the population density in both regions. The results of treatment in both regions were also analysed.Results: We did not find any differences in GDP and unemployment rates between the compared regions. The results of treatment were different in particular regions, but there was no clear tendency for worse results in voivodeships in Group I compared to voivodeships in Group II. However, the number of patients lost from observation was significantly higher in the regions from Group I than in those from Group II. There was also a significantly higher death rate from tuberculosis in younger patients (£ 59 years) from Group I than from Group II. This is additional proof that the epidemiological situation in the two regions was different. Finally, we found that the mean density of population in the regions from Group I was higher than that from Group II. The density of population may influence transmission of tuberculosis. There is also the possibility that the differences in the epidemiological situation in various regions of Poland are caused by historical events. In the past the epidemiological situation of tuberculosis was much worse in the east of Europe than in the west. Just after the Second World War, according to the changes of the Polish territory, many Polish citizens (mainly ancestors of those from Group I) were displaced from the east to the west.Conclusions: In conclusion, the greater number of patients lost from observation, together with the higher density of population in the regions from Group I in comparison with those from Group II, seems to be partly responsible for the difference in the epidemiological situation in the two regions. It is also possible that some patients from Group I are more susceptible to infection and disease caused by Mycobacterium tuberculosis due to their ancestors, who lived in the east of Europe.Wstęp: W pierwszej części pracy przedstawiono różnice sytuacji epidemiologicznej gruźlicy w regionach Polski o wysokich i niskich wskaźnikach zapadalności. Chorzy na gruźlicę w województwach o wyższej zapadalności byli młodsi, częstsze też były w tych województwach przypadki gruźlicy pierwotnej (gruźlicze zapalenie opłucnej i gruźlica węzłów chłonnych klatki piersiowej) w porównaniu z województwami o niższej zapadalności. Celem przedstawianej, drugiej części, pracy była ocena prawdopodobnych przyczyn różnej sytuacji epidemiologicznej gruźlicy w różnych regionach Polski.Materiał i metody: Analiza dotyczyła tych samych regionów, co w poprzedniej części pracy. Porównano 3 województwa z wyższymi wskaźnikami zapadalności (średnio w ciągu 3 lat od 23,7 do 32 na 100 000) — grupa I i 5 województw z niższymi wskaźnikami zapadalności (średnio od 12,2 do 18,6 na 100 000) — grupa II. Oceniono zamożność obu grup województw mierzoną wartością produktu krajowego brutto (PKB) przypadającą na mieszkańca, poziomem bezrobocia i statusem społecznym chorych. Porównano zagęszczenie ludności w obu regionach, oceniono wyniki leczenia.Wyniki: Nie wykazano różnic w profilu PKB i poziomie bezrobocia w obu grupach. Wyniki leczenia różniły się w poszczególnych województwach, ale przy rozpatrywaniu wszystkich województw łącznie nie było wyraźnej tendencji do wyników gorszych w grupie I w porównaniu z grupą II. Odsetek chorych, których wyniki leczenia nie były znane, był jednak istotnie większy w województwach grupy I. Istotnie większy był także w województwach grupy I odsetek zgonów z powodu gruźlicy osób w wieku do 59 lat, co stanowi kolejny dowód, że sytuacja epidemiologiczna gruźlicy w obu regionach była rzeczywiście różna. Na koniec stwierdzono, że zagęszczenie ludności w województwach grupy I było większe niż w grupie II. Wydaje się, że zagęszczenie ludności pełni znaczącą rolę w transmisji zakażenia prątkiem gruźlicy. Jest jednak możliwe, że na różnice w epidemiologii gruźlicy w obu porównywanych regionach mają również wpływ wydarzenia historyczne. Sytuacja epidemiologiczna gruźlicy była znacznie gorsza na wschodzie niż na zachodzie Europy. Bezpośrednio po zakończeniu II wojny światowej, zgodnie ze zmianą polskich granic, duża część populacji (obecnie zamieszkująca częściowo rejon województw grupy I) została przeniesiona ze wschodu na zachód.Wnioski: W konkluzji można stwierdzić, że większa liczba chorych straconych z obserwacji, łącznie z większym zagęszczeniem ludności w województwach grupy I w porównaniu z województwami grupy II, mogą być przyczyną różnic sytuacji epidemiologicznej gruźlicy w obu regionach. Dodatkową rolę może odgrywać fakt, że część populacji na terenie województw grupy I pochodzi historycznie ze wschodnich regionów i stąd może być bardziej wrażliwa na zakażenie prątkiem i na zachorowanie na gruźlicę, z powodu swoich przodków, którzy żyli na wschodzie Europy
Lung cancer in Poland in 1970-2004
Wstęp: Celem pracy jest ocena aktualnej sytuacji epidemiologicznej raka płuca w Polsce oraz tendencji epidemiologicznej
w latach 1970-2004.
Materiał i metody: Przedmiotem analizy były dane dotyczące zachorowalności i umieralności z powodu raka płuca pochodzące
z Krajowego Rejestru Nowotworów Centrum Onkologii. W ocenach stosowano metody epidemiologii opisowej. Wyniki: W 2004 roku zarejestrowano w Polsce 15 741 zachorowań na raka płuca wśród mężczyzn i 4618 wśród kobiet.
