    Classical solutions for a logarithmic fractional diffusion equation

    We prove global existence and uniqueness of strong solutions to the logarithmic porous medium type equation with fractional diffusion ?tu + (?)1/2 log(1 + u) = 0, posed for x ? R, with nonnegative initial data in some function space of LlogL type. The solutions are shown to become bounded and C? smooth in (x, t) for all positive times. We also reformulate this equation as a transport equation with nonlocal velocity and critical viscosity, a topic of current relevance. Interesting functional inequalities are involved

    Decreased incidence of pressure ulcers in intensive care: a program goal of improving care

    INTRODUCCIÓN. En sanidad, la aparición de úlceras por presión (upp), es considerado un fracaso asistencial y un gran reto al que se enfrenta el profesional de Enfermería. En cuidados intensivos, objetivamos una serie de factores que aumentan el riesgo de aparición y desarrollo de upp. Como enfermeros, tomamos conciencia de nuestro papel en la prevención y tratamiento de las upp en nuestra unidad de cuidados intensivos (UCI-1, Hospital Universitario Central de Asturias) y emprendimos medidas para disminuirlas y mejorar la calidad asistencial proporcionada. OBJETIVOS. Aumentar la calidad asistencial Disminuir las tasas de úlceras por presión MATERIAL Y MÉTODO. Estudio descriptivo, prospectivo. Muestra, N: 1265 enfermos. Realizándose seguimiento diario de enfermos a través de hoja de valoración específica. Los enfermos valorados presentaban riesgo medio- alto de padecer upp por escala Braden y riesgo bajo por dicha escala pero con condiciones particulares de riesgo objetivables. Se introducen medidas preventivas consensuadas por el equipo investigador (descritas en el trabajo de investigación). RESULTADOS. Bajada significativa de las tasas de incidencia a los cuatro meses de introducir medidas estandarizadas de prevención y mejora asistencial. CONCLUSIONES. La concienciación y unificación de criterios preventivos y de tratamiento resultó ser decisiva en la bajada significativa de la incidencia de upp y en el logro de aumento de la calidad asistencial real percibida por todo el equipo interdisciplinar.INTRODUCTION. In health, the occurrence of pressure ulcers (PU) is considered a failure of care and major challenge facing the nursing professional. In intensive care, objectify a series of factors that increase the risk of occurrence and development of PU. As nurses, we realize our role in the prevention and treatment of pressure ulcers in our intensive care unit (ICU-1, Hospital Universitario Central de Asturias) and we took measures to reduce and improve quality of care provided. OBJECTIVES. To augment quality of care To diminish rates of pressure ulcers MATERIALS AND METHODS. Prospective descriptive study. Sample, n = 1265 patients. Performing daily monitoring of patients through specific assessment sheet. The patients had rated medium-high risk of developing upp by Braden scale and low risk for this scale but with risk conditions to measure. Agreed preventive measures are introduced by the research team (described in the research). RESULTS. Llower incidence rates for four months to introduce standardized measures of prevention and improved care. CONCLUSIONS. The unification of awareness and prevention and treatment criteria proved to be instrumental in the significant decline in the incidence of PU and the achievement of increased quality of care received by all real interdisciplinary team

    Influence of the Cumulative Incidence of COVID-19 Cases on the Mental Health of the Spanish Out-of-Hospital Professionals

    This study aimed to analyze the psychological affectation of health professionals (HPs) of Spanish Emergency Medical Services (EMSs) according to the cumulative incidence (CI) of COVID19 cases in the regions in which they worked. A cross-sectional descriptive study was designed, including all HPs working in any EMS of the Spanish geography between 1 February 2021 and 30 April 2021. Their level of stress, anxiety and depression (DASS-21) and the perception of self-efficacy (GSES) were the study’s main results. A 2-factor analysis of covariance was used to determine if the CI regions of COVID-19 cases determined the psychological impact on each of the studied variables. A total of 1710 HPs were included. A third presented psychological impairment classified as severe. The interaction of CI regions with the studied variables did not influence their levels of stress, anxiety, depression or self-efficacy. Women, younger HPs or those with less EMS work experience, emergency medical technicians (EMT), workers who had to modify their working conditions or those who lived with minors or dependents suffered a greater impact from the COVID-19 pandemic in certain regions. These HPs have shown high levels of stress, anxiety, depression and medium levels of self-efficacy, with similar data in the different geographical areas. Psychological support is essential to mitigate their suffering and teach them to react to adverse events.This research was funded by Fundación ASISA and Sociedad Española de Urgencias y Emergencias (SEMES)

    Chromatin regulation by Histone H4 acetylation at Lysine 16 during cell death and differentiation in the myeloid compartment

