    Evoluzione delle attivita di Delivery in relazione all'applicazione del Jobs Act

    Obiettivo del presente lavoro Ăš analizzare l’evoluzione delle principali attivitĂ  svolte all’interno delle Agenzie per il lavoro in relazione all’applicazione del Jobs Act. Nel primo capitolo mi sono occupata del Jobs Act e delle sue caratteristiche. Ho effettuatato primariamente un excursus storico dei cambiamenti normativi connessi al mondo del lavoro nel contesto italiano. Questa analisi Ăš stata effettuata per mettere in evidenza come tali cambiamenti non siano altro che il frutto dell’interazione tra il contesto storico, politico, religioso e sociale caratteristico del nostro Paese. Successivamente, ho spostato l’attenzione sui vari decreti legislativi che compongono la legge n. 183 del 2014. Da questa analisi, Ăš emerso chiaramente come il Jobs Act sia riuscito a farsi strada in un momento in cui il problema del mercato del lavoro non poteva piĂč essere accantonato e necessitava di soluzioni immediate. Dopo aver presentato un quadro generale delle innovazioni apportate nel mondo del lavoro con l’introduzione del Jobs Act, ho trattato in maniera piĂč dettagliata il tema delle politiche attive previste da questo provvedimento. In relazione ad esse, possiamo dire che l’obiettivo principale del Governo Renzi sia stato quello di cercare di facilitare maggiormente l’incontro tra domanda e offerta di lavoro. Garantire servizi di orientamento e formativi per avvicinare i giovani al mondo del lavoro e percorsi di ricollocazione rivolti a coloro che, avendo perso la propria occupazione, cercano di reiserirsi. Per riuscire a fornire un risvolto pratico a queste iniziative, ho fatto riferimento alle Agenzie per il lavoro, che negli ultimi anni hanno ricoperto un ruolo sempre piĂč centrale nel mercato del lavoro in relazione al tema dell’occupazione e del reinserimento lavorativo. Per parlarne accuratamente, Ăš risultato necessario dedicare preliminarmente attenzione al tipo di rapporto che si instaura con l’introduzione dell’intermediazione lavorativa. Ci riferiamo qui ai contratti di lavoro in somministrazione. Nello specifico ho voluto analizzare il tipo di relazione che intercorre tra tre figure protagoniste (l’impresa somministratrice, il lavoratore e all’impresa utilizzatrice) e le modalitĂ  di concretizzazione del rapporto di lavoro tra di esse. Avendo dunque evidenziato il ruolo centrale delle Agenzie per il lavoro, nel quarto capitolo, ho analizzato dettagliatamente il caso concreto dell’Agenzia per il lavoro Gi Group. L’obiettivo Ăš stato quello di esplicare il funzionamento di questa importantissima multinazionale, presentando la Mission aziendale, i valori e i principi etici di condotta condivisi al suo interno. Questa analisi ha permesso di rendere ancora piĂč concreta l’idea di considerare Gi Group come una realtĂ  fortemente strutturata e orientata ad offrire servizi di elevata qualitĂ  volti allo sviluppo del mercato del lavoro. Nel quinto capitolo infine, ho fornito un esempio concreto di come il Jobs Act abbia condizionato il modo di operare all’interno delle Agenzie per il lavoro. Ho presentato ed esplicato in che cosa consiste l’attivitĂ  di Delivery all’interno di Gi Group. Con il termine Delivery, si identificano l’insieme delle attivitĂ  che vanno dal processo di ricerca e selezione del personale, alle attivitĂ  amministrative cosiddette “soft”. AttivitĂ  dunque in cui la volontĂ  di avvicinare la domanda e l’offerta di lavoro viene vista quotidianamente e in cui si cerca di guidare le persone verso una crescita personale e professionale. Per concludere, possiamo dire che questa analisi Ăš stata compiuta per fornire un quadro d’insieme sui diversi cambiamenti che l’avvento del Jobs Act ha comportato, cambiamenti che hanno indubbiamente modificato il mondo del lavoro in Italia. Inoltre, Ăš stato scelto il tema della Agenzie per il lavoro, proprio perchĂ© questa realtĂ  manifesta in maniera lampante i significativi cambiamenti che la nuova normativa ha comportato. Esse hanno progressivamente accresciuto il loro ruolo nel mercato, favorendo sempre piĂč lo sviluppo e la crescita del mercato del lavoro. Hanno progressivamente acquisito un ruolo centrale anche in relazione alle politiche attive, occupandosi dell’orientamento, della formazione e della ricollocazione delle persone nel mondo del lavoro