Współczynnik zachorowalności u mężczyzn wyniósł 85,2/105 i był ponad 3-krotnie wyższy niż u kobiet - 23,4/105. Rak
płuca jest najczęstszym nowotworem u mężczyzn (25,2% ogółu zachorowań) i 3. najczęstszym (po raku piersi i jelita
grubego) nowotworem u kobiet (7,8% ogółu nowotworów). Zachorowalność na raka płuca zarówno u mężczyzn, jak
i u kobiet wzrasta z wiekiem, osiągając najwyższe wartości w 70.-79. roku życia. Mediana wieku zarejestrowanych chorych
obojga płci wynosi 60-69 lat. Począwszy od lat 80. ubiegłego wieku rejestrowany jest spadek zachorowalności u mężczyzn,
początkowo w młodym wieku, a w następnych latach także w średnim wieku. Podobne tendencje wystąpiły również
u kobiet, ale 15-20 lat później. W 2004 roku co 3. zgon (16 565 - 32,3% - współczynnik 89,6/105) z powodu nowotworów
u mężczyzn był wywołany rakiem płuca. U kobiet rak płuca był 2. po raku piersi przyczyną zgonów z powodu nowotworów
(4641 zgonów - współczynnik 23,6/105). Zarówno w przypadku umieralności, jak i zachorowalności z powodu raka płuca
zarejestrowano w analizowanym okresie podobne tendencje.
Wnioski: W ostatniej dekadzie nastąpiły zahamowania tempa wzrostu, a nawet spadek zachorowalności i umieralności na
raka płuca wśród mężczyzn. Utrzymuje się nadal wzrost zachorowalności i umieralności z powodu raka płuca u kobiet.
Jednak w ostatnich latach tempo wzrostu osłabło. Poprawiła się struktura wieku zachorowań i umieralności, zahamowany
został wzrost tych współczynników wśród osób młodych i w średnim wieku, utrzymuje się on nadal w wieku podeszłym.
Mimo tych oznak poprawy, rak płuca jest nadal najczęściej występującym rakiem u mężczyzn i 3. co do częstości rakiem u kobiet.Introduction: The aim of this paper was to evaluate time trends of incidence and mortality from lung cancer by sex and age
in Poland since 1970 to 2004.
Material and methods: Basing on the data from National Cancer Registry - incidence and mortality due to lung cancer in
Poland 2004 and time trends in the period 1970-2004 were analyzed.
Results: The were 20 359 cases of lung cancer in Poland in 2004: 15 741 cases in men (crude rate - 85,2/105) and 4618
cases in women (crude rate - 23,8/105). Sex ratio was 3,4. Since 1971 lung cancer is the most frequent cancer in males: in
2004 - 25,2% of total male cancers, in female there are less frequently - 7,8% total cancers and took third place after
breast cancer - 20,5% and colorectal cancer - 10,4%. Incidence rate of lung cancer in males peaked in 1996 - 89,1/105
and then stabilized or event slightly decreased while corresponding rates in women steadily increased in whole period
1970-2004. The incidence of lung cancer for both men and women showed an increase with age and reached peak both in
elderly men and women (age group 70-79) respectively - 516,9/105 in males and 81,2/105 in females. Median age of
registered cases was as men as women in age group 60-69. In the reviewed period of time crude rate of incidence
increased in men - 231,5% in women - 390% while standardized rates increased less - 172% in men and 282% in
women. There were 16 565 deaths due lung cancer in men - crude rate 89,6/105 and 4641 deaths in women, crude rate -
23,6/105. Lung cancer is the leading cause of cancer-related mortality in men - 32,3% of all cancer deaths and took second
position in women - 12,2% (after breast cancer - 12,9%) of total cancers deaths in women.
In the analyzed periods of time - trend mortality was similar as times - trends incidence lung cancer. Both incidence and
mortality lung cancer varies by regions: incidence in males from 73,2 to 109,9; in females from 14,5 to 33,8; mortality: in
males from 73,9 to 99,8; in females from 13,0 to 30,9.
Conclusions: Lung cancer incidence and mortality rate in men has been on the stable level, even with the tendency to
decline in last decade in Poland. Incidence and mortality rates among women have continued to increase, although the trend
of increase has slowed in last years. Demographic distribution of lung cancer shows decline of the rates in young and middle
age population and continuation of increase in old age people.
Despite of some improvement of epidemiological situation in lung cancer in Poland it still remains the most common
malignancy in men and is on the third position in most frequent cancers in woman.

The epidemiological situation of tuberculosis in Poland: Part I. According to notification rates, the incidence of tuberculosis varies in different regions of Poland: is this true?
