    Síntesis Analítica sobre las Bondades Medicinales de la Jícama (Smallanthus Sonchifolius) 2015: Analytical summary on medical benefits of jicama (Smallanthus Sonchifolius ) 2015

    Las propiedades medicinales que se les atribuye a la Jícama o Yacón deben ser sustentadas científicamente, al tratarse de una especie con efectos beneficiosos para la salud. Smallanthus Sonchifolius, planta de la familia Asteraceae del grupo de raíces y tubérculos andinos. Cultivada en pequeñas cantidades en huertas familiares y desconocida para la mayoría de la población en Imbabura. El objetivo principal es Sintetizar analíticamente las bondades medicinales atribuidas a la Jícama. Investigación de diseño cuali-cuantitativa, no experimental, de tipo descriptiva transversal y documental realizado a través de Buscadores, Sitios Web confiables, en investigaciones realizadas entre los años 2005 a 2015. La información es recopilada en una ficha diseñada en Microsoft Excel, para su tabulación y elaboración de tablas y gráficos. Como resultados se mencionan 168 fuentes bibliográficas confiables, el mayor tipo de información es internacional, publicada en revistas científicas de países Americanos como: Argentina 20,59%, Brasil 17,65% y Ecuador 13,24%. La raíz es la parte más utilizada con el 85,53%, seguido de las hojas 11,84% y semillas 2,63%. Se reconocen efectos como hipoglicemiantes 25%, digestivos 21,05%, antioxidante 7,89%; para la obesidad 7,89% y otros. La información obtenida durante los últimos 10 años permitió reconocer en hojas y raíces tuberosas de la jícama el contenido fructooligosacáridos que benefician a la salud con efectivos efectos hipoglisemiante debido a su alto contenido de inulina que se almacena en raíces y hojas; y prebióticos debido a que se trata de un alimento que mantiene equilibrio de la flora bacteriana

    Porcine Escherichia coli isolates resistant to critically important antimicrobials for public health

    El objetivo del trabajo fue establecer el comportamiento de aislamientos de Escherichia coli (E. coli) resistentes a cefotaxima (CTX) y/o colistina, obtenidos de materia fecal de cerdos de la República Argentina, frente a antimicrobianos de importancia crítica para la salud pública. En 2018, se procesaron 56 muestras de 11 granjas localizadas en las provincias de Buenos Aires, Córdoba, Santa Fe y San Luis. Se obtuvieron 31 aislamientos resistentes a cefotaxima, 21 a colistina y 23 a ambos antimicrobianos. El 69 % de los aislamientos fue resistente a ácido nalidíxico y el 37,3 % a ciprofloxacina, antimicrobianos de máxima prioridad. No se aislaron E. coli resistentes a carbapenemes. Setenta y tres aislamientos se comportaron como multirresistentes, de los cuales el 50 % mostró resistencia frente a seis o siete grupos de antimicrobianos. La resistencia a cefalosporinas de tercera generación se relacionó principalmente con el gen blaCTX-M, particularmente, blaCTX-M-8/25, mientras que la resistencia a colistina se asoció con el gen mcr-1. De los resultados se desprende que existen cerdos en la República Argentina que portan E. coli con resistencia a múltiples antimicrobianos, incluyendo los de importancia crítica para medicina humana y veterinaria. Otros estudios complementarios permitirán determinar los clones de E. coli productores de ß-lactamasas de espectro extendido circulantes en granjas porcinas y su implicancia sobre la salud pública.The goal of this study was to establish the behavior of cefotaxime (CTX)-resistant Escherichia coli (E. coli) and/or colistin-resistant E. coli, isolated from fecal samples of pigs in Argentina to other antimicrobials classified by the WHO as critically important for public health. In 2018, 56 samples from 11 farms located in Buenos Aires, Córdoba, Santa Fe, and San Luis provinces were processed. Thirty-one cefotaxime-resistant E. coli, 21 colistin-resistant E. coli, and 23 cefotaxime-colistin-resistant E. coli were obtained. Sixty-nine percent of the E. coli isolated were resistant to nalidixic acid and 37.3 % to ciprofloxacin, antimicrobials classified as the highest priority. No carbapenem-resistant E. coli were isolated. Seventy-three isolates were classified as multiresistant, of which 50 % showed resistance against 6 or 7 antimicrobial groups. Third-generation cephalosporin resistance was mainly associated with the blaCTX-M gene, particularly blaCTX-M-8/25, while colistin resistance was associated with the mcr-1 gene. the study evidence that Argentinian pigs harbor multidrug-resistant E. coli isolates, including resistance to antimicrobials of critical importance for human and veterinary medicine. Complementary analyses will be necessary to determine extended-spectrum β-lactamase-producing E. coli clones circulating in pig farms and their implications for public health.Facultad de Ciencias Veterinaria

    Stability of temocillin in outpatient parenteral antimicrobial therapy: is it a real option?