    The GATA1s isoform is normally down-regulated during terminal haematopoietic differentiation and over-expression leads to failure to repress MYB, CCND2 and SKI during erythroid differentiation of K562 cells

    Background: Although GATA1 is one of the most extensively studied haematopoietic transcription factors little is currently known about the physiological functions of its naturally occurring isoforms GATA1s and GATA1FL in humans—particularly whether the isoforms have distinct roles in different lineages and whether they have non-redundant roles in haematopoietic differentiation. As well as being of general interest to understanding of haematopoiesis, GATA1 isoform biology is important for children with Down syndrome associated acute megakaryoblastic leukaemia (DS-AMKL) where GATA1FL mutations are an essential driver for disease pathogenesis. <p/>Methods: Human primary cells and cell lines were analyzed using GATA1 isoform specific PCR. K562 cells expressing GATA1s or GATA1FL transgenes were used to model the effects of the two isoforms on in vitro haematopoietic differentiation. <p/>Results: We found no evidence for lineage specific use of GATA1 isoforms; however GATA1s transcripts, but not GATA1FL transcripts, are down-regulated during in vitro induction of terminal megakaryocytic and erythroid differentiation in the cell line K562. In addition, transgenic K562-GATA1s and K562-GATA1FL cells have distinct gene expression profiles both in steady state and during terminal erythroid differentiation, with GATA1s expression characterised by lack of repression of MYB, CCND2 and SKI. <p/>Conclusions: These findings support the theory that the GATA1s isoform plays a role in the maintenance of proliferative multipotent megakaryocyte-erythroid precursor cells and must be down-regulated prior to terminal differentiation. In addition our data suggest that SKI may be a potential therapeutic target for the treatment of children with DS-AMKL

    The Reproducibility of a Kinematically-Derived Axis of the Knee versus Digitized Anatomical Landmarks using a Knee Navigation System

    Component position is critical to longevity of knee arthroplasties. Femoral component rotation is typically referenced from the transepicondylar axis (TEA), the anterior-posterior (AP) axis or the posterior condylar axis. Other studies have shown high variability in locating the TEA while proposing digitization of other landmarks such as the AP axis as a less-variable reference. This study uses a navigation system to compare the reproducibility of computing a kinematically-derived, navigated knee axis (NKA) to digitizing the TEA and AP axis. Twelve knees from unembalmed cadavers were tested. Four arthroplasty surgeons digitized the femoral epicondyles and the AP axis direction as well as flexed and extended the knee repeatedly to allow for NKA determination. The variance of the NKA axis determined under neutral loading conditions was smaller than the variance of the TEA axis when the kinematics were measured in the closed surgical condition (P<0.001). However, varus, valgus, and internal loading of the leg increased the variability of the NKA. Distraction of the leg during knee flexion and extension preserved the low variability of the NKA. In conclusion, a kinematically-derived NKA under neutral or distraction loading is more reproducible than the TEA and AP axis determined by digitization

    Accretion of Planetary Material onto Host Stars

    Accretion of planetary material onto host stars may occur throughout a star's life. Especially prone to accretion, extrasolar planets in short-period orbits, while relatively rare, constitute a significant fraction of the known population, and these planets are subject to dynamical and atmospheric influences that can drive significant mass loss. Theoretical models frame expectations regarding the rates and extent of this planetary accretion. For instance, tidal interactions between planets and stars may drive complete orbital decay during the main sequence. Many planets that survive their stars' main sequence lifetime will still be engulfed when the host stars become red giant stars. There is some observational evidence supporting these predictions, such as a dearth of close-in planets around fast stellar rotators, which is consistent with tidal spin-up and planet accretion. There remains no clear chemical evidence for pollution of the atmospheres of main sequence or red giant stars by planetary materials, but a wealth of evidence points to active accretion by white dwarfs. In this article, we review the current understanding of accretion of planetary material, from the pre- to the post-main sequence and beyond. The review begins with the astrophysical framework for that process and then considers accretion during various phases of a host star's life, during which the details of accretion vary, and the observational evidence for accretion during these phases.Comment: 18 pages, 5 figures (with some redacted), invited revie