Introduction: In 2012 the incidence rate of tuberculosis in Poland was 19.6/100,000 but these was great variability between regions concerning notification rates (from 10.9/100,000 to 30.2/100,000). The aim of the study was to assess whether there are elements that might confirm that these differences are true. To answer this question, we compared the population of TB patients from regions with higher notification rates to the population of patients from regions with lower notifications rates. The data collected during three consecutive years were analysed. We selected for comparison the regions with the lowest and highest notification rates and those in which the notification rates for 3 years (2010–2012) were relatively stable.Material and methods: Eight regions were chosen: three regions (Group I) with high notification rates (from 23.7 to 32.3/100,000 — mean rates in the analysed period of time) and five (Group II) with low notification rates (mean rates from 12.2 to 18.6/100,000).Results: It was found that the proportion of sputum culture-positive patients was significantly higher in Group II. Thus, the difference in the notification rate of cases with culture-confirmed tuberculosis was smaller than the difference in the whole notification rate. Nevertheless, it was still significant. Tubercle bacilli in patients from Group I were significantly more often resistant to one drug. The incidence of chronic fibro-cavernous disease and of tuberculous pneumonia was significantly higher in Group I. The proportion of patients with symptoms was higher in Group I than in Group II. In addition, patients in Group I had the so-called primary tuberculosis (tuberculous pleuritis and tuberculous lymphadenopathy in the chest) significantly more often. It was also found that among patients from Group I there were significantly more children, more (though not significantly) youngsters and significantly fewer elderly patients.Conclusions: Based on these observations, it was concluded that there is a real difference in the epidemiological situation of tuberculosis in the selected regions of Poland with high and low rates of notification. Possible causes of this situation will be presented in a following publication.Wstęp: W 2012 roku zapadalność na gruźlicę w Polsce wynosiła 19,6/100 000, ale utrzymywały się, stwierdzane od wielu lat, znaczne różnice zapadalności między województwami (od 10,9/100 000 do 30,2/100 000). Celem pracy była ocena, czy występują zjawiska, których obecność mogłaby potwierdzić, że te różnice są faktyczne i prawdziwe. Aby odpowiedzieć na to pytanie, postanowiono porównać populację chorych na gruźlicę z regionów kraju o wyższych wskaźnikach zgłoszonych zachorowań z populacją pacjentów z regionów o wskaźnikach niższych. Analizowano dane o przypadkach zebrane przez trzy kolejne lata. Wybrano województwa o najniższej i najwyższej zapadalności i jednocześnie z względnie stabilnymi współczynnikami zapadalności w okresie 3 lat (2010–2012).Materiał i metody: Wybrano 8 województw: trzy województwa o wysokiej zapadalności (średnio od 23,7 do 32,3/100 000 średnio w analizowanych latach) (grupa I) i pięć o zapadalności niskiej (od 12,2 do 18,6/ 100 000) (grupa II).Wyniki: Stwierdzono, że odsetek pacjentów z dodatnimi wynikami posiewów plwociny był istotnie wyższy w II grupie województw. Różnice w zapadalności na gruźlicę potwierdzoną bakteriologicznie były zatem mniejsze, choć nadal istotne, niż różnice całkowitej zapadalności. Odsetek chorych z opornością prątków na jeden lek był istotnie wyższy w I grupie województw niż w II. Udział gruźlicy włóknisto-jamistej i gruźliczego zapalenia płuc był istotnie wyższy w grupie I. Większy też był w tej grupie odsetek chorych z objawami gruźlicy. Ponadto, w grupie I występowały znamiennie częściej cechy gruźlicy pierwotnej (gruźlicze zapalenie opłucnej i gruźlica węzłów chłonnych klatki piersiowej). Stwierdzono również, że wśród chorych z województw I grupy było istotnie więcej dzieci, więcej, choć nieistotnie, młodzieży i istotnie mniej chorych w wieku podeszłym.Wnioski: Przedstawione dane pozwalają na wniosek, że między regionami Polski o małej i dużej rejestrowanej zapadalności istnieją rzeczywiste różnice sytuacji epidemiologicznej gruźlicy. Możliwe przyczyny tego zjawiska zostaną przedstawione w kolejnej publikacji
Interstitial lung disease associated with docetaxel in a patient treated for breast cancer — a case report
Docetaxel is a semisynthetic cytostatic drug that belongs to the family of taxoids. Docetaxel inhibits normal interphase and mitotic cellular function, causing cell death. Docetaxel is indicated for the treatment of breast, lung and prostate cancers, head and neck cancer and gastric adenocarcinoma. Interstitial pneumonitis is an uncommon side effect of docetaxel. Difficulties in recognizing granulomatosis with polyangiitis (GPA) in elderly patients undergoing diagnostic thoracotomy twice — a report of two cases
Granulomatosis with polyangiitis (GPA) is defined as a necrotizing granulomatous inflammation usually involving the upper and lower respiratory tract with necrotizing vasculitis affecting predominantly small to medium vessels. Because of non-specific symptoms, its radiological presentation, and the diversity of its clinical expression, it is not uncommon to for it to be misdiagnosed, especially in the elderly. Although biopsy and histological examination seem to be essential for GPA diagnosis, their results are sometimes ambiguous and not helpful in making a decision. In this report, we present difficulties in the recognition of GPA in two elderly patients in whom, despite twice performing a diagnostic thoracotomy, GPA was recognized almost 4 and 6 years after the first symptoms