    [Background] Temocillin is an interesting alternative to carbapenems for susceptible Enterobacteriaceae. Although its use in outpatient parenteral antimicrobial therapy (OPAT) programmes has generated interest, this has been hampered by the lack of stability data.[Objectives] The purpose of the present study was to evaluate the physical and chemical stability of temocillin at the recommended dose for its use in OPAT programmes, contained in polypropylene infusion bags or polyisoprene elastomeric devices at different temperatures, and to describe a novel LC-MS/MS developed for the quantification of temocillin.[Methods] Temocillin daily dose (6 g) was diluted in 500 mL of 0.9% sodium chloride to obtain a final concentration of 12 g/L. This solution was stored at 4°C, 25°C, 32°C and 37°C for 72 h, both in polypropylene infusion bags and in polyisoprene elastomeric pumps. Physical and chemical stability were evaluated during 72 h after manufacturing. Solutions were considered stable if colour, clearness and pH remained unchanged and if the percentage of intact drug was ≥90%.[Results] Temocillin attained the chemical stability criterion of ≥90% of the original concentration for the whole experiment in both devices at 4°C, 25°C and 32°C. At 37°C, temocillin was stable for 24 h but its concentration dropped below 90% from that timepoint. No precipitation occurred and minor colour changes were observed.[Conclusions] Temocillin is stable under OPAT conditions and it would be an appropriate candidate for the treatment of patients who can be discharged to complete therapy in an OPAT programme.     Anti-Spike antibodies 3 months after SARS-CoV-2 mRNA vaccine booster dose in patients on hemodialysis: the prospective SENCOVAC study

    Background: Patients on hemodialysis are at high-risk for complications derived from coronavirus disease 2019 (COVID-19). The present analysis evaluated the impact of a booster vaccine dose and breakthrough severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections on humoral immunity 3 months after the booster dose. Methods: This is a multicentric and prospective study assessing immunoglobulin G anti-Spike antibodies 6 and 9 months after initial SARS-CoV-2 vaccination in patients on hemodialysis that had also received a booster dose before the 6-month assessment (early booster) or between the 6- and 9-month assessments (late booster). The impact of breakthrough infections, type of vaccine, time from the booster and clinical variables were assessed. Results: A total of 711 patients [67% male, median age (range) 67 (20-89) years] were included. Of these, 545 (77%) received an early booster and the rest a late booster. At 6 months, 64 (9%) patients had negative anti-Spike antibody titers (3% of early booster and 29% of late booster patients, P =. 001). At 9 months, 91% of patients with 6-month negative response had seroconverted and there were no differences in residual prevalence of negative humoral response between early and late booster patients (0.9% vs 0.6%, P =. 693). During follow-up, 35 patients (5%) developed breakthrough SARS-CoV-2 infection. Antibody titers at 9 months were independently associated with mRNA-1273 booster (P =. 001), lower time from booster (P =. 043) and past breakthrough SARS-CoV-2 infection (P <. 001).     Role of age and comorbidities in mortality of patients with infective endocarditis

    [Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality. [Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65 years,65 to 80 years,and ≥ 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk. [Results]: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 ≥ 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients ≥80 years who underwent surgery were significantly lower compared with other age groups (14.3%,65 years; 20.5%,65-79 years; 31.3%,≥80 years). In-hospital mortality was lower in the <65-year group (20.3%,<65 years;30.1%,65-79 years;34.7%,≥80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%,≥80 years; p = 0.003).Independent predictors of mortality were age ≥ 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI ≥ 3 (HR:1.62; 95% CI:1.39–1.88),and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality. [Conclusion]: There were no differences in the clinical presentation of IE between the groups. Age ≥ 80 years, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group