    Innovations in total knee replacement: new trends in operative treatment and changes in peri-operative management

    The human knee joint can sustain damage due to injury, or more usually osteoarthritis, to one, two or all three of the knee compartments: the medial femorotibial, the lateral femorotibial and the patellofemoral compartments. When pain associated with this damage is unmanageable using nonsurgical techniques, knee replacement surgery might be the most appropriate course of action. This procedure aims to restore a pain-free, fully functional and durable knee joint. Total knee replacement is a well-established treatment modality, and more recently, partial knee replacement—more commonly known as bi- or unicompartmental knee replacement—has seen resurgence in interest and popularity. Combined with the use of minimally invasive surgery (MIS) techniques, gender-specific prosthetics and computer-assisted navigation systems, orthopaedic surgeons are now able to offer patients knee replacement procedures that are associated with (1) minimal risks during and after surgery by avoiding fat embolism, reducing blood loss and minimising soft tissue disruption; (2) smaller incisions; (3) faster and less painful rehabilitation; (4) reduced hospital stay and faster return to normal activities of daily living; (5) an improved range of motion; (6) less requirement for analgesics; and (7) a durable, well-aligned, highly functional knee. With the ongoing advancements in surgical technique, medical technology and prosthesis design, knee replacement surgery is constantly evolving. This review provides a personal account of the recent innovations that have been made, with a particular emphasis on the potential use of MIS techniques combined with computer-assisted navigation systems to treat younger, more physically active patients with resurfacing partial/total implant knee arthroplasty

    Echocardiographic AV-interval optimization in patients with reduced left ventricular function

    BACKGROUND: Ritter's method is a tool used to optimize AV delay in DDD pacemaker patients with normal left ventricular function only. The goal of our study was to evaluate Ritter's method in AV delay-interval optimization in patients with reduced left ventricular function. METHODS: Patients with implanted DDD pacemakers and AVB III° were assigned to one of two groups according to ejection fraction (EF): Group 1 (EF > 35%) and Group 2 (EF < 35%). AV delay optimization was performed by means of radionuclide ventriculography (RNV) and application of Ritter's method. RESULTS: For each of the patients examined, we succeeded in defining an optimal AV interval by means of both RNV and Ritter's method. The optimal AV delay determined by RNV correlated well with the delay found by Ritter's method, especially among those patients with reduced EF. The intra-class correlation coefficient was 0.8965 in Group 1 and 0.9228 in Group 2. The optimal AV interval in Group 1 was 190 ± 28.5 ms, and 180 ± 35 ms in Group 2. CONCLUSION: Ritter's method is also effective for optimization of AV intervals among patients with reduced left ventricular function (EF < 35%). The results obtained by RNV correlate well with those from Ritter's method. Individual programming of the AV interval is fundamentally essential in all cases

    The Evolution of Compact Binary Star Systems

    We review the formation and evolution of compact binary stars consisting of white dwarfs (WDs), neutron stars (NSs), and black holes (BHs). Binary NSs and BHs are thought to be the primary astrophysical sources of gravitational waves (GWs) within the frequency band of ground-based detectors, while compact binaries of WDs are important sources of GWs at lower frequencies to be covered by space interferometers (LISA). Major uncertainties in the current understanding of properties of NSs and BHs most relevant to the GW studies are discussed, including the treatment of the natal kicks which compact stellar remnants acquire during the core collapse of massive stars and the common envelope phase of binary evolution. We discuss the coalescence rates of binary NSs and BHs and prospects for their detections, the formation and evolution of binary WDs and their observational manifestations. Special attention is given to AM CVn-stars -- compact binaries in which the Roche lobe is filled by another WD or a low-mass partially degenerate helium-star, as these stars are thought to be the best LISA verification binary GW sources.Comment: 105 pages, 18 figure

