    Combined biological therapy with lanreotide autogel and cabergoline in the treatment of MEN-1-related insulinomas.

    Multiple endocrine neoplasia type 1 (MEN1) is a hereditary syndrome associated with the development of many endocrine tumors, involving mainly pituitary, parathyroids, pancreas, although a proliferative state interests all neuroendocrine system. MEN1 pancreatic neuroendocrine tumors (pNETs) are multiples and can secrete different hormones. The therapeutic approach is based on surgery which usually is followed by tumor relapse or persistence unless to be highly aggressive. Biotherapy with somatostatin analogs and dopamine agonists could be of great benefit to manage these patients without altering their life quality. We report a case of a 36-year-old MEN1 man affected with multicentric pNETs associated with insulinoma syndrome. Therapy with symptomatic agents (diazoxide), as well as biotherapy (lanreotide, cabergoline) was started. At 6-month follow-up, symptomatic agents were stopped and disease control was only based on lanreotide plus cabergoline. This combined biotherapy was able to control endocrine syndromes and tumor growth. Subsequently, a safer and selective surgical intervention on pNETs was performed. An excellent response to therapy with lanreotide autogel and cabergoline has been observed in a MEN1 patient with pNETs associated with insulinoma syndrome. The potential synergistic effects of lanreotide autogel and cabergoline on insulin-secreting neuroendocrine tumors are discussed

    The HIV-1 reservoir landscape in persistent elite controllers and transient elite controllers.

    BACKGROUNDPersistent controllers (PCs) maintain antiretroviral-free HIV-1 control indefinitely over time, while transient controllers (TCs) eventually lose virological control. It is essential to characterize the quality of the HIV reservoir in terms of these phenotypes in order to identify the factors that lead to HIV progression and to open new avenues toward an HIV cure.METHODSThe characterization of HIV-1 reservoir from peripheral blood mononuclear cells was performed using next-generation sequencing techniques, such as full-length individual and matched integration site proviral sequencing (FLIP-Seq; MIP-Seq).RESULTSPCs and TCs, before losing virological control, presented significantly lower total, intact, and defective proviruses compared with those of participants on antiretroviral therapy (ART). No differences were found in total and defective proviruses between PCs and TCs. However, intact provirus levels were lower in PCs compared with TCs; indeed the intact/defective HIV-DNA ratio was significantly higher in TCs. Clonally expanded intact proviruses were found only in PCs and located in centromeric satellite DNA or zinc-finger genes, both associated with heterochromatin features.     Contribution of the Sediment Flow Connectivity Index (SfCI) in Landscape Archaeology Investigations: Test Case of a New Interdisciplinary Approach

    The integration of geomorphological analysis in archaeological investigations is essential to describe physical geography and land morphology in order to understand the relationship between the environment and human activities. Recently, the sediment flow connectivity index (SfCI) has been demonstrated to be a powerful geomorphic indicator for defining the most sensitive areas to geomorphological modifications in a catchment. This work presents the experimental application of the SfCI for a landscape archaeological analysis in order to assess the contribution of the index to potentially recognize, monitor, and interpret the historical evidence in the evaluation of landscape evolution. The investigation was performed in the basin of Lama Camaggi in the Apulia region (southern Italy), characterized by precious archaeological evidence found on the surface during field surveys in the years 2001-2002 and 2012-2013. The results show (1) the correlation between high-sediment-connectivity areas and areas with high densities of archaeological sites, and (2) the capacity of the SfCI to identify surface processes that may potentially affect the readability of the archaeological records to support data interpretation. These results confirm the advantage of applying an interdisciplinary approach in archaeology and opens innovative research scenarios