    Histone H4 acetylation at Lysine 16 (H4K16ac) is a key epigenetic mark involved in gene regulation, DNA repair and chromatin remodeling, and though it is known to be essential for embryonic development, its role during adult life is still poorly understood. Here we show that this lysine is massively hyperacetylated in peripheral neutrophils. Genome-wide mapping of H4K16ac in terminally differentiated blood cells, along with functional experiments, supported a role for this histone post-translational modification in the regulation of cell differentiation and apoptosis in the hematopoietic system. Furthermore, in neutrophils, H4K16ac was enriched at specific DNA repeats. These DNA regions presented an accessible chromatin conformation and were associated with the cleavage sites that generate the 50 kb DNA fragments during the first stages of programmed cell death. Our results thus suggest that H4K16ac plays a dual role in myeloid cells as it not only regulates differentiation and apoptosis, but it also exhibits a non-canonical structural role in poising chromatin for cleavage at an early stage of neutrophil cell death

    A randomized, double-blind study on the efficacy of oral domperidone versus placebo for reducing SARS-CoV-2 viral load in mild-to-moderate COVID-19 patients in primary health care

    15 p.-3 fig.-3 tab.Introduction:The clinical effect of domperidone against COVID-19 has been investigated in a double-blind phase III clinical trial (EudraCT number 2021-001228-17). Domperidone has shown in vitro antiviral activity against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and potential immudolatory properties through the stimulation of prolactin secretion.Patients and methods:The efficacy of oral domperidone plus standard of care (SOC; n = 87) versus placebo plus SOC (n = 86) was evaluated in a 28-day randomized double-blind multicentre study in primary health care centres. A total of 173 outpatients with mild-to-moderate COVID-19 were included. Three daily doses of 10 mg (30 mg/day) of domperidone or placebo were administered for 7 days. Reduction of viral load on day 4 was the primary efficay endpoint. It was estimated in saliva samples by reverse transcription-quantitative polymerase chain reaction (RT-qPCR), as the cycle thresholds detected ORF1ab, N Protein and S Protein genes.Results:A significant reduction in the viral load was observed (p < 0.001) from baseline to days 4, 7 and 14 of the three genes studied with non-significant differences between domperidone and placebo groups. Twenty-three patients (13.3%) experienced adverse events, 14 patients in the domperidone group (16.1%) and 9 patients in the placebo group (10.5%). No patients needed to be hospitalized.Conclusion: Results do not prove the use of domperidone as antiviral in patients with COVID-19.This research was funded by CSIC (grant no.     Heritage and the Sea: Maritime History and Archaeology of the Global Iberian World (15th -18th centuries)

    This two-volume set highlights the importance of Iberian shipbuilding in the centuries of the so-called first globalization (15th to 18th), in confluence with an unprecedented extension of ocean navigation and seafaring and a greater demand for natural resources (especially timber), mostly oak (Quercus spp.) and Pine (Pinus spp.). The chapters are framed in a multidisciplinary and interdisciplinary line of research that integrates history, Geographic Information Sciences, underwater archaeology, dendrochronology and wood provenance techniques. This line of research was developed during the ForSEAdiscovery project, which had a great impact in the academic and scientific world and brought together experts from Europe and America. The volumes deliver a state-of-the-art review of the latest lines of research related to Iberian maritime history and archaeology and their developing interdisciplinary interaction with dendroarchaeology. This synthesis combines an analysis of historical sources, the systematic study of wreck-remains and material culture related to Iberian seafaring from the 15th to the 18th centuries, and the application of earth sciences, including dendrochronology.     Heterogeneous contributions of change in population distribution of body mass index to change in obesity and underweight NCD Risk Factor Collaboration (NCD-RisC)

    From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions

Artikulu honetan laburbildu egin nahi ditugu EEI-EEMen eraginen inguruko ikerketa esperimentalen eta epidemiologikoen bidez lortu diren emaitzak. Nahiz eta ikerketa eta proiektu ugari egon gaiaren inguruan, kontu ugari daude argitzeke, batez ere metodologia-mugek sortzen dituzten arazoek ez dutelako ondorio argirik ateratzeko aukera ematen. Aurrerantzean, ahalegin handiagoak egin beharko lirateke eremu elektromagnetikoen esposizioaren estimazio egokiagoa egite aldera.; Sources of electromagnetic fields of non-ionising radiation (EMF-NIR) are increasingly common in our life style, while discussion regards their potential health effects remains unclear. The aim of this study is to collect and summarize experimental and epidemiological studies published concerning health effects of EMF-NIR. Although there are plenty studies and projects on this subject, there are several unclear issues, especially because problems arising from methodological limitations preclude getting a clear conclusion. Greater effort should be made in the future in order to estimate better the exposure