    Health service utilization in IBD: comparison of self-report and administrative data

    <p>Abstract</p> <p>Background</p> <p>The reliability of self-report regarding health care utilization in inflammatory bowel disease (IBD) is unknown. If proven reliable, it could help justify self-report as a means of determining health care utilization and associated costs.</p> <p>Methods</p> <p>The Manitoba IBD Cohort Study is a population-based longitudinal study of participants diagnosed within 7 years of enrollment. Health care utilization was assessed through standardized interview. Participants (n = 352) reported the total number of nights hospitalized, frequency of physician contacts in the prior 12 months and whether the medical contacts were for IBD-related reasons or not. Reports of recent antibiotic use were also recorded. Actual utilization was drawn from the administrative database of Manitoba Health, the single comprehensive provincial health insurer.</p> <p>Results</p> <p>According to the administrative data, 15% of respondents had an overnight hospitalization, while 10% had an IBD-related hospitalization. Self-report concordance was highly sensitive (92%; 82%) and specific (96%; 97%, respectively). 97% of participants had contact with a physician in the previous year, and 69% had IBD-related visits. Physician visits were significantly under-reported and there was a trend to over-report the number of nights in hospital.</p> <p>Conclusions</p> <p>Self-report data can be helpful in evaluating health service utilization, provided that the researcher is aware of the systematic sources of bias. Outpatient visits are well identified by self-report. The discordance for the type of outpatient visit may be either a weakness of self-report or a flaw in diagnosis coding of the administrative data. If administrative data are not available, self-report information may be a cost-effective alternative, particularly for hospitalizations.</p

    Pre-ejection period by radial artery tonometry supplements echo doppler findings during biventricular pacemaker optimization

    <p>Abstract</p> <p>Background</p> <p>Biventricular (Biv) pacemaker echo optimization has been shown to improve cardiac output however is not routinely used due to its complexity. We investigated the role of a simple method involving computerized pre-ejection time (PEP) assessment by radial artery tonometry in guiding Biv pacemaker optimization.</p> <p>Methods</p> <p>Blinded echo and radial artery tonometry were performed simultaneously in 37 patients, age 69.1 ± 12.8 years, left ventricular (LV) ejection fraction (EF) 33 ± 10%, during Biv pacemaker optimization. Effect of optimization on echo derived velocity time integral (VTI), ejection time (ET), myocardial performance index (MPI), radial artery tonometry derived PEP and echo-radial artery tonometry derived PEP/VTI and PEP/ET indices was evaluated.</p> <p>Results</p> <p>Significant improvement post optimization was achieved in LV ET (286.9 ± 37.3 to 299 ± 34.6 ms, p < 0.001), LV VTI (15.9 ± 4.8 cm to 18.4 ± 5.1 cm, p < 0.001) and MPI (0.57 ± 0.2 to 0.45 ± 0.13, p < 0.001) and in PEP (246.7 ± 36.1 ms to 234.7 ± 35.5 ms, p = 0.003), PEP/ET (0.88 ± 0.21 to 0.79 ± 0.17, p < 0.001), and PEP/VTI (17.3 ± 7 to 13.78 ± 4.7, p < 0.001). The correlation between comprehensive echo Doppler and radial artery tonometry-PEP guided optimal atrioventricular delay (AVD) and optimal interventricular delay (VVD) was 0.75 (p < 0.001) and 0.69 (p < 0.001) respectively. In 29 patients with follow up assessment, New York Heart Association (NYHA) class reduced from 2.5 ± 0.8 to 2.0 ± 0.9 (p = 0.004) at 1.8 ± 1.4 months.</p> <p>Conclusion</p> <p>An acute shortening of PEP by radial artery tonometry occurs post Biv pacemaker optimization and correlates with improvement in hemodynamics by echo Doppler and may provide a cost-efficient approach to assist with Biv pacemaker echo optimization.</p
