    Body composition parameters correlate with the endoscopic severity in Crohn’s disease patients treated with infliximab

    BackgroundThe disease activity status and behavior of Crohn’s disease (CD) can reflect the severity of the disease, and changes in body composition are common in CD patients.AimsThe aim of this study was to investigate the relationship between body composition parameters and disease severity in CD patients treated with infliximab (IFX).MethodsPatients with CD assessed with the simple endoscopic score (SES-CD) and were treated with IFX were retrospectively collected, and body composition parameters at the level of the 3rd lumbar vertebrae were calculated from computed tomography (CT) scans of the patients. The correlation of patients’ body composition parameters with disease activity status and disease behavior was analyzed, and the diagnostic value of the relevant parameters was assessed using receiver operating characteristic (ROC) curves.ResultsA total of 106 patients were included in this study. There were significant differences in the subcutaneous adiposity index (SAI) (p = 0.010), the visceral adiposity index (VAI) (p < 0.001), the skeletal muscle mass index (SMI) (p < 0.001), and decreased skeletal muscle mass (p < 0.001) among patients with different activity status. After Spearman and multivariate regression analysis, SAI (p = 0.006 and p = 0.001), VAI (p < 0.001 and p < 0.001), and SMI (p < 0.001and p = 0.007) were identified as independent correlates of disease activity status (both disease activity and moderate-to-severe activity), with disease activity status independently positively correlated with SAI and SMI and independently negatively correlated with VAI. In determining the disease activity and moderate-to-severe activity status, SMI performed best relative to SAI and VAI, with areas under the ROC curve of 0.865 and 0.801, respectively. SAI (p = 0.015), SMI (p = 0.011) and decreased skeletal muscle mass (p = 0.027) were significantly different between different disease behavior groups (inflammatory disease behavior group, complex disease behavior group) but were not independent correlates (p > 0.05).ConclusionBody composition parameters of CD patients treated with IFX correlate with the endoscopic disease severity, and SMI can be used as a reliable indicator of disease activity status

    Diagnosis of benign and malignant thyroid nodules by a dual-layer spectral detector CT-based nomogram

    IntroductionPreoperative diagnosis of benign and malignant thyroid nodules is crucial for appropriate clinical treatment and individual patient management. In this study, a double-layer spectral detector computed tomography (DLCT)-based nomogram for the preoperative classification of benign and malignant thyroid nodules was developed and tested. MethodsA total of 405 patients with pathological findings of thyroid nodules who underwent DLCT preoperatively were retrospectively recruited. They were randomized into a training cohort (n=283) and a test cohort (n=122). Information on clinical features, qualitative imaging features and quantitative DLCT parameters was collected. Univariate and multifactorial logistic regression analyses were used to screen independent predictors of benign and malignant nodules. A nomogram model based on the independent predictors was developed to make individualized predictions of benign and malignant thyroid nodules. Model performance was evaluated by calculating the area under the receiver operating characteristic curve (AUC), calibration curve and decision curve analysis(DCA). ResultsStandardized iodine concentration in the arterial phase, the slope of the spectral hounsfield unit(HU) curves in the arterial phase, and cystic degeneration were identified as independent predictors of benign and malignant thyroid nodules. After combining these three metrics, the proposed nomogram was diagnostically effective, with AUC values of 0.880 for the training cohort and 0.884 for the test cohort. The nomogram showed a better fit (all p > 0.05 by Hosmer−Lemeshow test) and provided a greater net benefit than the simple standard strategy within a large range of threshold probabilities in both cohorts. DiscussionThe DLCT-based nomogram has great potential for the preoperative prediction of benign and malignant thyroid nodules. This nomogram can be used as a simple, noninvasive, and effective tool for the individualized risk assessment of benign and malignant thyroid nodules, helping clinicians make appropriate treatment decisions

    Experimental Study on Vortex-Induced Vibration of Tension Leg and Riser for Full Depth Mooring Tension Leg Platform

    According to the geometric parameters of the tension leg platform, the test model was made with a scale ratio of 1:61. The model was used to conduct the full-depth simulation test of uniform flow and wave current combination in the test pool. The model test results showed that when the reduced speed was between 5.5 and 8.5, and the lateral motion response of the platform was the most significant. In the interval of the reduced speed, the response frequency of transverse vortex-induced motion was close to the natural transverse frequency of the platform, and resonance occurred. The amplitude of surge motion increased with the increase of reduced speed. Due to the pull of the floating body, the tension of tension legs and risers increased with the flow rate but did not increase significantly in the floating body lock zone. The mooring tension had a certain limiting effect on the floating body sway. The displacement modes of tension legs and risers were greatly affected by the flow velocity. With the flow velocity increasing, the mode order increased. In addition, the increase in tension caused by the large displacement of the floating body had a certain impact on the displacement amplitude. The wave could reduce the sway of the floating body and strengthen the surge. Therefore, under the combined action of wave and current, the tension amplitude of the tension leg and riser was increased compared with that under the uniform flow. The conclusions obtained in this paper could be used for reference in the engineering design of tension legs and risers
